    Anti-inflammatory effect of hemodialysis treatment in chronic dialysis patients with COVID-19: comparison between two dialyzers (ATA vs PMMA)

    Background I filtri dializzatori ad alto flusso potrebbero mitigare la “tempesta citochinica" nell'infezione da Sars-COV-2, ma il loro impatto nei pazienti in dialisi cronica non è accertato. Lo scopo delle studio è valutare l’effetto del filtro in triacetato asimmetrico di cellulosa (ATA) e in polimetilmetacrilato (PMMA) sui marcatori infiammatori in pazienti in dialisi cronica affetti da SARS-CoV-2. Metodi Si tratta di uno studio prospettico osservazionale su pazienti in trattamento emodialitico cronicp con COVID-19 arruolati da marzo 2020 a Maggio 2021.Le variabili cliniche, la conta leucocitaria, la IL-6, la proteina C-reattiva (PCR), la procalcitonina (PCT) e la ferritina sono state determinate al basale. I valori ematici di PCR, PCT, e IL-6 sono stati determinati pre e post-dialisi per ogni seduta effettuata (i valori ottenuti sono stati corretti per ’emoconcentrazione). I pazienti sono stati trattati con emodiafiltrazione online con un filtro ad alto flusso in PMMA o ATA. L’end-point primario è stato valutare l’effetto dei due filtri sulle molecole infiammatorie, in particolare sulla reduction ratio (RR) della IL-6. Risultati Dei 74 pazienti arruolati, 48 sono trati trattati con filtro ATA e 26 con filtro PMMA (420 vs 191 sedute dialitiche). La RR percentuale mediana della IL-6 è risultata maggiore nel gruppo ATA (17,08% IQR -9,0 - 40.0 vs 2,95% IQR -34,63 – 27,32. Anche le RR percentuale di PCR e PCT sono state maggiori nel gruppo ATA. La regressione logistica multipla avente come variabile dipendente una IL-6RR maggiore del 25%, ha mostrato che ATA determinava una maggiore probabilità di raggiungere l’outcome dopo correzione per i parametri infiammatori pre-dialisi (OR 1,721 95% CI 1,176 – 2,538 p=0,0056). Al contrario una PCR elevata riduceva la probabilità di ottenere una IL-6RR significativa (OR 0,9101 95% CI 0,868 – 0,949, p<0.0001). Conclusioni Nella nostra popolazione il filtro ATA ha mostrato un migliore profilo antiinfiammatorio.Background High flux hemodialysis membranes may modulate the cytokine storm of SARS-CoV-2, but their impact in chronic hemodialysis (CHD) patients is not assessed. Aim of the study was the evaluation of asymmetric cellulose triacetate (ATA) and polymethylmethacrylate (PMMA) dialyzers on inflammatory markers in CHD patients with SARS-CoV-2. Methods A prospective, observational study on CHD patients affected by SARS-CoV-2 was carried out and patients were enrolled from March 2020 to May 2021.Pre and post-dialysis C reactive protein (CRP), procalcitonin (PCT), and interleukin-6 (IL-6) were determined at each session and corrected for hemoconcentration. Patients who underwent on-line hemodiafiltration (OLHDF) with PMMA dialyzer or ATA were compared. The primary endpoint was to assess the differences in the reduction ratio per session (RR) of CRP, PCT and IL-6. Results Seventy-four patients were enrolled, 48 were treated with ATA and 26 with PMMA (420 vs 191 dialysis sessions.Median IL-6RR% was higher for ATA (17.08%, IQR -9.0 - 40.0 vs 2.95%, IQR -34.63 – 27.32). CRP and PCT showed higher RR with ATA in comparison to PMMA. When IL-6RR >25% was the dependent variable in the multiple logistic regression analysis only ATA showed a significant correlation (OR 1.721. 95% CI 1.176 - 2.538, p=0.0056) while higher CRP favoured the risk of lower IL6RR (OR 0.9101 95% CI 0.868 - 0.949, p<0.0001). Conclusion In SARS-CoV-2 CHD patients treated with OLHDF, ATA showed a better anti-inflammatory profile than PMMA, in particular regarding IL-6 RR

    Circulating tumour cells in colorectal cancer

    Abstract Background The detection of circulating tumour cells (CTC) in blood sample in patients with early or advanced colorectal cancer has a potential prognostic value. Methods The challenge of CTC detection is related to the requirement of high sensitivity combined with high specificity method. CTCs detection can be distinguished between indirect and direct methods. The former ones are based on the recognition of tissue-, organ- or tumour-specific markers by immuno-histochemistry (indirect immuno-mediated methods) or (real-time) RT-PCR (indirect molecular methods), whilst the latter are related to CTCs selection based on the physical properties of density and sizes. Ongoing and future isolation by size of epithelial tumour cells (ISET) developments concerning automated image analysis on the filter and transmission of high definition images through the web for 'on line' cytopathological consultations are aimed to speed up the work of cytopathologists on CTC/ circulating tumour microemboli (CTM) detection. Conclusions CTC detection in colorectal cancer (CRC) correlates with pathological stage and clinical outcome in particular in those patients with advanced disease. CRC CTC level before and after CT are an independent prognostic factor for progression-free and overall survival. The positive prognostic value of complete clearance CTC after surgery may be useful to select patients for adjuvant chemotherapy

