The Association Between Empathy And Prosocial Story Themes And Internalizing Symptoms In Preschool-aged Children
Little is known about the association between empathic and prosocial tendencies (caring) and internalizing psychopathology in young children. Associations between caring and internalizing problems (INT) were examined in young children (N = 63). Children’s caring was measured using a developmentally appropriate story narrative task about mothers in distress; narratives were rated for themes of caring. No general associations between caring and INT were found. A marginally significant, negative quadratic relation between themes of empathy and INT was found in boys (low levels of INT were related to both high and low levels of empathy). Children’s concern reactions were marginally, negatively associated with INT in children of nondepressed mothers. Overall, findings indicate that associations between caring and INT in preschool-aged children are present only under specific conditions, highlighting the importance of gender and maternal psychopathology in elucidating the role of caring in complicated risk cascades that may result in INT
Medical Student Attitudes Toward Cell Phone Use on Rounds
Purpose: Mobile devices are ubiquitous and serve as an important resource for on-the-spot research about patient questions and conditions. However, even when a device is used to address patient needs, patients and members of the medical team may assume the student is using their phone for non-clinical activities and perceive the student as uninterested and disengaged. Medical students are aware of this potential for misperception, and as such, we designed a study to investigate the attitudes of third and fourth year medical students towards cell phone use on internal medicine rounds.
Methods: Our target learner population was third and fourth year medical students on their internal medicine rotations. Voluntary and anonymous surveys generated data in aggregate assessing the attitudes of students towards cell phone use while on rounds.
Results and Conclusions: The results of our study show that medical students have a strong desire to use smart phones for professional reasons on rounds. The data also show that despite this desire, students do not feel comfortable accessing their technology for fear of professional repercussions from both attendings and patients. An overwhelming 75% of students reported that they would use their smart phones more frequently if formal guidelines were in place encouraging professional cell phone use. We hope to develop an intervention to eliminate the fear of professional repercussions medical students currently experience as digital learners using smart phones. This intervention would seek to bolster medical education and patient care by adding transparency to cell phone use between medical students, attendings and the patients they treat
Children\u27s Empathy Responses and their Understanding of Mother\u27s Emotions
This study investigated children\u27s empathic responses to their mother\u27s distress to provide insight about child factors that contribute to parental socialization of emotions. Four- to six-year-old children (N=82) observed their mother\u27s sadness and anger during a simulated emotional phone conversation. Children\u27s facial negative affect was rated and their heart rate variability was recorded during the conversation, and their emotion understanding of the conversation was measured through their use of negative emotion words and perspective-taking themes (i.e., discussing the causes or resolution of mother\u27s emotions) in narrative accounts of the conversation. There were positive quadratic relationships between HRV and ratings of facial affect, narrative references to mother\u27s negative emotions, and perspective-taking themes. High and low HRV was associated with high facial negative affect, suggesting well-regulated sympathy and poorly regulated personal distress empathic responses, respectively. Moderate HRV was associated with low facial negative affect, suggesting minimal empathic engagement. High and low HRV were associated with the highest probabilities of both emotion understanding indicators, suggesting both sympathy and personal distress responses to mother\u27s distress facilitate understanding of mother\u27s emotions. Personal distress may motivate attempts to understand mother\u27s emotions as a self-soothing strategy, whereas sympathy-related attempts to understand may be motivated by altruism
Quadratic associations between empathy and depression and the moderating influence of dysregulation
Empathic tendencies have been associated with interpersonal and psychological benefits, but empathy at extreme levels or in combination with certain personal characteristics may contribute to risk for depression. This study tested the moderating role of cognitive emotion regulation in depression’s association with empathy using nonlinear models. Young adults (N=304; 77% female; M=19 years) completed measures of cognitive emotion regulation strategies, depression, and affective and cognitive empathy. Individuals with good regulation had low levels of depression overall and their depression symptoms were lowest when levels of affective empathy were average. Individuals with poor regulation had high levels of depression overall, particularly when levels of empathy were moderate to high. Extremely high and low levels of cognitive empathy were associated with elevated depression, and this association was not moderated by regulation. These findings suggest tendencies to respond empathically to others’ needs is neither an adaptive nor maladaptive characteristic but rather moderate empathy, particularly in the context of good regulation, may offer the greatest protection against depression
Stress, Motivation and Professional Satisfaction among Health Care Workers in HIV/AIDS Care and Treatment Centers in Urban Tanzania: A Cross-Sectional Study.
