Séptima Lección Jesús Culebras. Respuesta inflamatoria sistémica y disfunción/ fracaso multiorgánico tras una agresión: implicaciones metabólicas
Following any aggression, the body starts an infl ammatory response, mediated by humoral and cellular factors, intended to limit the process and eventually to heal. In some cases, either due to the intensity or the duration of the aggression or due to an inadequate response of the host, secondary to genetic polymorphisms, malnutrition or other causes, a state of hyper activation of infl ammatory cells is originated, with liberation of immature cells and activation of monocytes and macrophages, which liberate very powerful pro infl ammatory mediators that induce a state of generalized systemic inflammation. Many processes may originate this infl ammatory response, (sepsis, trauma, burns, pancreatitis, etc.) with activation of leukocyte, endothelial, coagulation and neuroendocrine response systems, generating a complex of mediators (cytokines, adhesion molecules, growth factors, etc.). Clinically, the response is characterized by inflammation, anorexia, stillness, increase of vascular permeability, factors that originate edema and vasodilatation, which is followed by hypotension, tachycardia and increased cardiac output. On the other hand, the metabolic response to stress is part of the mechanism of adaptation, generated by the organism in order to survive the acute disease by means of increasing energetic substrates to vital tissues. As a result of this complex metabolic response, the control of substrate utilization is only partially regulated because, being impaired the mechanisms of energy supply, the organism seeks alternative substrates.Ante cualquier agresión, el organismo pone en marcha una respuesta inflamatoria, mediada por factores humorales y celulares, que tiende a

con liberación de células inmaduras y activación de monocitos y macrófagos, que liberan poderosos mediadores proinflamatorios e inducen
un estado de inflamación sistémica generalizada. Numerosos procesos pueden dar lugar a esta respuesta inflamatoria (sepsis, traumatismos,
quemaduras, pancreatitis, etc.) con activación de los sistemas leucocitarios, endoteliales, de la coagulación y de la respuesta neuroendocrina,
lo que genera un complejo entramado de mediadores (citocinas, moléculas de adhesión y factores de crecimiento, entre otros). Clínicamente, la
respuesta se caracteriza por inflamación, anorexia, inmovilidad, aumento en la permeabilidad vascular que condiciona la aparición de edema,
vasodilatación que se acompaña de hipotensión, taquicardia e incremento del gasto cardiaco. Por otro lado, la respuesta metabólica al estrés
forma parte de ese mecanismo de adaptación, que el organismo genera para sobrevivir a la enfermedad aguda aumentando el aporte de sustratos
energéticos a los tejidos vitales. Como resultado de esa compleja respuesta metabólica, el control de la utilización de los sustratos energéticos
esta solo parcialmente regulado por su disponibilidad, debido a que al estar alterados los mecanismos de producción energética, el organismo busca sustratos alternativo
Aging and hyponutrition; A challenge for the sustainability of the NHS: Conclusions of the 9th abbot-SENPE debate forum
La desnutrición es un problema común en todos los
niveles de atención sanitaria, desde atención primaria a
especializada y en centros de atención geriátrica. Su incidencia
en los hospitales es del 40% y en las residencias de
mayores supera el 60%. Esto último es de gran importancia,
tienen una alta relación con el progresivo envejecimiento
de la población europea, y es la mayor y más
frecuente causa de discapacidad en la población anciana
que vive en su domicilio o en instituciones.
Países como Holanda, Dinamarca o el Reino Unido han
desarrollado Planes Estratégicos Integrales para luchar
contra la desnutrición desarrollando e implantando guías,
estableciendo cribados obligatorios en los ingresos y altas
hospitalarias, en las residencias en ancianos, etc. En nuestro
país, en una acción conjunta de SENPE y la Fundación
Abbott, estamos desarrollando un Plan Estratégico Integral
(Proyecto + nutridos) en el que establecemos recomendaciones
claras, precisas y validadas para efectuar cribaje
nutricional tanto en pacientes hospitalizados, como en los
institucionalizados y en los mayores ambulatorios.
En este tema deben de ser tenidos en cuenta los
aspectos sociales y financieros. La desnutrición es con
mucha frecuencia deficientemente reconocida y tratada.