    Time evolution of restless legs syndrome in haemodialysis patients

    Abstract Background Restless legs syndrome (RLS) is characterized by an urge to move the extremities, accompanied by paraesthesiae, in the evening and at night. Uraemic RLS, a type of secondary RLS, occurs commonly in chronic kidney disease and end-stage renal disease. Progression of uraemic RLS over time is unclear. Therefore we investigated the prevalence, progression over time, risk factors and impact on survival of uraemic RLS in a cohort of dialysis patients. Methods We reviewed at the 7-year follow-up a cohort of haemodialysis (HD) patients we had previously investigated for RLS, through interviews, validated questionnaires and analysis of demographic and clinical data. Results At the 7-year follow-up, RLS was present in 16% of patients, with a persistence rate of 33%. A correlation was obtained between RLS and older age, diabetes, low albumin and low body mass index. RLS was associated with reduced overall survival (median survival of 3.3 versus 3.7 years), particularly with the continuous form of RLS (1.61 years). There was a higher incidence of myocardial infarction and peripheral vascular disease, although not reaching statistical significance. RLS patients had absolute higher scores in all quality of life domains. A large majority of study patients (96%) reported being symptom-free within a few days or weeks following kidney transplantation. Conclusions The development of RLS, especially the continuous form, in patients undergoing HD has important consequences associated with decreased survival. Our results indicated an association between uraemic RLS and ageing, diabetes and malnutrition. Considerable efforts should be focused on the treatment of RLS, since it significantly and persistently impacts the quality of life of HD patients. Kidney transplantation could represent an effective treatment option for that RLS impacts on dialysis patients' quality of life, thus confirming the secondary nature of RLS in most HD patients

    Efficacy of Supra-HFR in Removing FGF23 and Cytokines: A Single Session Analysis

    Background/Aim: Supra hemodiafiltration with reinfusion of the endogenous ultrafiltrate (Supra-HFR) is a dialysis technique used to improve uremic toxin removal in the range of the middle molecular weight molecules. Supra-HFR does not require the preparation and online infusion of high purity dialysis water because it allows the production of an endogenous ultrafiltrate that undergoes detoxification through an adsorbing resin. Patients and Methods: We investigated the ability of Supra-HFR to remove fibroblast growth factor 23 (FGF23), interleukin 6 (IL-6), tumor necrosis factor alpha (TNFalpha), interleukin 8 (IL-8), and transforming growth factor alpha (TGF-alpha) after a single session dialysis in nine patients affected by end stage renal disease (ESRD). The same patients underwent a single session of online hemodiafiltration (OLHDF) to evaluate possible differences in FGF23 and IL-6 levels. Results: A significant reduction in FGF23 was observed with both Supra-HFR (p=0.001) and OL-HDF.     Early Light Chains Removal and Albumin Levels with a Double Filter-Based Extracorporeal Treatment for Acute Myeloma Kidney

    Renal impairment in Multiple Myeloma (MM) represents one of the most important factors that influences patient survival. In fact, before the introduction of modern chemotherapy, less than 25% of patients with acute kidney injury (AKI) and MM who required dialysis recovered sufficient renal function to become independent from dialysis, with a median overall survival of less than 1 year. There are many other factors involved in determining patient survival. In this study we aimed to investigate the role of double filter-based extracorporeal treatment for removal of serum free light chains (sFLC) in acute myeloma kidney (AKI for MM) and to evaluate patient overall survival. All patients received Bortezomib-based chemotherapy and extracorporeal treatment for sFLC removal. For each session 2 dialyzers of the same kind were used. The dialytic dose was not related to the degree of renal function but to the removal of sFLC. The factors that have been found to be significantly associated with lower mortality were reduction of sFLC at day 12 and day 30, &gt;50% reduction of sFLC at day 30, number of sessions and independence from dialysis. Among baseline characteristics, albumin level was statistically associated with the patients' outcome. Our analysis highlights the importance of the early treatment for removal of sFLC in AKI for MM. These results indicate that the early removal of sFLC can improve patient's outcome

