    Prevalência da cárie dentária em crianças de 3 e 5 anos residentes em um município brasileiro de pequeno porte

    Despite the reduction in the prevalence of dental caries, very little is known about the prevalence of the disease, especially in deciduous teeth, in small Brazilian cities (Apesar do declínio da prevalência da cárie dentária, muito pouco se conhece sobre a prevalência da doença, especialmente na dentadura decídua, em municípios brasileiros de pequeno porte (< 20.000 habitantes). Este estudo teve por objetivo avaliar a prevalência da doença, em crianças de 3 e 5 anos de idade, do município de Cambira - PR. Foram examinadas todas as crianças que participaram da campanha nacional de vacinação contra poliomielite em 2002 nas duas unidades básicas de saúde do município. A cobertura da campanha foi de 99,5% na população de 1 a 5 anos. Dois cirurgiões dentistas previamente treinados e calibrados realizaram os exames clínicos após a escovação dentária das crianças e registraram a experiência de cárie através do índice ceo. Os resultados mostraram não haver diferença estatisticamente significante na prevalência da doença nos gêneros masculino e feminino e entre zona rural e urbana. Aos 3 anos, 68,50% das crianças estavam livres de cárie e 31,10% aos 5 anos de idade. Na população total, 11,19% das crianças apresentavam 50,86% dos dentes cariados encontrados. O índice ceo, aos 3 e 5 anos na população estudada foi, respectivamente 2,10 e 3,51 . Estes dados mostram que o município ainda não atingiu as metas propostas pela O.M.S. para o ano 2.000, o que indica a necessidade de um maior investimento nos programas destinados à prevenção e controle da doença

    Avaliação de sistemas cerâmicos de baixa fusão combinados com titânio grau 2 e 5 por ensaio flexão e microscopia eletrônica de varredura

    The bond strength by three point bending strength of two metal substrates (commercially pure titanium or grade II, and Ti-6Al-4V alloy or grade V) combined to three distinct low-fusing ceramic systems (LFC) and the nature of porcelain-metal fracture by scanning electron microscopy (SEM) were evaluated. The results were compared to a combination of palladium-silver (Pd-Ag) alloy and conventional porcelain (Duceram VMK68). Sixty metal strips measuring 25x3x0.5mm were made - 30 of titanium grade II and 30 of titanium grade V, with application of the following types of porcelain: Vita Titankeramik, Triceram or Duceratin (10 specimens for each porcelain). The porcelains were bonded to the strips with dimensions limited to 8x3x1mm. The control group consisted of ten specimens Pd-Ag alloy/Duceram VMK68 porcelain. Statistical analyses were made by one-way analysis of variance (ANOVA) and Tukey test at 5% significance level. Results showed that the bond strength in control group (48.0MPa ± 4.0) was significantly higher than the Ti grade II (26.7MPa ± 4.1) and Ti grade V (25.2MPa ± 2.2) combinations. When Duceratin porcelain was applied in both substrates, Ti grade II and Ti grade V, the results were significantly lower than in Ti grade II/Vitatitankeramik. SEM analysis indicated a predominance of adhesive fractures for the groups Ti grade II and Ti grade V, and cohesive fracture for control group Pd-Ag/Duceram. Control group showed the best bond strength compared to the groups that employed LFC. Among LFC, the worst results were obtained when Duceratin porcelain was used in both substrates. SEM confirmed the results of three point bending strength.Foram avaliados dois substratos metálicos (titânio comercialmente puro ou grau 2 e a liga Ti-6Al-4V ou grau 5) combinados com a três sistemas cerâmicos de baixa fusão (PBF) sobre a resistência de união pelo teste de flexão de três pontos e a natureza da fratura porcelana-metal através da microscopia eletrônica de varredura (MEV). Os resultados foram comparados a combinação da liga paládio-prata (Pd-Ag) com porcelana convencional (Duceram VMK68). Foram confeccionadas sessenta tiras de metal medindo 25x3x0.5mm, sendo 30 de titânio grau 2 e 30 de titânio grau 5 sobre os quais foram aplicadas as porcelanas: Vita Titankeramik, Triceram e Duceratin (10 espécies de cada porcelana) nas dimensões de 8x3x1mm. O grupo controle era composto de 10 especies de Pd-Ag alloy/ com a porcelana Duceram VMK68. Na análise estatística utilizou-se análise de variância (ANOVA) e o teste de Tukey em nível de significância de 5%. Os resultados indicaram que a resistência de união do grupo controle (48.0 ± 4.0) foi estatisticamente significante maior que nos substratos Ti-2 (26.7 ± 4.1) e Ti-5 (25.2 ± 2.2). Os resultados dos substratos de Ti-2 e Ti-5 com a porcelana Duceratin foram estatisticamente significante menores quando comparados ao Ti-2 com a porcelana Vitatitankeramik. A análise pela MEV indicou fraturas predominantemente do tipo adesiva para as amostras de Ti-2 e Ti-5, e coesivas para o grupo controle PdAg/Duceram. O grupo controle apresentou maior resistência de união comparadas às amostras que empregaram as PBF. Entre as porcelanas de baixa fusão, os menores resultados foram obtidos com a porcelana Duceratin em ambos os substratos. A análise pela MEV confirmou os resultados do teste de flexão

    Micromarsupialization technique in oral ranula: report of two clinical cases in child patient