Shortages of health care workers (HCWs) represents a serious challenge to ensuring effective HIV care in resource-limited settings (RLS). Stress, motivation, and job satisfaction have been linked with HCW retention and are important in addressing HCW shortages. In this cross-sectional study HCW stress, motivation and perceived ability to meet patient needs were assessed in PEPFAR-supported urban HIV care and treatment clinics (CTCs) in Tanzania. A self-administered questionnaire measuring motivation, stress, and perceived ability to and meet patient needs was given to HCWs at 16 CTCs. Scales measuring HCW satisfaction, motivation, and stress were developed using principle components analysis. Hierarchical linear models were used to explore the association of HCW and site characteristics with reported satisfaction, stress, motivation, and ability to meet patients' needs.\ud
Seventy-three percent (279) of HCWs completed the questionnaire. Most (73%) HCWs reported minimal/no work-related stress, with 48% reporting good/excellent motivation, but 41% also reporting feeling emotionally drained. Almost all (98%) reported feeling able to help their patients, with 68% reporting work as rewarding. Most reported receipt of training and supervision, with good availability of resources. In the multivariate model, direct clinical providers reported lower motivation than management (p < 0.05) and HCWs at medium-sized sites reported higher motivation than HCWs at larger sites (p < 0.05). HCWs at small and medium sites were more likely to feel able to help patients than those from larger sites (p < 0.05 and p < 0.001 respectively). Despite significant patient loads, HCWs in these PEPFAR-supported CTCs reported high levels of motivation, job satisfaction, ability to meet patients' needs, low levels of stress but significant emotional toll. Understanding the relationship between support systems such as strong supervision and training and these outcomes is critical in designing interventions to improve motivation, reduce stress and increase retention of HCWs
Readability of Colorectal Cancer Online Information: A Brief Report
Purpose: Colorectal cancer (CRC) is the third leading cause of cancer-related death in the United States. A decline in deaths caused by CRC has been largely attributable to screening and prompt treatment. Motivation, shown to influence cancer-related screening and treatment decisions, can be shaped by information from the Internet. The extent to which this information is easily readable on cancer-related websites is not known. The purpose of this study was to assess the readability levels of CRC information on 100 websites.
Methods: Using methods from a prior study, the keyword, “colorectal cancer,” was searched on a cleared Internet browser. Scores for each website (n = 100) were generated using five commonly recommended readability tests.
Results: All five tests demonstrated difficult readability for the majority of the websites.
Conclusions: Online information related to CRC is difficult to read and highlights the need for developing cancer-related online material that is understandable to a wider audience
Mediation of the Translocation of nNOSÎĽ During Unloading-Induced Atrophy of Skeletal Muscle via NOX2 Inhibition
Mechanical unloading results in detachment of the mu-splice variant of neuronal nitric oxidase synthase (nNOSμ) from the dystrophin-glycoprotein complex and sarcolemma and translocation to the cytosol. We recently found that reactive oxygen species (ROS) play a role in nNOSμ translocation during unloading and muscle atrophy. NOX2, an isoform of NADPH oxidase and source of ROS, may play a causal role in nNOSμ translocation. The purpose of the study was to determine the effectiveness of NOX2 peptidyl inhibition in reducing the translocation of nNOSμ from the sarcolemma and subsequently soleus CSA. Adult male Fisher 344 rats were randomly assigned to one of three groups: CON (control), HU-S (hind limb unloaded with gp91ds-tat scramble) and HU-G (hind limb unloaded with gp91ds-tat). The hind limb unloading period was 7 days. Mean body weights for CON (353.26 g ± 15.47), HU-S (305.14 g ± 18.18) and HU-G (306.34 g ± 16.84) at the beginning of the experiment were not significantly different. Muscle mass/body mass ratio for the gastrocnemius complex (gastrocnemius, plantaris and soleus) was significantly reduced in HU-S rats (10.08 mg/g ± 0.24) but was maintained in HU-G rats (10.88 mg/g ± 0.47). SMASH analysis revealed that average soleus CSA in HU-G rats (3293.08 μm2 ± 46.82) decreased significantly less than HU-S rats (2606.66 μm2 ± 33.46) compared with ambulatory controls (p \u3e 0.0001). Immunofluorescence and staining of nNOS activity with NADPH Diaphorase of soleus tissue showed considerable loss of sarcolemmal nNOSμ in the HU-S group while the HU-G group revealed substantial maintenance of nNOSμ at the sarcolemma. The results of this study suggest that NOX2 inhibition via gp91ds-tat is effective in reducing the translocation of nNOSμ from the sarcolemma to the cytosol and maintaining CSA of the soleus with mechanical unloading via the inhibition of NOX2

