    SSI on the dynamic behavior of a historical masonry building: experimental vs. numerical results

    A reliable procedure to identify the dynamic behaviour of existing masonry buildings is described in the paper, referring to a representative case study: a historical masonry palace located in Benevento (Italy). Since the building has been equipped with a permanent dynamic monitoring system by the Department of Civil Protection, some of the recorded data, acquired in various operating conditions, have been analysed with basic instruments of the Operational Modal Analysis in order to identify the main eigenfrequencies and vibration modes of the structure. The obtained experimental results have been compared to the numerical outcomes provided by three detailed Finite Element (FE) models of the building. The influence of Soil-Structure Interaction (SSI) has been also introduced in the FE model by a sub-structure approach where concentrated springs were placed at the base of the building to simulate the effect of soil and foundation on the global dynamic behaviour of the structure. The obtained results evidence that subsoil cannot a priori be disregarded in identifying the dynamic response of the building

    Uncovering the worth of the past: the relationship between corporate museums' strategic orientation and dual performance

    The main aim of the study is to empirically investigate the relationship between strategic orientation and a dual conceptualization of performance (i.e. non-economic performance and economic performance) in the research context of corporate museums, which are owned and run by private companies. Furthermore, the study aims to explore the nature of the relationship between the dual performance, shedding light on the relevance of non-economic results for this peculiar category of museums. The study is based on survey data from 105 Italian corporate museums, which represent almost the entire population in the country (91%). A structural model was estimated using SmartPLS software in order to examine the direct and indirect effects of strategic orientation on corporate museums' non-economic and economic performance. The findings show that only if corporate museums are able to achieve non-economic performance, creating value for the owning company and the local community, they can also have good results in economic terms. Thus, the non-economic performance acts as a mediator into the relationship between strategic orientation and economic performance. The current work is a pioneer study for the empirical investigation of performance within corporate museums. The empirical model of the study, based on a dual conceptualization of performance and a mediation analysis, is completely innovative in this research context

    High GADA titer increases the risk of insulin requirement in LADA patients: a 7-year follow-up (NIRAD study 7).

    OBJECTIVE: The aim of this study was to determine whether glutamic acid decarboxylase antibody (GADA) titer and other clinical parameters could define the risk of progression to insulin therapy in latent autoimmune diabetes in adults (LADA) patients during a 7-year follow-up. METHODS: This study involved 220 LADA and 430 type 2 diabetes subjects followed up for 7 years from the time of GADA screening to evaluate their progression toward insulin therapy. Kaplan-Meier curves and multivariate logistic regression analysis were performed to identify the markers capable of influencing this progression. RESULTS: During the follow-up, the drop out was 4% in both groups. A total of 119 (56.1%) out of 212 LADA patients required insulin during the 7 years of follow-up. The Kaplan-Meier plots showed that 74/104 (71.1%) of high GADA titer required insulin compared with 45/108 (41.6%) of low GADA titer and with 86/412 (20.9%) of type 2 diabetes (P<0.0001 for both). A BMI of ≤25 kg/m2 and IA-2IC and zinc transporter 8 (ZnT8) positivity were also shown as the markers of faster progression (P<0.0001 for both). The proportion of LADA patients requiring insulin was significantly higher in the group of subjects treated also with sulfonylurea in the first year from diagnosis compared with those treated with diet and/or insulin sensitizers (P<0.001). The multivariate analysis confirmed that the presence of high GADA titer was a significant predictor of insulin requirement (P<0.0001, OR=6.95). CONCLUSIONS: High GADA titer, BMI ≤ 25, ZnT8 and IA-2IC positivity and sulfonylurea treatment, in the first year from diagnosis, significantly increase the progression toward insulin requirement in LADA patients

    General anaesthesia versus local anaesthesia for carotid surgery (GALA): a multicentre, randomised controlled trial

    BACKGROUND: The effect of carotid endarterectomy in lowering the risk of stroke ipsilateral to severe atherosclerotic carotid-artery stenosis is offset by complications during or soon after surgery. We compared surgery under general anaesthesia with that under local anaesthesia because prediction and avoidance of perioperative strokes might be easier under local anaesthesia than under general anaesthesia. METHODS: We undertook a parallel group, multicentre, randomised controlled trial of 3526 patients with symptomatic or asymptomatic carotid stenosis from 95 centres in 24 countries. Participants were randomly assigned to surgery under general (n=1753) or local (n=1773) anaesthesia between June, 1999 and October, 2007. The primary outcome was the proportion of patients with stroke (including retinal infarction), myocardial infarction, or death between randomisation and 30 days after surgery. Analysis was by intention to treat. The trial is registered with Current Control Trials number ISRCTN00525237. FINDINGS: A primary outcome occurred in 84 (4.8%) patients assigned to surgery under general anaesthesia and 80 (4.5%) of those assigned to surgery under local anaesthesia; three events per 1000 treated were prevented with local anaesthesia (95% CI -11 to 17; risk ratio [RR] 0.94 [95% CI 0.70 to 1.27]). The two groups did not significantly differ for quality of life, length of hospital stay, or the primary outcome in the prespecified subgroups of age, contralateral carotid occlusion, and baseline surgical risk. INTERPRETATION: We have not shown a definite difference in outcomes between general and local anaesthesia for carotid surgery. The anaesthetist and surgeon, in consultation with the patient, should decide which anaesthetic technique to use on an individual basis.