Ello tiene un impacto negativo sobre los pacientes individuales
en términos de morbilidad, mortalidad, independencia
y calidad de vida, y sobre los sistemas de cuidado
sanitario en términos de uso de recursos y costesHyponutrition is a common problem at all health care
levels, from primary to specialized care, as well as in geriatric
care. Its incidence in a hospital setting is 40% and 60%
in nursing homes. This is very important, it is highly related
with progressive aging of the European population, and is
the biggest and most frequent cause of disability among the
elderly population living at home or institutions.
Countries such as Holland, Denmark, or the United
Kingdom have developed Comprehensive Strategic Plans
to fight against hyponutrition by developing and implementing
guidelines, establishing mandatory screenings at the
moment of hospital admission and discharge, at nursing
homes, etc. In our country, a combined action of SENPE
and Abbott Foundation is developing a Comprehensive
Strategic Plan (+ nutridos Project) in which clear, precise,
and validated recommendations are established to perform
nutritional screening both in hospitalized patients and institutionalized
and ambulatory elderly people.
This issue has to take into account the social and financial
aspects. Hyponutrition is many times insufficiently
recognized and treated. This has a negative impact on the
individual patient in terms of morbidity, mortality, independence,
and quality of life, as well as on the health care
systems in terms of use of health care resources and cost
Training model for integral treatment of patients with eating disorders resistant to change
La necesidad de buscar tratamientos eficaces para los
pacientes con Anorexia o Bulimia nerviosa ha inducido a
los profesionales que les atienden a elaborar nuevas formas
de tratamiento que tengan en cuenta las variables
causantes de su resistencia al cambio. Los pacientes del
presente estudio (2006-2009) presentan las siguientes
características: 340 pacientes que llevan 7 o más años de
evolución y/ o han intentado numerosos tratamiento previos
sin que se haya logrado iniciar y/o mantener los cambios
deseados, que les hayan permitido recuperarse de
forma estable. En cuanto al tratamiento propuesto, la
relación paciente-equipo terapéutico se apoya en los principios
del modelo de capacitación. Éste, considera al
paciente de forma integral, le informa y le provee de
recursos para que incremente su compromiso de cambio.
Enseña al paciente a cuidarse física y psíquicamente
como medio de recuperar su salud y salir del trastorno de
forma estable. Incluye a los familiares como apoyo esencial
en la recuperación de su allegado/a. Los terapeutas
requieren amplia experiencia en el tratamiento de TCA,
flexibilidad, capacidad para integrarse con otros compañeros
aunque utilicen modelos teóricos diferentes, habilidades
para realizar sesiones grupales, capacidad para
manejar emociones negativas y tolerancia a la frustración.
Finalmente, el modelo que a continuación se presenta
se ha puesto en práctica, ha recuperado a pacientes
cuya permanencia en el trastorno superaba 15 años de
evolución y laboralmente tenían la invalidez permanenteThe need to find effective treatments for patients with
Anorexia or Bulimia nervosa has led to the professionals
who care for them to develop new forms of treatment that
take into account the variables that cause resistance to
change. Patients in this study (2006-2009) have the following
characteristics: 340 patients who have 7 or more
years of evolution and/or have tried numerous previous
treatments without having succeeded in starting and / or
maintaining the desired changes, that allowed them to
recover steadily. As the proposed treatment, the patienttreatment
team is based on the principles of the training
model. It considers the patient holistically, it informs and
provides him with resources to increase its commitment
to change. Teaches the patient to take care physically and
mentally as a way to regain their health and leave the
disorder in a stable way. Includes family members as
essential support in the recovery of their closest. Therapists
require extensive experience in the treatment of ED,
flexibility, ability to integrate with other team members
even if they use different theoretical models, skills for
group sessions, ability to handle negative emotions and
frustration tolerance. Finally, the model presented below

the disorder exceeded 15 years of development and led to

All for One and One for All: Voluntary Physicians in the Intensive Medicine Units During the COVID-19 Outbreak in Spain
COVID-19; Intensive care medicine; Hospital organizationCOVID-19; Medicina intensiva; Organització hospitalàriaCOVID-19; Medicina Intensiva; Organización hospitalariaObjectives: Our purpose was to determine the intensive care units' (ICU's) medical staff surge capacity during the coronavirus disease 2019 (COVID-19) outbreak in Spring 2020 in Spain.
