    Ein Netzwerk von IDS-Sensoren für Angriffsstatistiken

    Angriffserkennende Systeme (Intrusion Detection Systems, IDS) sind mittlerweile eine etablierte Technik, um Informationen über die Gefährdung der eigenen Systeme zu gewinnen. Moderne IDS realisieren ein Framework einer Vielzahl unterschiedlicher Sensoren, um eine möglichst vollständige Datenerfassung durchzuführen. Der Einsatz vieler Sensoren, die über das gesamte Internet verteilt sind, ermöglicht die Beobachtung der unterschiedlichen Angriffscharakteristiken in den verschiedenen Teilnetzen. Durch Korrelation der Angriffe auf die Sensoren lassen sich auch Rückschlüsse auf das Verhalten von Angreifern und auf die Verbreitungsstrategien von Würmern und Viren ziehen. Im Rahmen des Projektes eCSIRT.net wurde ein Netz aus international verteilten Sensoren aufgebaut, das die gesammelten Angriffsdaten zur Erzeugung von entsprechenden Statistiken nutzt. Weiterhin können die gesammelten Daten den Netzbetreibern behilflich sein, Vorfälle zu erkennen und zu bearbeiten. Dieser Artikel stellt die Realisierung des Sensor-Netzwerkes vor und zeigt erste Ergebnisse der Datensammlung

    Perceived need for treatment and non-utilization of outpatient psychotherapy in old age: two cohorts of a nationwide survey

    Beackground: Older adults with mental health problems may benefit from psychotherapy; however, their perceived need for treatment in relation to rates of non-utilization of outpatient psychotherapy as well as the predisposing, enabling, and need factors proposed by Andersen's Model of Health Care Utilization that account for these differences warrant further investigation. Methods: We used two separate cohorts (2014 and 2019) of a weighted nationwide telephone survey in Germany of German-speaking adults with N=12,197 participants. Across the two cohorts, 12.9% (weighted) reported a perceived need for treatment for mental health problems and were selected for further analyses. Logistic Generalized Estimation Equations (GEE) was applied to model the associations between disposing (age, gender, single habiting, rural residency, general health status), enabling (education, general practitioner visit) non-utilization of psychotherapy (outcome) across cohorts in those with a need for treatment (need factor). Results: In 2014, 11.8% of 6087 participants reported a perceived need for treatment due to mental health problems. In 2016, the prevalence increased significantly to 14.0% of 6110 participants. Of those who reported a perceived need for treatment, 36.4% in 2014 and 36.9%in 2019 did not see a psychotherapist - where rates of non-utilization of psychotherapy were vastly higher in the oldest age category (59.3/52.5%; 75+) than in the youngest (29.1/10.7%; aged 18-25). Concerning factors associated with non-utilization, multivariate findings indicated participation in the cohort of 2014 (OR 0.94), older age (55-64 OR 1.02, 65-74 OR 1.47, 75+ OR 4.76), male gender (OR 0.83), lower educational status (OR 0.84), rural residency (OR 1.38), single habiting (OR 1.37), and seeing a GP (OR 1.39) to be related with non-utilization of psychotherapy; general health status was not significantly associated with non-utilization when GP contact was included in the model. Conclusion: There is a strong age effect in terms of non-utilization of outpatient psychotherapy. Individual characteristics of both healthcare professionals and patients and structural barriers may add to this picture. Effective strategies to increase psychotherapy rates in those older adults with unmet treatment needs are required

    Critical fields and growth rates of the Tayler instability as probed by a columnar gallium experiment

    Many astrophysical phenomena (such as the slow rotation of neutron stars or the rigid rotation of the solar core) can be explained by the action of the Tayler instability of toroidal magnetic fields in the radiative zones of stars. In order to place the theory of this instability on a safe fundament it has been realized in a laboratory experiment measuring the critical field strength, the growth rates as well as the shape of the supercritical modes. A strong electrical current flows through a liquid-metal confined in a resting columnar container with an insulating outer cylinder. As the very small magnetic Prandtl number of the gallium-indium-tin alloy does not influence the critical Hartmann number of the field amplitudes, the electric currents for marginal instability can also be computed with direct numerical simulations. The results of this theoretical concept are confirmed by the experiment. Also the predicted growth rates of the order of minutes for the nonaxisymmetric perturbations are certified by the measurements.     The barrier-free trocar technique in three laparoscopic standard procedures

    Background : Numerous technical and surgical innovations took place in laparoscopic surgery in the recent past. It is debatable whether single-access surgery or natural orifice surgery (NOS) will establish for several standard procedures. Most of the NOS-procedures are controversial and single-access surgery still has to prove its equality in controlled trials. In the intention to reduce the ingress incisons and to facilitate instrumentation, we decided to test the barrier-free AirSeal® -trocar in clinical practice. Materials and Methods : Laparoscopically we performed a cholecystectomy, gastric wedge-resection and a fundoplication using the barrier-free AirSeal® 12-mm-trocar. This trocar works without any mechanical barrier so that via this trocar the use of two instruments is possible. Results : All three operations were successful. Conclusion : Laparoscopic standard procedures are feasible using this barrier-free trocar without a higher degree of difficulty. Because of the facilitated instrumentation, it is possible to work more efficiently and to maintain the focus on the surgical field

    CONKO-005: Adjuvant Chemotherapy With Gemcitabine Plus Erlotinib Versus Gemcitabine Alone in Patients After R0 Resection of Pancreatic Cancer: A Multicenter Randomized Phase III Trial

    Purpose Gemcitabine is standard of care in the adjuvant treatment of resectable pancreatic ductal adenocarcinoma (PDAC). The epidermal growth factor receptor tyrosine kinase inhibitor erlotinib in combination with gemcitabine has shown efficacy in the treatment of advanced PDAC and was considered to improve survival in patients with primarily resectable PDAC after R0 resection. Patients and Methods In an open-label, multicenter trial, patients were randomly assigned to one of two study arms: gemcitabine 1,000 mg/m(2) days 1, 8, 15, every 4 weeks plus erlotinib 100 mg once per day (GemErlo) or gemcitabine (Gem) alone for six cycles. The primary end point of the study was to improve disease-free survival (DFS) from 14 to 18 months by adding erlotinib to gemcitabine. Results In all, 436 patients were randomly assigned at 57 study centers between April 2008 and July 2013. A total of 361 instances (83%) of disease recurrence were observed after a median follow-up of 54 months. Median treatment duration was 22 weeks in both arms. There was no difference in median DFS (GemErlo 11.4 months; Gem 11.4 months) or median overall survival (GemErlo 24.5 months; Gem 26.5 months). There was a trend toward long-term survival in favor of GemErlo (estimated survival after 1, 2, and 5 years for GemErlo was 77%, 53%, and 25% v 79%, 54%, and 20% for Gem, respectively). The occurrence or the grade of rash was not associated with a better survival in the GemErlo arm. Conclusion To the best of our knowledge, CONKO-005 is the first study to investigate the combination of chemotherapy and a targeted therapy in the adjuvant treatment of PDAC.
