    Cohomological Hasse principle and motivic cohomology for arithmetic schemes

    In 1985 Kazuya Kato formulated a fascinating framework of conjectures which generalizes the Hasse principle for the Brauer group of a global field to the so-called cohomological Hasse principle for an arithmetic scheme. In this paper we prove the prime-to-characteristic part of the cohomological Hasse principle.     The association between experimental and clinical pain measures among persons with fibromyalgia and chronic fatigue syndrome

    Evoked or experimental pain is often used as a model for the study of clinical pain, yet there are little data regarding the relationship between the two. In addition, there are few data regarding the types of stimuli and stimulus intensities that are most closely related to clinical pain. In this study, 36 subjects with fibromyalgia (FM), chronic fatigue syndrome (CFS), or both syndromes were administered measures of clinical pain and underwent a dolorimetry evaluation. Subjects also underwent experimental pain testing utilizing heat and pressure stimulation. Stimulation levels evoking low, moderate and high sensory intensity, and comparable levels of unpleasantness, were determined for both types of stimuli using random staircase methods. Clinical pain was assessed using visual analogue ratings and the short form of the McGill Pain Questionnaire (MPQ). Ratings of heat pain sensation were not significantly associated with clinical pain ratings, with the exception of unpleasantness ratings at high stimulus intensities. Pain threshold and tolerance as assessed by dolorimetry were significantly associated with average measures of clinical pain. Both intensity and unpleasantness ratings of pressure delivered using random staircase methods were significantly associated with clinical pain at low, moderate and high levels, and the strength of the association was greater at increasingly noxious stimulus intensities. These findings suggest that random pressure stimulation as an experimental pain model in these populations more closely reflects the clinical pain for these conditions.     Wellness and Multiple Sclerosis: The National MS Society Establishes a Wellness Research Working Group and Research Priorities

    Background: People with multiple sclerosis (MS) have identified “wellness” and associated behaviors as a high priority based on “social media listening” undertaken by the National MS Society (i.e. the Society). Objective: The Society recently convened a group that consisted of researchers with experience in MS and wellness-related research, Society staff members, and an individual with MS for developing recommendations regarding a wellness research agenda. Method: The members of the group engaged in focal reviews and discussions involving the state of science within three approaches for promoting wellness in MS, namely diet, exercise, and emotional wellness. Results: That process informed a group-mediated activity for developing and prioritizing research goals for wellness in MS. This served as a background for articulating the mission and objectives of the Society’s Wellness Research Working Group. Conclusion: The primary mission of the Wellness Research Working Group is the provision of scientific evidence supporting the application of lifestyle, behavioral, and psychosocial approaches for promoting optimal health of mind, body, and spirit (i.e. wellness) in people with MS as well as managing the disease and its consequences

    On the Whitehead spectrum of the circle

    The seminal work of Waldhausen, Farrell and Jones, Igusa, and Weiss and Williams shows that the homotopy groups in low degrees of the space of homeomorphisms of a closed Riemannian manifold of negative sectional curvature can be expressed as a functor of the fundamental group of the manifold. To determine this functor, however, it remains to determine the homotopy groups of the topological Whitehead spectrum of the circle. The cyclotomic trace of B okstedt, Hsiang, and Madsen and a theorem of Dundas, in turn, lead to an expression for these homotopy groups in terms of the equivariant homotopy groups of the homotopy fiber of the map from the topological Hochschild T-spectrum of the sphere spectrum to that of the ring of integers induced by the Hurewicz map. We evaluate the latter homotopy groups, and hence, the homotopy groups of the topological Whitehead spectrum of the circle in low degrees. The result extends earlier work by Anderson and Hsiang and by Igusa and complements recent work by Grunewald, Klein, and Macko.Comment: 52 page

    X-band electron paramagnetic resonance on doped Er3+ and Yb3+ ions in Y0.99(Yb,Er)0.01Ba2Cu3OX compounds with different oxygen contents in the wide temperature range (4-120)K have been made. In the superconducting species, the strong dependencies of the linewidth and resonance line position from the sweep direction of the applied magnetic field are revealed at the temperatures significantly below TC. The possible origins of the observed hysteresis are analyzed.     DEvIANT: Discovering Significant Exceptional (Dis-)Agreement Within Groups