Methods: A multicenter retrospective survey was performed addressing the medical specialties present in the ICUs and the increase in bed capacity during this period.
Results: Sixty-seven centers (62.04%) answered the questionnaire. The ICU bed capacity during the pandemic outbreak increased by 160% (95% confidence interval [CI], 128.97-191.03%). The average number of beds per intensive care medicine (ICM) specialist was 1.5 ± 0.60 and 3.71 ± 2.44 beds/specialist before and during the COVID-19 outbreak, respectively. Non-ICM specialists and residents were present in 50 (74.63%) and 23 (34.3%) ICUs during the outbreak, respectively. The number of physicians (ICM and non-ICM residents and specialists) in the ICU increased by 89.40% (95% CI, 64.26114.53%). The increase in ICM specialists was, however, 4.94% (95% CI, -1.35-11.23%). Most non-ICM physicians were anesthetists, followed by pediatricians and cardiologists.
Conclusions: The majority of ICUs in our study were able to rapidly expand critical care capacity by adapting areas outside of the normal ICU to manage critically ill patients, and by extending the critical care staff with noncritical care physicians working as force multipliers
Una emulsión lipídica basada exclusivamente en aceite de pescado revierte la colestasis
Prolonged parenteral nutrition (PN) leads to liver damage.
Recent interest has focused on the lipid component
of PN. A lipid emulsion based on w-3 fatty acids decrease
conjugated bilirubin. A mixed lipid emulsion derived
from soybean, coconut, olive, and fish oils reverses jaundice.
Here we report the reversal of cholestasis and the improvement
of enteral feeding tolerance in 1 infant with
intestinal failure-associated liver disease. Treatment involved
the substitution of a mixed lipid emulsion with one
containing primarily omega-3 fatty acids during 37 days.
Growth and biochemical tests of liver function improved
significantly. This suggests that fat emulsions made from
fish oils may be more effective means of treating this
condition compared with an intravenous lipid emulsion
containing soybean oil, medium -chain triglycerides, olive
oil, and fish oilLa nutrición parenteral prolongada produce daño hepático.
Recientemente se ha comunicado el efecto de las
emulsiones lipídicas intravenosas basadas exclusivamente
en ácidos grasos omega-3 en la resolución de la colestasis.
Lo mismo se ha observado con el uso de emulsiones
lipídicas mixta derivadas del aceite de soja, coco, oliva y
pescado.
Comunicamos la desaparición de colestasis y mejoría
de la tolerancia enteral en un niño con enfermedad
hepática asociada a nutrición parenteral. El tratamiento
consistió en sustituir una emulsión lipídica mixta por
otra que contenía de forma exclusiva aceite de pescado
durante 37 días. El crecimiento y los datos bioquímicos
de función hepática mejoraron de forma significativa.
Este caso sugiere que emulsiones lipídicas intravenosas a
partir de aceite de pescado pueden ser mas eficaces para
tratar la colestasis si se comparan con emulsiones mixta
Dispositivo de fijación, cierre y acoplamiento para catéter de perfusión intravenosa
Introducción: los medios de fijación a la piel de un catéter
venoso periférico, de un catéter venoso central o de
un catéter arterial central presentan los siguientes graves
inconvenientes: ser la sede de infecciones a nivel del lugar
de la punción, provocar una saturación de la zona anatómica,
la cual resulta muy difícil de soportar e incómodo
para el paciente portador, y suponer un riesgo para el
personal de enfermería cuando se usan hilos de sutura
para fijar el soporte del catéter.
Objetivo: por lo anteriormente expuesto, se ha detectado
la necesidad de diseñar un dispositivo que reduzca la
complejidad y aparatosidad de los sistemas conocidos y
empleados en la actualidad, favoreciendo la asepsia y la
movilidad del paciente.
