    TV Viewing, Independent of Physical Activity and Obesogenic Foods, Increases Overweight and Obesity in Adolescents

    The aim of this study was to estimate the prevalence of overweight and obesity (OAO) and associated risk factors in a representative sample of students aged 11-20 years in Urmia, Iran. In this population-based cross-sectional study, a multistage random cluster-sampling method was used, through which 2,498 students were selected. OAO were defined based on criteria set by the US Center for Health Statistics in collaboration with the US Center for Chronic Disease Prevention and Health Promotion under the Centers for Disease Control and Prevention (CDC). OAO risk factors were assessed using a questionnaire containing questions about TV viewing, nutrition, physical activities (PA), social and economic factors. Contents of the questionnaire were validated by calculating the content validity ratio (CVR) and content validity index (CVI), based on the responses elicited from 15 experts. Reliability of the questionnaire was obtained from a test and re-test of the questionnaire completed by 15 students. To analyze the data, \u3c72-test, t-test, and multiple logistic regression analysis were conducted. The prevalence of OAO was found to be 14.1% among the 11-20 years old students of junior and senior high schools. The results of multiple logistic regression analysis indicated that the educational level of mothers, type of school, and the time spent on viewing TV were associated with an increased risk of OAO while obesogenic foods and PA had no effect on the frequency of OAO [Odds ratio (OR) for the time spent on watching TV one hour more than usual equals 1.27 at p=0.001]. The direct correlation between TV viewing and OAO, which is independent of PA and obesogenic foods, needs to be carefully investigated through randomized clinical trials and cohort studies

    Maternal weight gain might predict child’s weight at the 12th and 24th months of age in Damavand city, North-East Tehran

    The purpose of this study was to investigate the possible association between pregnancy weight gain and child weight at the 12th and 24th months of age in Damavand city, North-East Tehran, Iran. Materials and Methods: In a cross-sectional study, using existing data collected by the urban health centers, records of 266 women who attended urban health centers for prenatal care and delivered between March 2004 and March 2008 with singleton term pregnancy were analyzed. Data on pre-pregnancy weight and height, total pregnancy weight gain, mother’s age, parity, neonatal birth weight, birth order, mother’s education, working status, breastfeeding practice and child weight at the 12th and 24th months of age were extracted. Pregnant Women were categorized based on their pre-pregnancy body mass index (BMI) as underweight, normal, and overweight and obese. Chi-square and ANOVA tests were employed to analyze data using SPSS software. Mean (±SD) of gestational weight gain was 11.4±3.9 kg. On average, 34% of mothers had inadequate weight gain, while 44 and 22% gained appropriate and excessive weight, respectively, based on the US Institute of Medicine (IoM-1990) guidelines. About 11, 56, 20 and 13% of mothers were defined as underweight, normal, overweight, and obese based on their BMI values. Initial BMI and educational level was associated with pregnancy weight gain. There was a significant difference between mean child weight at the 12th and 24th months of ages in all three categories of pregnancy weight gain, i.e. inadequate (<6.8 Kg), normal (6.8–11.3 Kg), and excessive (>11.3 Kg) (p< 0.01). Gestational weight gain seemed associated with child weight. Educating and assisting pregnant women to meet the weight gain within recommendations range might therefore be an important strategy to prevent child malnutrition, both under- and overweight, in early life.

    Arsenic intakes from spices and sheep tail (Donbeh) in a high oesophageal cancer area: Pilot study from Iran

    Arsenic is a carcinogenic and toxicant element widely found in the environment and might be part of causation web of Oesophageal Cancer (OC), which has one of the highest incidence rates in Golestan Province, Northeast Iran. In this pilot study, we collected 10 Sheep tail (donbeh) and 9 mixed spices samples, as widely consumed foods in the region, from main counties in the province and analyzed for arsenic concentrations using AOAC atomic absorption spectrophotometry method. Findings showed that arsenic levels in all samples were less than the safe level of 1 ppm. Total arsenic intake from sheep tail and mixed spices sources were estimated at 4.68 and 4.24 µg/day in men and women, respectively. Therefore, the study suggests that arsenic intake from these commonly consumed foods do not seem as a major hazard for OC etiology

    Can current supply figures support the nutritional recommendation of fish consumption in Iran?

