    Keck Interferometer: from development phase to facility-class instrument

    The Keck Interferometer is entering a regular limited observational phase. A restricted number of observers are expected to use the instrument over the course of the next few years in a shared-risk capacity. To facilitate this, the W. M. Keck Observatory and the Jet Propulsion Laboratory are following a Handover procedure consisting of a number of stages related to the science modes of the instrument as they reach completion. The first of these is the Visibility Science mode that involves only the two Keck telescopes. Other modes to follow are Nulling, Differential Phase, Astrometry, and Imaging. The process includes defining a reasonable level of functionality of each mode, training observatory staff to maintain and schedule tasks related to the upkeep of each mode, and defining and documenting each of the subsystems related to each mode. Here we discuss the outline of the Handover plan and report on its progress to date

    Keck Interferometer autoaligner: algorithms and techniques

    The Keck Interferometer includes an autoalignment system consisting of pop-up targets located at strategic locations along the beam trains of each arm of the instrument along with a sensor and control system. We briefly describe the hardware of the system and then proceed to a description of the two operational modes of the system. These are: 1) to provide an initial alignment of the coude paths in each arm, and 2) to recover coude alignments between changes of the static delay sled positions. For the initial alignment mode, we review the system performance requirements along with the software used for image acquisition and centroiding. For coudé alignment recovery, we describe beam-train surveys through the static delay (Long Delay Line) and criteria for a successful recovery of a coudé alignment. Finally, we describe the results of testing of the autoalignment system

    The association of depression and all-cause and cause-specific mortality: an umbrella review of systematic reviews and meta-analyses

    Background: Depression is a prevalent and disabling mental disorder that frequently co-occurs with a wide range of chronic conditions. Evidence has suggested that depression could be associated with excess all-cause mortality across different settings and populations, although the causality of these associations remains unclear. Methods: We conducted an umbrella review of systematic reviews and meta-analyses of observational studies. PubMed, PsycINFO, and Embase electronic databases were searched through January 20, 2018. Systematic reviews and meta-analyses that investigated associations of depression and all-cause and cause-specific mortality were selected for the review. The evidence was graded as convincing, highly suggestive, suggestive, or weak based on quantitative criteria that included an assessment of heterogeneity, 95% prediction intervals, small-study effects, and excess significance bias. Results: A total of 26 references providing 2 systematic reviews and data for 17 meta-analytic estimates met inclusion criteria (19 of them on all-cause mortality); data from 246 unique studies (N = 3,825,380) were synthesized. All 17 associations had P < 0.05 per random effects summary effects, but none of them met criteria for convincing evidence. Associations of depression and all-cause mortality in patients after acute myocardial infarction, in individuals with heart failure, in cancer patients as well as in samples from mixed settings met criteria for highly suggestive evidence. However, none of the associations remained supported by highly suggestive evidence in sensitivity analyses that considered studies employing structured diagnostic interviews. In addition, associations of depression and all-cause mortality in cancer and post-acute myocardial infarction samples were supported only by suggestive evidence when studies that tried to adjust for potential confounders were considered. Conclusions: Even though associations between depression and mortality have nominally significant results in all assessed settings and populations, the evidence becomes weaker when focusing on studies that used structured interviews and those that tried to adjust for potential confounders. A causal effect of depression on all-cause and cause-specific mortality remains unproven, and thus interventions targeting depression are not expected to result in lower mortality rates at least based on current evidence from observational studies

    Crystal clear or tin ear: how do medical students interpret derogatory comments about patients and other professionals?

    Purpose: To assess the learning environment at our medical school, third-year medical students complete an 11-item survey called the Learning Environment for Professionalism (LEP) at the end of each clerkship. The LEP survey asks about the frequency of faculty and resident professional and unprofessional behaviors that students observed; two of the items specifically address derogatory comments. This study used focus group methodology to explore how medical students interpret the derogatory comments they reported on the LEP survey. Methods: Seven focus groups were conducted with 82 medical students after they completed the LEP survey. Analysis of focus group transcripts was performed to better understand the nature and meaning that students ascribe to derogatory comments. Results: The study results provide insights into the types of derogatory comments that medical students heard during their clerkship rotations, why the comments were made and how they were interpreted. Emergent themes, labeled by the authors as 1) ‘onstage-offstage’, 2) ‘one bad apple’, and 3) ‘pressure cooker environment’, highlight the contextual aspects and understandings ascribed by students to the derogatory comments. Incidentally, students felt that the comments were not associated with fatigue, but were associated with cumulative stress and burn-out. Conclusions: The results suggest students have a clear understanding of the nature of unprofessional comments made by role models during clerkships and point to important systems-related issues that could be leveraged to improve clinical learning environments
