Formulation and Characterization of Phytostanol Ester Solid Lipid Nanoparticles for the Management of Hypercholesterolemia: An ex vivo Study.
Background: Phytostanols are naturally occurring compounds that reduce blood cholesterol levels significantly. However, their aqueous insolubility poses formulation challenges. Aim: To formulate and characterize solid lipid nanoparticle carriers for phytostanol esters to enhance the bioavailability of phytostanols. Methods: Phytostanol ester solid lipid nanoparticles were formulated by the microemulsion method. They were characterized for particle size distribution, polydispersity index, shape, surface charge, entrapment efficiency, stability, chemical structure, and thermal properties. The uptake of the formulation by cell lines, HepG2 and HT-29, and its effect on cell viability were evaluated. Results: The formulation of solid lipid nanoparticles was successfully optimised by varying the type of lipids and their concentration relative to that of surfactants in the present study. The optimised formulation had an average diameter of (171 ± 9) nm, a negative surface charge of (â 23.0 ± 0.8) mV and was generally spherical in shape. We report high levels of drug entrapment at (89 ± 5)% in amorphous form, drug loading of (9.1 ± 0.5)%, nanoparticle yield of (67 ± 4)% and drug excipient compatibility. The biological safety and uptake of the formulations were demonstrated on hepatic and intestinal cell lines. Conclusion: Phytostanol ester solid lipid nanoparticles were successfully formulated and characterized. The formulation has the potential to provide an innovative drug delivery system for phytostanols which reduce cholesterol and have a potentially ideal safety profile. This can contribute to better management of one of the main risk factors of cardiovascular disease
Adverse pregnancy outcomes in sickle cell trait: a prospective cohort study evaluating clinical and haematological parameters in postpartum mothers and newborns
Background:
Sickle cell trait (SCT) is a congenital condition caused by the inheritance of a single allele of the abnormal haemoglobin beta gene, HbS. Carriers of SCT are generally asymptomatic, and they do not manifest the clinical and haematological abnormalities of sickle cell anaemia (SCA). However, there is evidence that they display some symptoms in stressful situations. Pregnancy is a stressful physiological event, and it is not clear if SCT adversely affects pregnancy outcomes, particularly in those from developing countries where people regularly suffer from nutritional insufficiency.
Objective:
This study aims to investigate pregnancy outcomes in Sudanese women with SCT.
Subjects and methods:
Pregnant women with (HbAS, n=34) and without (HbAA, n=60) SCT were recruited during their first trimester at El Obeid Hospital, Kordofan, Western Sudan. Following appropriate ethical approval and informed consent from the participants, detailed anthropometric, clinical, haematological, obstetric, and birth outcome data were registered. In addition, blood samples were collected at enrolment and at delivery.
Results:
At enrolment in the first trimester, the SCT group did not manifest SCA symptoms, and there was no difference in the haematological parameters between the SCT and control groups. However, at delivery, the women with SCT, compared with the control group, had lower levels of hemoglobin (Hb, p=0.000), packed cell volume (PCV, p=0.000), mean corpuscular haemoglobin (MCH, p=0.002) and neutrophil counts (p=0.045) and higher mean corpuscular volume (MCV, p=0.000) and platelet counts (p=0.000). Similarly, at delivery, the babies of SCT women had lower birth weight (p=0.000), lower Hb (p=0.045), PCV (p=0.000), MCH (p=0.000), and higher neutrophil (p=0.004) and platelet counts (p=0.000) than the babies of the healthy control group. Additionally, there were more miscarriages, stillbirths, and admissions to the Special Care Baby Unit (SCBU) in the SCT group.