       "Fish consumption twice per week" recommendation has been adopted in many countries, including Iran, by Nutrition experts. Data derived from food balance sheets (FBSs) and national household food consumption surveys (NHFCSs) show that fish consumption has been increased in the last 20 years in Iran. The gap between supply and recommendation figures in order to analyze the feasibility of this policy however needs to be determined. In this study, we took current figures of fish supply and consumption in Iran and calculated the amount of fish needed to support the recommendation of fish consumption. Data obtained from FAOSTAT-FBSs in 1980-82, 1990-92 and 2000-02, and NHFCS reports in 1992-95 and 2001-03. Fish supplies needed to fulfill the nutritional policy were calculated based on 120 and 180 gr/caput/wk scenarios. Sharp increase happened in the average fish supply from 1980s to 1990s, but slowed down afterwards. In early 2000, fish availability and intake were 4.73 and 4.43 kg (as raw-whole fish)/capita/year, respectively. The amount of fish required to fulfill the recommendation were however calculated as 10.97 and 16.43 kg/caput/yr based on the two scenarios, respectively. This study reveals that the gap between present fish consumption and the amounts for nutritional goal is still big. Whether bridging this gap in terms of feasibility, ecological, environmental and logistical burdens is attainable, needs more evaluation. Nutrition educators should be aware of the effects of their campaigns on the nationwide food policy as well as on issues such as consumer demand, prices, and environment.

    Efficacy and Safety of the Irinotecan, Capecitabine, and Oxaliplatin (IOX) Regimen in Metastatic Gastric Cancer: A Single Arm Phase II Trial

    Background: Gastric cancer is one of the most common malignancies worldwide with a high case mortality rate. In metastatic gastric cancer, a proper combination of chemotherapy could increase the survival rate. The goal of this study is to evaluate the efficacy and safety of the combination regimen of irinotecan, oxaliplatin, and Xeloda in metastatic gastric cancer. Methods: A total of 45 patients with metastatic gastric cancer and good performance status according to the Eastern Cooperative Oncology Group (score: 0-1) received the irinotecan, oxaliplatin, and Xeloda chemotherapy regimen. Demographic data, responses to treatment, and adverse effects were gathered for all cases. Overall survival and progression-free survival rates for patients were calculated using the Kaplan-Meier estimate. Results: Patients’ mean age was 58.3 ± 11.3 years (range: 24-81). There were 73.4% male patients and 26.6% female patients. Anorexia and weight loss were the most common symptoms. Overall response rate was 50%. The majority of toxicities were anemia, nausea and vomiting (grades 1 and 2), diarrhea (grades 1 and 2), neutropenia, alopecia, and hand and foot syndrome. The one-year progression-free survival rate was 31.5 ± 7.5%, whereas the twoyear progression-free survival rate was zero. The one-year overall survival rate was 34.91 ± 8.5%. Patients had a two-year overall survival rate of 7.7 ± 6.6%. Diffuse type cancer was linked to an inferior outcome. Conclusion: Regardless of our limited number of patients, this combination could be a suitable regimen for metastatic gastric cancer in terms of low toxicity, acceptable response rate, and survival results

    Three doses of a recombinant conjugated SARS-CoV-2 vaccine early after allogeneic hematopoietic stem cell transplantation: predicting indicators of a high serologic response—a prospective, single-arm study

    BackgroundAllogeneic hematopoietic stem cell transplant (allo-HSCT) recipients must be vaccinated against SARS-CoV-2 as quickly as possible after transplantation. The difficulty in obtaining recommended SARS-CoV-2 vaccines for allo-HSCT recipients motivated us to utilize an accessible and affordable SARS-CoV-2 vaccine with a recombinant receptor-binding domain (RBD)–tetanus toxoid (TT)-conjugated platform shortly after allo-HSCT in the developing country of Iran.MethodsThis prospective, single-arm study aimed to investigate immunogenicity and its predictors following a three-dose SARS-CoV-2 RBD–TT-conjugated vaccine regimen administered at 4-week (± 1-week) intervals in patients within 3–12 months post allo-HSCT. An immune status ratio (ISR) was measured at baseline and 4 weeks (± 1 week) after each vaccine dose using a semiquantitative immunoassay. Using the median ISR as a cut-off point for immune response intensity, we performed a logistic regression analysis to determine the predictive impact of several baseline factors on the intensity of the serologic response following the third vaccination dose.ResultsThirty-six allo-HSCT recipients, with a mean age of 42.42 years and a median time of 133 days between hematopoietic stem cell transplant (allo-HSCT) and the start of vaccination, were analyzed. Our findings, using the generalized estimating equation (GEE) model, indicated that, compared with the baseline ISR of 1.55 [95% confidence interval (CI) 0.94 to 2.17], the ISR increased significantly during the three-dose SARS-CoV-2 vaccination regimen. The ISR reached 2.32 (95% CI 1.84 to 2.79; p = 0.010) after the second dose and 3.87 (95% CI 3.25 to 4.48; p = 0.001) after the third dose of vaccine, reflecting 69.44% and 91.66% seropositivity, respectively. In a multivariate logistic regression analysis, the female sex of the donor [odds ratio (OR) 8.67; p = 0.028] and a higher level donor ISR at allo-HSCT (OR 3.56; p = 0.050) were the two positive predictors of strong immune response following the third vaccine dose. No serious adverse events (i.e., grades 3 and 4) were observed following the vaccination regimen.ConclusionsWe concluded that early vaccination of allo-HSCT recipients with a three-dose RBD–TT-conjugated SARS-CoV-2 vaccine is safe and could improve the early post-allo-HSCT immune response. We further believe that the pre-allo-HSCT SARS-CoV-2 immunization of donors may enhance post-allo-HSCT seroconversion in allo-HSCT recipients who receive the entire course of the SARS-CoV-2 vaccine during the first year after allo-HSCT