Método: el desarrollo presentado en este artículo se refiere
a un sistema mecánico de fijación, cierre y acoplamiento
para catéter de perfusión intravenosa. El sistema dispone
de una pieza circular de pequeño espesor con al menos dos
orificios para su fijación subcutánea, centralmente de uno
o más conductos para acoplar lúmenes, y de una parte cilíndrica
hueca que sobresale a uno de los lados, exteriormente
roscada e interiormente lisa con un tetón de posición. Para
uso domiciliario se dispone de un tapón de estanqueidad
con uno o más tubos de pequeño diámetro que se acoplan
en los orificios para lúmenes, una ranura de posición, un
tirador y un tapón roscado ciego para el cierre hermético
con la parte roscada. Para uso hospitalario se dispone de
un tapón intermedio con una o más entradas para lúmenes,
así como de un tapón roscado hueco que permite su acoplamiento
hermético con la parte roscada.
Resultados: el desarrollo descrito en este artículo va a
ser utilizado en dos ambientes: ambiente domiciliario y
ambiente hospitalario. Además, reduce el riesgo de infección
y saturación de la zona anatómica de los pacientes

Objective: to develop a fixing device that simplifies

Methods: the device herein described is composed by
a mechanical fixation, a closing system and coupling for
intravenous catheters. The system has a thin circular piece
with at least two holes for subcutaneous fixation, one
or several conducts for lumina and a hollow cylindrical
part in one side, screwed exteriorly and flat inside, with
an oriented protuberance. A watertight plug with one or
several thin tubes that adapt to the lumina, a positional
slot, a handle and a solid screwed tap for perfect closure
are available for at home use. An intermediate plug with
one or several lumina and a screwed hollow plug are provided
for in hospital use.
Results: the above described device is intended to be
used in two settings: in hospital and at home. It is supposed

Asthma, obesity and diet
El asma y la obesidad son dos trastornos de gran
impacto en la salud pública que han aumentado su prevalencia
en los últimos años. Numerosos estudios han relacionado
ambas entidades.
La mayoría de los estudios prospectivos demuestran
que la obesidad es un factor de riesgo para el diagnóstico
“de novo” de asma. Además, los resultados de diversos
estudios sugieren que así como la ganancia de peso
aumenta el riesgo de asma, la pérdida mejora su evolución.
En general, los estudios prospectivos encuentran
una asociación positiva entre el índice de masa corporal
(IMC) basal y el posterior desarrollo de asma, lo que
sugiere que es el exceso de peso el que podría favorecer el
desarrollo de asma, aunque estos resultados no son tan
concluyentes cuando se estudia la asociación entre hiperreactividad
bronquial con el IMC.
Existen distintos factores que podrían explicar esta
asociación. La obesidad es capaz de reducir la compliance
pulmonar, los volúmenes pulmonares y el diámetro de de
las vías respiratorias periféricas, así como alterar los
volúmenes sanguíneos pulmonares y la relación ventilación-
perfusión. Además, el aumento del funcionamiento
normal del tejido adiposo en sujetos obesos conduce a un
estado proinflamatorio sistémico, que produce un
aumento de las concentraciones séricas de numerosas
citoquinas, fracciones solubles de sus receptores y quimiocinas.
Muchos de estos mediadores son sintetizados y
secretados por células del tejido adiposo y reciben el nombre
genérico de adipocinas, entre las que se incluyen IL-6,
IL-10, eotaxina, TNF- , TGF- 1, PCR, leptina y adiponectina.
Por último, se han identificado regiones específicas
del genoma humano que están relacionadas tanto con
el asma como con la obesidad.
La mayoría de los estudios apuntan a que la obesidad
es capaz de aumentar la prevalencia y la incidencia de
asma, aunque este efecto parece ser moderado. El tratamiento
de los asmáticos obesos debe incluir un programa
de control de peso.Asthma and obesity have a considerable impact on
public health and their prevalence has increased in recent
years. Numerous studies have linked both disorders.
Most prospective studies show that obesity is a risk factor
for asthma and have found a positive correlation between
baseline body mass index (BMI) and the subsequent
development of asthma, although these results are not
conclusive when studying the association between airway
hyperresponsiveness with BMI. Furthermore, several
studies suggest that whereas weight gain increases the
risk of asthma, weight loss improves the course of the illness.