Observation operators for assimilation of satellite observations in fluvial inundation forecasting
Images from satellite-based synthetic aperture radar (SAR) instruments contain large amounts of information about the position of flood water during a river flood event. This observational information typically covers a large spatial area, but is only relevant for a short time if water levels are changing rapidly. Data assimilation allows us to combine valuable SAR derived observed information with continuous predictions from a computational hydrodynamic model and thus to produce a better forecast than using the model alone. In order to use observations in this way a suitable observation operator is required. In this paper we show that different types of observation operator can produce very different corrections to predicted water
levels; this impacts on the quality of the forecast produced.We discuss the physical mechanisms by which different observation operators update modelled water levels and introduce a novel observation operator for inundation forecasting. The performance of the new operator is compared in synthetic experiments with that of two more conventional approaches. The conventional approaches both use observations of water levels derived from SAR to correct model predictions. Our new operator is instead 10
designed to use backscatter values from SAR instruments as observations; such an approach has not been used before in an ensemble Kalman filtering framework. Direct use of backscatter observations opens up the possibility of using more information from each SAR image and could potentially speed up the time taken to produce observations needed to update model predictions. We compare the strengths and weaknesses of the three different approaches with reference to the physical mechanisms by which each of the observation operators allow data assimilation to update water levels in synthetic twin experiments in an idealised domain
Genetic and environmental influences on affiliation with deviant peers during adolescence and early adulthood
Adolescence and early adulthood is a time when peer groups become increasingly influential in the lives of young people. Youth exposed to deviant peers risk susceptibility to externalizing behaviors and related psychopathology. In addition to environmental correlates of deviant peer affiliation, a growing body of evidence suggests that affiliation with deviant peers is heritable. This study examined the magnitude of genetic and environmental influences on affiliation with deviant peers, changes in the relative importance of these factors, and which of these factors contribute to the stability of affiliation across this critical developmental period using a longitudinal twin study design that assessed same-sex twins (485 monozygotic pairs, 271 dizygotic pairs) at three discrete ages-15-, 18-, and 21-years-old. Biometric models revealed that genetic influences increased with age. New genetic influences appeared during late adolescence, and no new genetic influences emerged by age 21. Environmental influences shared by sibling pairs decreased with age, while the proportion of nonshared environmental effects unique to each individual remained relatively stable over the course of development. Shared environmental influences were largely age-overlapping whereas nonshared environmental influences were largely age-specific. In summary, this study found variance in affiliation with deviant peers is explained by shared and nonshared environment effects as well as by genetic influences (46% by age 21), supporting the role of genetically influenced selection factors. The shared environment was almost exclusively responsible for the stability in late adolescence, while genetic influences were primarily responsible for stability in early adulthood

Observation impact, domain length and parameter estimation in data assimilation for flood forecasting
Accurate inundation forecasting provides vital information about the behaviour of fluvial flood water. Using data assimilation with an Ensemble Transform Kalman Filter we combine forecasts from a numerical hydrodynamic model with synthetic observations of water levels. We show that reinitialising the model with corrected water levels can cause an initialisation shock and demonstrate a simple novel solution. In agreement with others, we find that although assimilation can accurately correct water levels at observation times, the corrected forecast quickly relaxes to the open loop forecast. Our new work shows that the time taken for the forecast to relax to the open loop case depends on domain length; observation impact is longer-lived in a longer domain. We demonstrate that jointly correcting the channel friction parameter as well as water levels greatly improves the forecast. We also show that updating the value of the channel friction parameter can compensate for bias in inflow
