    Bakhtin and Dostoevsky shared the conviction that human life must be understood in terms of temporality. Both thinkers were obsessed with time’s relation to life as people experience it. For each, a rich sense of humanity demanded a chronotope of open time. In many respects, the views of Bakhtin and Dostoevsky coincide. Theologically speaking, one could fairly call them both heretics, as we shall see. Their differences reflect their different starting points. Bakhtin began with ethics, whereas Dostoevsky thought about life first and foremost in terms of psychology. For Bakhtin, any viable view of the world had first of all to give a rich meaning to moral responsibility. Dostoevsky could accept no view that was false to his sense of how the human mind thought and felt

    Recovery of methamphetamine associated cardiomyopathy predicted by late gadolinium enhanced cardiovascular magnetic resonance

    Methamphetamine is known to cause a cardiomyopathy which may be reversible with appropriate medical therapy and cessation of use. Late gadolinium enhancement cardiovascular magnetic resonance (CMR) has been shown to identify fibrosis in ischemic and non-ischemic cardiomyopathies. We present a case of severe methamphetamine-associated cardiomyopathy in which cardiac function recovered after 6 months. Evaluation by CMR using late gadolinium enhancement was notable for an absence of enhancement, suggesting an absence of irreversible myocyte injury and a good prognosis. CMR may be useful to predict recovery in toxin-associated non-ischemic cardiomyopathies

    Vascular endothelial function is improved after active mattress use

    Objective: Active mattresses are used to prevent, treat and relieve pressure ulcers (PU) by intermittent contact pressure/relief. However, no studies have directly assessed the vascular endothelial response to long-term active mattress use. This study investigated the hypothesis that eight weeks use of an active mattress would lead to improvements in vascular endothelial function in healthy participants. Methods: Physiological parameters of baseline skin temperature (BskT), resting blood flow (RBF) and endothelial function as measured using post-occlusive reactive hyperaemia (PORH), were assessed at baseline (week 0); following eight weeks of sleeping on an active mattress, and after an eight week washout period (at week 16). Results: We recruited 10 healthy participants (four male, age 52.7±8.5 years, six female age 51.8±17.5 years). Following active mattress use RBF, PORH and BskT at the hallux pulp increased by 336%, 197% and 3.5°C, respectively. Mean values increased from 24.3±38.3 perfusion units to 106.0±100.3 perfusion units (p=0.021) and from 13,456±10,225 to 40,252±23,995 perfusion units x seconds (p=0.003) and from 22.9±2.5°C to 26.4±1.9°C (p<0.001), respectively. Conclusion: Active mattress use for eight weeks leads to significant improvements in RBF, PORH, and BskT. These results suggest that active mattress use can improve endothelial function. Future research is required to explore the potential of active mattress use in the treatment and management of diseases and conditions that would benefit from an improved endothelial function

    Solitary fibrous tumors of the pleura: a clinical-pathological characterization emphasizing changes in lung function

    Solitary fibrous tumors of the pleura (SFTP) represent less than 5% of all pleural tumors with a higher susceptibility for men in the sixth and seventh decades of life. Imaging and histopathological analyses indicate that these tumors have an unpredictable clinical course since larger tumors are often associated with malignancy; however, this assumption is not universal. In addition, there are no systematic descriptions of lung function in either the basal or post-surgical scenarios of SFTP patients. We describe a series of 3 patients between 4th and 5th decade of life with a benign histopathological SFTP without local recurrence in their follow-up. We also analyzed changes in lung function, emphasizing possible associations with clinical recovery

    A Unified Account of the Moral Standing to Blame

    Recently, philosophers have turned their attention to the question, not when a given agent is blameworthy for what she does, but when a further agent has the moral standing to blame her for what she does. Philosophers have proposed at least four conditions on having “moral standing”: 1. One’s blame would not be “hypocritical”. 2. One is not oneself “involved in” the target agent’s wrongdoing. 3. One must be warranted in believing that the target is indeed blameworthy for the wrongdoing. 4. The target’s wrongdoing must some of “one’s business”. These conditions are often proposed as both conditions on one and the same thing, and as marking fundamentally different ways of “losing standing.” Here I call these claims into question. First, I claim that conditions (3) and (4) are simply conditions on different things than are conditions (1) and (2). Second, I argue that condition (2) reduces to condition (1): when “involvement” removes someone’s standing to blame, it does so only by indicating something further about that agent, viz., that he or she lacks commitment to the values that condemn the wrongdoer’s action. The result: after we clarify the nature of the non-hypocrisy condition, we will have a unified account of moral standing to blame. Issues also discussed: whether standing can ever be regained, the relationship between standing and our "moral fragility", the difference between mere inconsistency and hypocrisy, and whether a condition of standing might be derived from deeper facts about the "equality of persons"

    A dearth of small particles in the transiting material around the white dwarfWD 1145+017

    White dwarf WD 1145+017 is orbited by several clouds of dust, possibly emanating from actively disintegrating bodies. These dust clouds reveal themselves through deep, broad, and evolving transits in the star's light curve. Here, we report two epochs of multiwavelength photometric observations of WD 1145+017, including several filters in the optical, Ks and 4.5 μmbands in 2016 and 2017. The observed transit depths are different at these wavelengths. However, after correcting for excess dust emission at Ks and 4.5 μm, we find the transit depths for the white dwarf itself are the same at all wavelengths, at least to within the observational uncertainties of ~5-10 per cent. From this surprising result, and under the assumption of low optical depth dust clouds, we conclude that there is a deficit of small particles (with radii s ≲ 1.5 μm) in the transiting material. We propose a model wherein only large particles can survive the high equilibrium temperature environment corresponding to 4.5 h orbital periods around WD 1145+017, while small particles sublimate rapidly.