    We strive to find contexts (i.e., subgroups of entities) under which exceptional (dis-)agreement occurs among a group of individuals , in any type of data featuring individuals (e.g., parliamentarians , customers) performing observable actions (e.g., votes, ratings) on entities (e.g., legislative procedures, movies). To this end, we introduce the problem of discovering statistically significant exceptional contextual intra-group agreement patterns. To handle the sparsity inherent to voting and rating data, we use Krippendorff's Alpha measure for assessing the agreement among individuals. We devise a branch-and-bound algorithm , named DEvIANT, to discover such patterns. DEvIANT exploits both closure operators and tight optimistic estimates. We derive analytic approximations for the confidence intervals (CIs) associated with patterns for a computationally efficient significance assessment. We prove that these approximate CIs are nested along specialization of patterns. This allows to incorporate pruning properties in DEvIANT to quickly discard non-significant patterns. Empirical study on several datasets demonstrates the efficiency and the usefulness of DEvIANT.     Observation of dipole-mode vector solitons

    We report on the first experimental observation of a novel type of optical vector soliton, a {\em dipole-mode soliton}, recently predicted theoretically. We show that these vector solitons can be generated in a photorefractive medium employing two different processes: a phase imprinting, and a symmetry-breaking instability of a vortex-mode vector soliton.     A primary care, multi-disciplinary disease management program for opioid-treated patients with chronic non-cancer pain and a high burden of psychiatric comorbidity

    BACKGROUND: Chronic non-cancer pain is a common problem that is often accompanied by psychiatric comorbidity and disability. The effectiveness of a multi-disciplinary pain management program was tested in a 3 month before and after trial. METHODS: Providers in an academic general medicine clinic referred patients with chronic non-cancer pain for participation in a program that combined the skills of internists, clinical pharmacists, and a psychiatrist. Patients were either receiving opioids or being considered for opioid therapy. The intervention consisted of structured clinical assessments, monthly follow-up, pain contracts, medication titration, and psychiatric consultation. Pain, mood, and function were assessed at baseline and 3 months using the Brief Pain Inventory (BPI), the Center for Epidemiological Studies-Depression Scale scale (CESD) and the Pain Disability Index (PDI). Patients were monitored for substance misuse. RESULTS: Eighty-five patients were enrolled. Mean age was 51 years, 60% were male, 78% were Caucasian, and 93% were receiving opioids. Baseline average pain was 6.5 on an 11 point scale. The average CESD score was 24.0, and the mean PDI score was 47.0. Sixty-three patients (73%) completed 3 month follow-up. Fifteen withdrew from the program after identification of substance misuse. Among those completing 3 month follow-up, the average pain score improved to 5.5 (p = 0.003). The mean PDI score improved to 39.3 (p < 0.001). Mean CESD score was reduced to 18.0 (p < 0.001), and the proportion of depressed patients fell from 79% to 54% (p = 0.003). Substance misuse was identified in 27 patients (32%). CONCLUSIONS: A primary care disease management program improved pain, depression, and disability scores over three months in a cohort of opioid-treated patients with chronic non-cancer pain. Substance misuse and depression were common, and many patients who had substance misuse identified left the program when they were no longer prescribed opioids. Effective care of patients with chronic pain should include rigorous assessment and treatment of these comorbid disorders and intensive efforts to insure follow up

    Chronic non-specific low back pain - sub-groups or a single mechanism?

Clinical trials evaluating the efficacy of a variety of interventions for chronic non-specific low back pain indicate limited effectiveness for most commonly applied interventions and approaches. Discussion: Many clinicians challenge the results of clinical trials as they feel that this lack of effectiveness is at odds with their clinical experience of managing patients with back pain. A common explanation for this discrepancy is the perceived heterogeneity of patients with chronic non-specific low back pain. It is felt that the effects of treatment may be diluted by the application of a single intervention to a complex, heterogeneous group with diverse treatment needs. This argument presupposes that current treatment is effective when applied to the correct patient. An alternative perspective is that the clinical trials are correct and current treatments have limited efficacy. Preoccupation with sub-grouping may stifle engagement with this view and it is important that the sub-grouping paradigm is closely examined. This paper argues that there are numerous problems with the sub-grouping approach and that it may not be an important reason for the disappointing results of clinical trials. We propose instead that current treatment may be ineffective because it has been misdirected. Recent evidence that demonstrates changes within the brain in chronic low back pain sufferers raises the possibility that persistent back pain may be a problem of cortical reorganisation and degeneration. This perspective offers interesting insights into the chronic low back pain experience and suggests alternative models of intervention. Summary: The disappointing results of clinical research are commonly explained by the failure of researchers to adequately attend to sub-grouping of the chronic non-specific low back pain population. Alternatively, current approaches may be ineffective and clinicians and researchers may need to radically rethink the nature of the problem and how it should best be managed