Different factors could explain this association. Obesity
is capable of reducing pulmonary compliance, lung
volumes and the diameter of peripheral respiratory airways
as well as affecting the volume of blood in the lungs
and the ventilation-perfusion relationship. Furthermore,
the increase in the normal functioning of adipose tissue in
obese subjects leads to a systemic proinflammatory state,
which produces a rise in the serum concentrations of several
cytokines, the soluble fractions of their receptors and
chemokines. Many of these mediators are synthesized
and secreted by cells from adipose tissue and receive the
generic name of adipokines, including IL-6, IL-10,
eotaxin, TNF- , TGF- 1, PCR, leptin y adiponectin.
Finally, specific regions of the human genome which are
related to both asthma and obesity have been identified.
Most studies point out that obesity is capable of
increasing the prevalence and incidence of asthma,
although this effect appears to be modest. The treatment

Consenso multidisciplinar sobre el abordaje de la desnutrición hospitalaria en España
Rationale: Disease-related malnutrition constitutes a
highly prevalent healthcare problem with high costs associated.
In Spain, the prevalence of malnutrition in hospitalized
patients has been reported from 30% to 50%.
Objectives: Main purposes of this consensus document
were to establish recommendations that facilitate decision-
making and action to prevent and early-diagnose
disease-related hospital malnutrition, on the management
of nutritional support methods and actions to evaluate
nutritional treatment compliance and efficacy.
Methods: A systematic bibliographical search of authors
was performed, complemented by updated bibliography
by author references up to 2010. From this review, some
recommendations were defined, modified and critically
evaluated by the representatives of scientific societies in a
consensus conference (Dec 2010) following a structured
brainstorming technique: the Metaplan® technique. A
double validation process was undertaken until final recommendations
were obtained.
Results: 30 consensus recommendations for the prevention
and management of hospital malnutrition are
presented in this document. Recommendations cover all
clinical care settings as well as prevention, screening,
diagnosis, treatment and follow-up of disease-related
malnutrition.
Conclusions: Nutritional screening is strongly recommended
at all clinical settings when nutritional risk factors
are identified or there is clinical suspicion of malnutrition.
Nutritional assessment should be designed and
performed according to centers’ resources, but clearly
identified protocols should be availableLa desnutrición relacionada con la enfermedad constituye
un problema sanitario de elevada prevalencia y altos
costes. En España, la prevalencia de desnutrición de los
pacientes hospitalizados se ha estimado entre el 30% y el
50%.
Objetivos: El objetivo principal de este consenso fue
establecer recomendaciones para facilitar la toma de
decisiones para la prevención y el diagnostico precoz de la
desnutrición hospitalaria, el manejo del soporte nutricional,
y las acciones para evaluar el cumplimiento de la
intervención nutricional y su eficacia.
Métodos: Se realizó una búsqueda sistemática de autor
complementada por bibliografía actualizada por referencias
de autor hasta el año 2010. A partir de esta revisión,
se definieron algunas recomendaciones que fueron criticadas
y modificadas por los representantes de las Sociedades
Científicas participantes en una conferencia de
consenso (Diciembre 2010) siguiendo una técnica de
brainstorming estructurado: la técnica Metaplan®. Se
realizaron dos vueltas de validación de las recomendaciones
hasta obtener las recomendaciones finales.
Resultados: Este documento presenta 30 recomendaciones
para la prevención y el manejo de la desnutrición
hospitalaria. Las mismas cubren todas las áreas de actuación
clínica así como la prevención, cribado, diagnóstico,
tratamiento y seguimiento de la desnutrición hospitalaria
relacionada con la enfermedad.
Conclusiones: Se recomienda enérgicamente el cribado
nutricional en todas las áreas de actuación clínica cuando
se identifiquen factores de riesgo nutricional o sospecha
clínica de desnutrición. La valoración del estado nutricional
debe diseñarse y realizarse de acuerdo a los recursos
disponibles en cada centro, disponiendo de claros protocolos
de actuació
