We present a case having CAPD admitted in our hospital that came with complaint of pain abdomen and cloudy peritoneal effluent, from which pure culture of Leclercia adecarboxylata had been isolated L. adecarboxylata is a rarely reported gram negative human pathogen, very easily misdiagnosed as Escherichia coli

    Rapid Acclimatisation to High Altitude by Intermittent Hypoxia Training at Sea Level Role of Biochemical Markers

    Rapid induction of soldiers to high altitude under emergency situation may lead to higher incidence of acute mountain sickness (AMS) and other high altitude illness. Intermittent Hypoxia Training (IHT) at sea level before going to high altitude is an approach for rapid acclimatisation. This approach may be helpful to reduce the occurrence of AMS and leads to better acclimatisation at high altitude in shorter duration. The present study evaluates the role of biochemical markers of acclimatisation after IHT before induction to actual high altitude. The study participants were Indian Army Personnel (n=30) and they were divided into two groups of control (n=16) and IHT exposed (n=14). The intermittent hypoxia training was administered at 12 per cent Oxygen for 4h/day for 4 days at sea level using normobaric hypoxia chamber and within 24 hrs - 48 hrs the subjects were airlifted to Leh, Ladakh, India at 11,700 ft. Preconditioning with IHT may be beneficial in maintaining antioxidant levels and ameliorate oxidative stress at high altitude. The hypoxia responsive proteins like Hemeoxygenase -1 (HO-1) and Vascular endothelial growth factor (VEGF) and the cytoprotective stress proteins, which facilitate the acclimatisation, may also get benefited by IHT exposure

    APO-1/Fas gene: Structural and functional characteristics in systemic lupus erythematosus and other autoimmune diseases

    Systemic lupus erythematosus (SLE) is an autoimmune disorder affecting multiple organ systems. It is characterized by the presence of autoantibodies reactive against various self-antigens. Susceptibility to SLE is found to be associated with many major histocompatibility complex (MHC) and non-MHC genes, one of which is APO-1/Fas gene, which is present on chromosome 10 in humans. The APO-1/Fas promoter contains consensus sequences for binding of several transcription factors that affect the intensity of Fas expression in cells. The mutations in the APO-1/Fas promoter are associated with risk and severity in various autoimmune diseases and other malignancies. The APO-1/Fas receptor is expressed by many cell types. Two forms of APO-1/Fas protein that are involved in regulation of apoptosis have been identified. Fas receptor-mediated apoptosis plays a physiological and pathological role in killing of infected cell targets. In this review, we have focused on APO-1/Fas gene structure, promoter variants and its association with SLE and other autoimmune diseases. Functional aspects of Fas receptor in apoptosis are also discussed