    The Effect of Two Different Doses of Cuminum Cyminum Extract on Serum Glycemic Indices and Inflammatory Factors in Patients with Diabetes Type II: A Randomized Double-Blind Controlled Clinical Trial

    Background & objectives: Type 2 diabetes is a metabolic disorder considered as one of the most common chronic diseases in many countries, including Iran. One of the reducing factors in insulin sensitivity is increased serum levels of inflammatory factors like hsCRP and TNF-&alpha;. On the other hand, in recent years, Cuminum Cyminum is known as one of the potent herbs in the treatment of diabetes type 2 in animal studies. The aim of this study was to evaluate the effect of two doses of 50 and 100 mg of Cuminum Cyminum extract on glycemic indices and indicators of inflammation in human subjects with type 2 diabetes mellitus. Methods: This study is a double blind randomized clinical trial performed in Urmia city on 2015. Samples were selected based on inclusion criteria and randomly divided into 3 groups: two groups receiving Cuminum Cyminum extract of 50 and 100 mg per day and the control (placebo) group. Before and 8 weeks after intervention, fasting blood sugar, serum insulin and inflammatory profile of hsCRP and TNF-&alpha; were measured. Data were analyzed by SPSS and the effect of supplementation was studied. Results: The mean age of the all participants was 47.33&plusmn;6.36. Results showed that the mean fasting blood sugar (FBS), glycosylated hemoglobin (HbA1c), and serum insulin reduced significantly in the both groups receiving Cuminum extract of&nbsp; 50 and 100 mg(p<0.01)., but increased in the placebo group. Average hsCRP and TNF-&alpha; levels have shown significant reduction at the beginning and end of the study in either group receiving 50 and 100 mg of supplementation (p<0.01). Conclusion: This study indicates that short-term daily supplementation of Cuminum Cyminum extract at the doses of 50 or 100mg in patients with type 2 diabetes during 8 weeks may act to reduce the serum levels of insulin, blood glucose, glycosylated hemoglobin and hsCRP and TNF-&alpha; levels, and also may be fruitful for control of this disease

    A Nutritional Study on Weight Status among Urban Male High School Students and Its Related Determinants in Urmia, IRAN

    Background and Objectives: Several studies have so far been carried out to determine the prevalence of overweight and obesity among Iranian children but few studies have investigated the relationship between eating habits, socio-demographic differentials and obesity in these children. So, this study tried to assess the body weight status among the urban high school boy students of Urmia, Iran. Materials and Methods: In this cross-sectional study, we measured weight, height and BMI of 1026 boy students with an age range of 15 to 19 years by multi-stage sampling. Then a questionnaire was completed based on their dietary habits and behaviors related to their physical activity. Overweight and obesity were defined based upon the World Health Organization&rsquo;s growth charts. Statistical analysis was fulfilled by estimating absolute and relative frequencies. Results: Underweight prevalence in the studied population was 9.84% (95% CI: 8.49%-10.72%). The prevalence of overweight and obesity among the studied students was 12.77% (95% CI: 11.89%-13.3%), and 5.07% (95% CI: 4.5%-5.9%), respectively. Factors significantly correlated with the participants' body weight were the frequency of energy dense foods intake during a week (P<0.001), duration of computer use per day (P<0.01), and the kind of their schools (P<0.05). No significant association was found between other studied dietary variables (such as the time spent for eating over 24-hours, and breakfast eating times per week) and overweight/obesity. Conclusions: In the present study, we found that two changeable parameters (i.e. the number of times for intake of energy dense foods during a week, and working with computers) could influence the 15-19 years old school boys' body weight in the study region.