    Impact of Single Hemodialysis Treatment on immune Cell Subpopulations

    : Hemodialysis (HD) is known to trigger a chronic inflammatory status, affecting the innate and acquired immune response. This study was aimed at a comparative analysis of immune cell subsets, proliferation, and apoptosis in subjects receiving chronic HD treatment with respect to a healthy control. Regardless of the dialysis filter used, we observed a reshaping of the acquired immune component both with respect to healthy patients and between the various sessions of dialysis treatment, with an impairment of CD3 cells, along with an increase in CD4 and CD8 cell populations producing pro-inflammatory factors such as IL-17 and IFN-gamma. The population of B cells, monocytes and NK cells were not impaired by the dialysis procedure. These results confirmed the high impact of the HD treatment on the patient's immune system, underlying the imbalance of T cell counterparts

    KIT/PDGFRA Variant Allele Frequency as Prognostic Factor in Gastrointestinal Stromal Tumors (GISTs): Results From a Multi-Institutional Cohort Study

    Background: The patient selection for optimal adjuvant therapy in gastrointestinal stromal tumors (GISTs) is provided by nomogram based on tumor size, mitotic index, tumor location, and tumor rupture. Although mutational status is not currently used to risk assessment, tumor genotype showed a prognostic influence on natural history and tumor relapse. Innovative measures, such as KIT/PDGFRA-mutant-specific variant allele frequency (VAF) levels detection from next-generation sequencing (NGS), may act as a surrogate of tumor burden and correlate with prognosis and overall survival of patients with GIST, helping the choice for adjuvant treatment. Patients and methods: This was a multicenter, hospital-based, retrospective/prospective cohort study to investigate the prognostic role of KIT or PDGFRA-VAF of GIST in patients with radically resected localized disease. In the current manuscript, we present the results from the retrospective phase of the study. Results: Two-hundred (200) patients with GIST between 2015 and 2022 afferent to 6 Italian Oncologic Centers in the EURACAN Network were included in the study. The receiver operating characteristic (ROC) curves analysis was used to classify "low" vs. "high" VAF values, further normalized on neoplastic cellularity (nVAF). When RFS between the low and high nVAF groups were compared, patients with GIST with KIT/PDGFRA nVAF &gt; 50% showed less favorable RFS than patients in the group of nVAF ≤ 50% (2-year RFS, 72.6% vs. 93%, respectively; P = .003). The multivariable Cox regression model confirmed these results. In the homogeneous sub-population of intermediate-risk, patients with KIT-mutated GIST, the presence of nVAF &gt;50% was statistically associated with higher disease recurrence. Conclusion: In our study, we demonstrated that higher nVAF levels were independent predictors of GIST prognosis and survival in localized GIST patients with tumors harboring KIT or PDGFRA mutations.     Folic Acid and Vitamin B12 Administration in CKD, Why Not?

    Patients affected by chronic kidney disease (CKD) or end-stage renal disease (ESRD) experience a huge cardiovascular risk and cardiovascular events represent the leading causes of death. Since traditional risk factors cannot fully explain such increased cardiovascular risk, interest in non-traditional risk factors, such as hyperhomocysteinemia and folic acid and vitamin B12 metabolism impairment, is growing. Although elevated homocysteine blood levels are often seen in patients with CKD and ESRD, whether hyperhomocysteinemia represents a reliable cardiovascular and mortality risk marker or a therapeutic target in this population is still unclear. In addition, folic acid and vitamin B12 could not only be mere cofactors in the homocysteine metabolism; they may have a direct action in determining tissue damage and cardiovascular risk. The purpose of this review was to highlight homocysteine, folic acid and vitamin B12 metabolism impairment in CKD and ESRD and to summarize available evidences on hyperhomocysteinemia, folic acid and vitamin B12 as cardiovascular risk markers, therapeutic target and risk factors for CKD progression

    The sodium-glucose cotransporter 2 inhibitors (SGLT2i) are a relatively new class of antidiabetic drugs that, in addition to emerging as an effective hypoglycemic treatment, have been shown to improve, in several trials, both renal and cardiovascular outcomes. In consideration of the renal site of action and the associated osmotic diuresis, a negative sodium balance has been postulated during SGLT2i administration. Although it is presumable that sodium and water depletion may contribute to some positive actions of SGLT2i, evidence is far from being conclusive and the real physiologic effects of SGLT2i on sodium remain largely unknown. Indeed, no study has yet investigated how SGLT2i change sodium balance in the long term and especially the pathways through which the natriuretic effect is expressed. Furthermore, recently, several experimental studies have identified different pathways, not directly linked to tubular sodium handling, which could contribute to the renal and cardiovascular benefits associated with SGLT2i. These compounds may also modulate urinary chloride, potassium, magnesium, phosphate, and calcium excretion. Some changes in electrolyte homeostasis are transient, whereas others may persist, suggesting that the administration of SGLT2i may affect mineral and electrolyte balances in exposed subjects.