Conclusions:
The study revealed that SCT is associated with adverse pregnancy outcomes, including maternal and neonatal anaemia, low birth weight, and increased risk of stillbirth, miscarriage, and admission to SCBU. Therefore, pregnant women with SCT should be given appropriate pre-conceptual advice and multidisciplinary antenatal and postnatal care
Interpreting household survey data intended to measure insecticide-treated bednet coverage: results from two surveys in Eritrea
BACKGROUND: As efforts are currently underway to roll-out insecticide-treated bednets (ITNs) to populations within malarious areas in Africa, there is an unprecedented need for data to measure the effectiveness of such programmes in terms of population coverage. This paper examines methodological issues to using household surveys to measure core Roll Back Malaria coverage indicators of ITN possession and use. METHODS: ITN coverage estimates within Anseba and Gash Barka Provinces from the 2002 Eritrean Demographic and Health Survey, implemented just prior to a large-scale ITN distribution programme, are compared to estimates from the same area from a sub-national Bednet Survey implemented 18 months later in 2003 after the roll-out of the ITN programme. RESULTS: Measures of bednet possession were dramatically higher in 2003 compared to 2002. In 2003, 82.2% (95% confidence interval (CI) 77.4â87.0) of households in Anseba and Gash Barka possessed at least one ITN. RBM coverage indicators for ITN use were also dramatically higher in 2003 as compared to 2002, with 76.1% (95% CI 69.9â82.2) of children under five years old and 52.4% (95% CI 38.2â66.6) of pregnant women sleeping under ITNs. The ITN distribution programme resulted in a gross increase in ITN use among children and pregnant women of 68.3% and 48% respectively. CONCLUSION: Eritrea has exceeded the Abuja targets of 60% coverage for ITN household possession and use among children under five years old within two malarious provinces. Results point to several important potential sources of bias that must be considered when interpreting data for ITN coverage over time, including: disparate survey universes and target populations that may include non-malarious areas; poor date recall of bednet procurement and treatment; and differences in timing of surveys with respect to malaria season

Breeding and moulting fasts in macaroni penguins: Do birds exhaust their fat reserves?
1. Plasma concentrations of urea, uric acid and total lipid were compared in pre- and late-fast breeding and moulting macaroni penguins (Eudyptes chrysolophus) to test the hypothesis that birds exhaust their lipid reserves and initiate marked protein utilisation towards the end of natural fasts.
2. Male and female macaroni penguins fasted for a minimum of 29â32 days and 20 days during the breeding and moult fasts, and the difference in body weight over the sample period (reflecting body weight loss) was 31â34% and 41â47%, respectively.
3. There was no significant increase in plasma urea or uric acid at the end of either fast, nor any decrease in plasma lipid concentrations compared to pre-fast birds.
4. These results suggest that macaroni penguins continue to rely mainly on lipid reserves during the later stages of natural fasts. This is consistent with post-fast body composition data for other small penguin species
Dynamics of plasma nutrients and metabolites in moulting macaroni (Eud yptes chrysolophus) and gentoo (Pygoscelis papua) penguins
1.1. Dynamics of plasma nutrients and metabolites in moulting macaroni (Eudyptes chrysolophus) and gentoo (Pygoscelis papua) penguins were studied.2.2. The late moult macaroni and gentoo penguins had significantly elevated levels of total lipid and cholesterol, α-tocopherol (vitamin E) and alkaline phosphatase activity compared to early moult birds.3.3. Conversely, the mean concentrations of total protein, albumin, globulin, urate, creatinine, iron, calcium, inorganic phosphate, all-trans retinol and the activity of aspartate transaminase were lower in the late moult groups in both species.4.4. Mean concentrations of lipid, cholesterol, retinol (vitamin A), α-tocopherol (vitamin E), total protein, albumin, globulin, calcium and sodium were higher in the pre-moult than in the early moult macaronis.5.5. Dietary influence may partly account for the higher nutrient status of the pre-moult birds.6.6. Both the early and late moult macaronis had raised levels of total lipid, cholesterol, a-tocopherol and potassium compared to the corresponding gentoos.7.7. Total protein, albumin, globulin, sodium and inorganic phosphate concentrations, however, were higher in the later species.8.8.