     Oral ranulas are pseudocysts resulting from the retention of mucus or extravasation of saliva that often affect children. This article aims to report two clinical cases of oral ranulas and discuss the treatment using the micromarsupialization technique. Case 1: 6-year-old girl, attended the Emergency Room of the Bebe Clinic of the State University of Londrina complaining of a “ball under the tongue” lasting approximately 3 months. Clinically, reddish bubble of approximately 1.5 cm in diameter was noted located on the right side of the dome-shaped mouth floor, sessile base and fluctuating. Case 2: 5-year-old girl, attended the same service with a complaint of “bubble on the tongue” lasting approximately 2 months. Clinically, pink-colored bubble similar to the mucosa of approximately 1 cm in diameter was observed located on the left side of the floor of the mouth with a circumscribed shape, sessile base and fluctuating. Based on the clinical aspects and the current history of the lesions, the diagnosis was oral ranula and the surgical treatment performed was micromarsupialization. In a clinical follow-up of 8 and 6 months, respectively, total regression of the lesions was observed, with no clinical signs of recurrence. Based on the reported clinical cases, it can be concluded that the use of micromarsupialization technique represents an effective surgical procedure in children due to its quick execution, conservative approach, without the use of local infiltrative anesthesia, well tolerated and with good prognosis

    Initial inflammatory response after the pulpotomy of rat molars with MTA or ferric sulfate

    Purpose: To compare, both qualitatively and quantitatively, the inflammatory cells, vascular density and IL-6 immunolabeled cells present in the pulp after pulpotomy with white MTA versus 15.5% ferric sulfate (FS). Methodology: Forty-eight mandibular first molars from 24 Wistar rats were divided into MTA or FS groups and subdivided according to the period after pulpotomy procedure (24, 48 and 72 hours). Four teeth (sound and untreated) were used as controls. Histological sections were obtained and assessed through the descriptive analysis of morphological aspects of pulp tissue and the quantification of inflammatory cells, vascular density and interleukin-6 (IL-6) expression. Data were statistically analyzed (p&lt;0.05). Results: The number of inflammatory cells was similar in both groups, being predominantly localized at the cervical radicular third. In the MTA group, increased inflammation was observed at 48 hours. Vascular density was similar in both groups and over time, being predominant in the medium radicular third. No correlation was found between the number of inflammatory cells and the vascular density. Pulp tissue was more organized in MTA-treated teeth. In both groups, a weak to moderate IL-6 expression was detected in odontoblasts and inflammatory cells. Comparing both groups, there was a greater IL-6 expression in the cervical radicular third of teeth treated with MTA at 24 hours and in the medium and apical thirds at 72 hours, while in the FS group a greater IL-6 expression was found in the apical third at 24 hours. Conclusion: The MTA group presented better histological features and greater IL-6 expression than the FS group. However, no difference was observed between the groups regarding the inflammatory status and vascularization, suggesting the usefulness of FS as a low-cost alternative to MTA

    Ellis-Van Creveld Syndrome, neonatal teeth and breastfeeding impairment: a case report

    ABSTRACT Because of multisystemic impairment in patients with Ellis-van Creveld syndrome, multidisciplinary care may be demanded since birth to assure breastfeeding. This report presents a case of an Ellis-van Creveld infant that was facing breastfeeding difficulties because of maxillary neonatal teeth. A 3 months old male infant with Ellis-van Creveld syndrome was referred to Pediatric Dentistry Department because of two upper neonatal teeth causing breastfeeding difficulties. Clinical examination revealed that teeth position was compatible to 51 and 61, and both presented uncommon ectopic soft tissue placement, conical crown and hypoplastic enamel covered by a large amount of dental biofilm. Radiography indicated they were of normal series and had 2/3 of crown completion. Due to teeth mobility that impaired breastfeeding, treatment option was exodontia. Early tooth eruption, such as in natal and neonatal teeth, by itself can’t be considered a reason for exodontia. But exodontia must be considered when an early erupted tooth(s) impairs breastfeeding, especially in systemically compromised infants. In this present case report, after teeth extraction, the infant was able to breastfeed and gain weight properly

    Anti-dentine Salivary SIgA in young adults with a history of dental trauma in deciduous teeth

    Anti-dentin autoantibodies are associated with inflammatory root resorption in permanent teeth and are modulated by dental trauma and orthodontic force. However, it is not known whether deciduous tooth trauma can stimulate the development of a humoral immune response against dentin. The aim of this study was to evaluate the levels of salivary SIgA reactivity against human dentin extract in young adults with a history of trauma in the primary dentition. A sample of 78 patients, aged 18 to 25, who had completed an early childhood (0 to 5 years old) caries prevention program years earlier at the Universidade Estadual de LondrinaPediatric Clinic, underwent radiographic examination and salivary sampling. Anti-dentin SIgA levels were analyzed by immunoenzymatic assay and Western blotting. Although dental trauma to deciduous teeth had occurred in 34 (43.6%) of the patients, no differences in SIgA levels were detected between individuals who had experienced trauma and those who had not (p > 0.05). Multivariate regression analysis showed no association between dental trauma and SIgA levels (p > 0.05). Patients with a history of deciduous trauma presented low levels of anti-dentin antibodies, associated with orthodontic root resorption (p < 0.05). Western blot analysis showed that salivary antibodies recognized a single band of approximately 45 kDa in dentin extract. We concluded that salivary SIgA recognizes a specific component of the dentin matrix and that anti-dentin antibodies were not triggered by trauma to primary teeth. However, trauma to deciduous teeth may down-modulate SIgA in response to orthodontic root response.</p
