    Comparing pre- and post-pandemic greenhouse gas and noise emissions from road traffic in Rome (Italy): a multi-step approach

    This study presents the results of a traffic simulation analysis and emissions (greenhouse gas and noise) assessment comparing pre-pandemic (2019) and post-pandemic (2022) periods. The estimation of road traffic demand is based on conventional data sources and floating car data; next, the traffic simulation procedure was performed providing road network traffic volumes, which are the input for the emission models. The diffusion of teleworking, e-commerce, as well as the digitization of many processes, services and activities, lead to a significant change in urban mobility. Results show a significant though still not complete resumption of commuters travel activity (−10% compared to pre-pandemic period) in the morning peak-hour. This translates into an 11% reduction of greenhouse gas emissions and a 0.1% increase in noise emissions

    Aortic aneurysms, chronic kidney disease and metalloproteinases

    Metalloproteinases (MPs) are proteolytic enzymes involved in extracellular matrix deposition, regulation of cellular signals of inflammation, proliferation, and apoptosis. Metalloproteinases are classified into three families: Matrix-MPs (MMPs), A-Disintegrin-and-Metalloprotease (ADAMs), and the A-Disintegrin-and-Metalloproteinase-with-Thrombospondin-1-like-Domains (ADAMTS). Previous studies showed that MPs are involved in the development of aortic aneurysms (AA) and, concomitantly, in the onset of chronic kidney disease (CKD). CKD has been, per se, associated with an increased risk for AA. The aim of this review is to examine the pathways that may associate MPs with CKD and AA. Several MMPs, such as MMP-2, -8, -9, and TIMP-1 have been shown to damage the AA wall and to have a toxic effect on renal tubular cells, leading to fibrosis. Similarly, ADAM10 and 17 have been shown to degrade collagen in the AA wall and to worsen kidney function via pro-inflammatory stimuli, the impairment of the Renin-Angiotensin-Aldosterone System, and the degradation of structural proteins. Moreover, MMP-2 and -9 inhibitors reduced aneurysm growth and albuminuria in experimental and human studies. It would be important, in the future, to expand research on MPs from both a prognostic, namely, to refine risk stratification in CKD patients, and a predictive perspective, likely to improve prognosis in response to targeted treatments

    On Transport Monitoring and Forecasting during COVID-19 Pandemic in Rome

    This paper presents the results of a study on the Rome mobility system aiming at estimating the impacts of the progressive lockdown, imposed by the government, due to the Covid-19 pandemic as well as to support decision makers in planning the transport system for the restart towards a post-Covid "new normal". The analysis of data obtained by the transport monitoring system has been fundamental for both investigating effects of the lockdown and feeding transport models to predict the impacts on future actions. At first, the paper focuses on the so-called transport analytics, by describing mobility trends for the multimodal transportation system of Rome. Then, the results of the simulated scenarios to design public transport services, able to ensure passengers social distancing required in the first post-Covid months, are presented and discussed

    COVID-19 and the Kidney: From Epidemiology to Clinical Practice

    The new respiratory infectious disease coronavirus disease 2019 (COVID-19) that originated in Wuhan, China, in December 2019 and caused by a new strain of zoonotic coronavirus, named severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2), to date has killed over 630,000 people and infected over 15,000,000 worldwide. Most of the deceased patients had pre-existing comorbidities; over 20% had chronic kidney disease (CKD). Furthermore, although SARS-CoV-2 infection is characterized mainly by diffuse alveolar damage and acute respiratory failure, acute kidney injury (AKI) has developed in a high percentage of cases. As AKI has been shown to be associated with worse prognosis, we believe that the impact of SARS-CoV-2 on the kidney should be investigated. This review sets out to describe the main renal aspects of SARS-CoV-2 infection and the role of the virus in the development and progression of kidney damage. In this article, attention is focused on the epidemiology, etiology and pathophysiological mechanisms of kidney damage, histopathology, clinical features in nephropathic patients (CKD, hemodialysis, peritoneal dialysis, AKI, transplantation) and prevention and containment strategies. Although there remains much more to be learned with regards to this disease, nonetheless it is our hope that this review will aid in the understanding and management of SARS-CoV-2 infection

    This study presents the result of a traffic simulation analysis based on Floating Car Data and a noise emission assessment to show the impact of mobility restriction for COVID-19 containment on urban vehicular traffic and road noise pollution on the road network of Rome, Italy. The adoption of strong and severe measures to contain the spreading of Coronavirus during March-April 2020 generated a significant reduction in private vehicle trips in the city of Rome (-64.6% during the lockdown). Traffic volumes, obtained through a simulation approach, were used as input parameters for a noise emission assessment conducted using the CNOSSOS-EU method, and an overall noise emissions reduction on the entire road network was found, even if its extent varied between road types

    Autoimmune Hypophysitis with Late Renal Involvement: A Case Report

    We report a case of a 50-year-old male admitted to the Endocrinology Unit because of persistent headaches, nausea, feeling tired, sudden weight loss, cold intolerance, decreased appetite, and lack of sex interest. Diagnostic workup showed a 6-millimeter pituitary tumor without signs of compression, and a condition of progressive panhypopituitarism. After 12 months of hormone replacement therapy, the patient was hospitalized because of sudden weight gain, periorbital-peripheral edema, severe dyslipidemia, hypertension, and proteinuria. Corticosteroid therapy was shifted from oral to continuous intravenous infusion, and once the diagnosis of “immune complex-mediated glomerulonephritis with mesangial deposits suggestive for membranoproliferative glomerulonephritis type IIIIgG4-positive” was made, the immunosuppressant mycophenolate (1500 mg/day) was started. After a 6-month follow-up, the complete resolution of renal symptoms was accompanied by the disappearance of a pituitary lesion and the patient was back to prior hormone replacement therapy. Autoimmune hypophysitis (AH) is a rare autoimmune inflammatory disease of the pituitary gland that can impair hormone secretion and function. IgG4-hypophysitis is rare and is usually associated with other IgG4-related diseases. Herein, we describe a rare case of AH associated with late renal disease, and without any other organ involvement

    Hyperkalemia in CKD: an overview of available therapeutic strategies

    : Hyperkalemia (HK) is a life-threatening condition that often occurs in patients with chronic kidney disease (CKD). High serum potassium (sKsK) is responsible for a higher risk of end-stage renal disease, arrhythmias and mortality. This risk increases in patients that discontinue cardio-nephroprotective renin-angiotensin-aldosterone system inhibitor (RAASi) therapy after developing HK. Hence, the management of HK deserves the attention of the clinician in order to optimize the therapeutic strategies of chronic treatment of HK in the CKD patient. The adoption in clinical practice of the new hypokalaemic agents patiromer and sodium zirconium cyclosilicate (SZC) for the prevention and chronic treatment of HK could allow patients, suffering from heart failure and chronic renal failure, to continue to benefit from RAASi therapy. We have updated a narrative review of the clear variables, correct definition, epidemiology, pathogenesis, etiology and classifications for HK among non-dialysis CKD (ND CKD) patients. Furthermore, by describing the prognostic impact on mortality and on the progression of renal damage, we want to outline the strategies currently available for the control of potassium (K+) plasma levels
