    Determining Factors of Periprosthetic Infection: A Proposal for a Cross-Cutting Study in An Ecuadorian Referral Hospital

    The objective of this research was to determine the factors associated with the development of periprosthetic infection in patients undergoing prosthetic joint replacement. A descriptive and retrospective study was conducted with 478 patients who underwent surgery at a Specialty Hospital in Quito, Ecuador, between January 2010 and December 2015. The results showed that 3.8% of the patients presented periprosthetic infection, and a significant association was found between infection and urinary tract infection and smoking. In addition, the National Nosocomial Infections Surveillance Index (NNIS) showed that patients with moderate-high risk had a 7.47 times higher risk of infection. In conclusion, there are modifiable factors associated with periprosthetic infection, and the NNIS index can provide a quick and easy estimate of the risk of developing this complication

    Recent Advances in the Diagnosis and Management of Choledochal Cysts

    Aim: Choledochal cysts are a rare congenital anomaly in which the intra- and/or extrahepatic bile ducts are abnormally dilated. The aim of this study is to provide an updated review on the diagnosis and treatment of these cysts. Material and method: A literature search was performed in Medline and Scopus databases to select relevant articles, obtaining a total of 38 articles. An association has been observed with advanced maternal age, overweight of the newborn and birth after the estimated delivery date. Statistics and Result: The recommended treatment is resection of the cysts to restore the normal flow of bile into the intestine. Although the prognosis is generally favorable, the risk of malignancy should be kept in mind and periodic check-ups should be performed to detect any signs of malignancy

    Aim: The prosthetic joint replacement procedure is a significant advance in medicine, improving functionality and quality of life in patients with arthropathies. Although it offers many benefits, it is exposed to complications, periprosthetic infection being one of the most worrisome. This infection affects the patient's quality of life; therefore, it is crucial to identify the risk factors associated with this complication. Material and method: The objective of this study is to determine the factors linked to the development of this infection in patients undergoing prosthetic joint replacement. A descriptive and retrospective approach was used, analyzed with descriptive statistics, frequency, percentage and chi square, using the SPSS system ver 2.0. Patients operated on in a prestigious Specialty Hospital in the city of Quito, Ecuador, from January 2010 to December 2015 were analyzed. The study included 478 patients, 62.6% being women, with a mean age of 70.3 years. Statistics and Result: Hip arthroplasty was the most frequent intervention (59.2%). The overall incidence of periprosthetic infection was 3.8% (n=18). During the analysis, urinary tract infection and smoking were identified as being associated with periprosthetic infection. The NNIS index showed that patients with moderate-high risk are 7.47 times more likely to develop infections

    Respuesta al tratamiento en pacientes con enfermedad inflamatoria intestinal y su calidad de vida en la ciudad de Quito, Cuenca y Guayaquil en el periodo enero – abril 2018

    La enfermedad inflamatoria intestinal (EII), comprende un grupo de padecimientos inflamatorios crónicos cuyos principales representantes son: la colitis ulcerativa (CU) y la enfermedad de Crohn (EC). La etiología exacta es desconocida, pero se cree que tiene origen multifactorial. Objetivos: Determinar la respuesta al tratamiento, en EII y su calidad de vida. Tipo de estudio: Se realizó un estudio analítico multicéntrico. Muestra: 206 pacientes ecuatorianos, con diagnóstico de EII, procedentes del servicio de gastroenterología de los hospitales: Carlos Andrade Marín, José Carrasco Arteaga y Teodoro Maldonado. Materiales y métodos: Previa autorización de los comités de ética de cada hospital, se obtuvieron los datos de los pacientes con diagnóstico de EII y, un grupo de estos pacientes (34), luego de firmar del consentimiento informado, realizaron el test IBDQ-32 para medir calidad de vida. Plan de análisis: Mediante el programa SPSS V24. Se realizó análisis descriptivo, calculando medidas de tendencia central, dispersión y asociación. Resultados: Se evaluaron 206 pacientes. Encontrándose, 148 pacientes para CU y 58 para EC. La relación CU – EC, fue 2.5 -1. Ambas fueron más frecuentes en el sexo masculino. La edad promedio de diagnóstico para las dos variedades fue de 42 años. El tratamiento predominante en CU, fue 5ASA, con un 54% y, en EC, la terapia biológica, con el 45% de los casos. La calidad de vida, es percibida como baja en: el sexo femenino, EC y aquellos con recaídas. 13 Conclusiones: La EII, ha presentado en nuestro pais un incremento similar, al reportado en otros países sudamericanos. Las tasas de respuesta al tratamiento, se encuentran en el rango esperado según la literatura mundial. Recomendaciones: Se sugiere, la elaboración de protocolos de manejo, aplicables a nuestra realidad sanitaria, establecer objetivos, en relación al tiempo de diagnóstico y la inclusión de la calidad de vida, en la valoración integral de éstos pacientes
