    The creation of genetic basic population of rainbow trout (Onchorhynchus mykiss) based on study of genetic variation in brood stocks using microsatellite

    In order to perform the project, 446 samples of rainbow trout (Onchorhynchus mykiss) from 24 different regions in Iran were collected. About 2-3 g of caudal fin samples was collected from each specimen and preserved in absolute ethyl alcohol and then transferred to the genetic laboratory. Genomic DNA was extracted using the phenol-chloroform method and then DNA content and quality was determined using spectrophotometry and agarose gel electrophoresis, respectively. Polymerase Chain Reaction (PCR) of genomic DNA fin samples was carried out using 10 pairs of microsatellite primers. All PCR products were electrophoresed on 6% polyacrylamide gel and stained with silver nitrate. Following the scoring of alleles, all parameters including allelic frequency, effective number of allele, observed and expected heterozygosity, shanon index, measurement of similarity and genetic distance and Hardy-Weinberg equilibrium, Fst , Rst and gene flow were calculated using AMOVA analysis in the GenAlex and Popgene programs. The results showed that 8 pairs of microsatellite primers were polymorphic. In total, 50 alleles were determined with the range size of 64-280 bp. The locus omyf had maximum number of allele (26) and loci OTSG 474 and Strurruta58 had minimum number of allele (5). The observed heterozygosity was between 0.86 and 0.964. Hardy-Weinberg departure was observed for all loci from farms 18, 15, 4, E20 and 21 and were disequilibrium (P<0.05). The farms 14, 8, 7 and 6 were equilibrium at 3 loci, but showed disequilibrium in other loci. The other farms were equilibrium at 1 or 2 loci and disequilibrium at 8 or 9 loci. The FST results showed that maximum FST (0.24) were between farms 1 and 11in which had minimum of gene flow (3.7). Minimum FST (0.04) were between farms 8 and 9 in which had maximum of gene flow (346). Based on the results of AMOVA analysis, significant differences were detected between all farms (P<0.01). Furthermore, based on Nei 's standard (1972) maximum genetic distance (0.89) were observed between farms 2 and 11 and maximum genetic similarity (0.15) were detected between farms 3 and 4. This result suggests that the unique genetic variation of rainbow trout in hatchery farms of Iran represents a highly valuable genetic resource and provide useful information for creating a based population in the future breeding programs

    New findings: What is the central question of this study? Can low-volume high-intensity interval training and continuous moderate-intensity exercise modulate oscillatory and retrograde shear, blood flow and flow-mediated arterial dilatation in patients with type 2 diabetes? What is the main finding and its importance? Low-volume high-intensity interval training, by increasing anterograde shear and decreasing retrograde shear and oscillatory index, can increase nitric oxide production and consequently result in increased flow-mediated dilatation and outward arterial remodelling in patients with type 2 diabetes. Abstract: Atherosclerosis in patients with type 2 diabetes is characterized by endothelial dysfunction associated with impaired flow-mediated dilatation (FMD) and increases retrograde and oscillatory shear. The present study investigated endothelium-dependent vasodilatation and shear rate in patients with type 2 diabetes at baseline and follow-up after 12 weeks of low-volume high-intensity interval training (LV-HIIT) or continuous moderate-intensity training (CMIT). Seventy-five sedentary patients with type 2 diabetes and untreated pre- or stage I hypertension were randomly divided into LV-HIIT, CMIT and control groups. The LV-HIIT group intervention was 12 intervals of 1.5 min at 85�90 maximal heart rate (HRmax) and 2 min at 55�60 HRmax. The CMIT group intervention was 42 min of exercise at 70 HRmax for three sessions per week during 12 weeks. High-resolution Doppler ultrasound was used to measure FMD, arterial diameter, anterograde and retrograde blood flow, and shear rate patterns. Brachial artery FMD increased significantly in the LV-HIIT group (3.83 ± 1.13 baseline, 7.39 ± 3.6 follow-up), whereas there was no significant increase in the CMIT group (3.45 ± 0.97 baseline, 4.81 ± 2.36 follow-up) compared to the control group (3.16 ± 0.78 baseline, 4.04 ± 1.28 follow-up) (P &lt; 0.05). Retrograde shear in the LV-HIIT group decreased significantly (P &lt; 0.05), and no significant decrease in retrograde shear was seen in the CMIT group. Anterograde shear after LV-HIIT increased significantly (P &lt; 0.05) but was unchanged in the CMIT group. However, oscillatory shear index in both exercise groups decreased significantly (P = 0.029). Nitrite/nitrate (NOx) level increased in both exercise groups, but the increase was greater in the LV-HIIT group (P &lt; 0.001). The results indicate that by increasing NOx, HIIT decreases the oscillatory shear-induced improvement in FMD and outward artery remodelling in patients with type 2 diabetes. © 2018 The Authors.     Applying accreditation standards in a self-evaluation process: The experience of Educational Development Center of Tehran University of Medical Sciences

    Introduction: Educational Development Centers (EDCs), as the coordinator in education development in Medical Sciences universities, in order to improve their quality should evaluate their activities. In spite of remarkable performance of Tehran University of Medical Sciences (TUMS) EDC in previous national rankings, but it faces many challenges and problems. This paper provided the process, results and lessons learned from a self-evaluation experience conducted at TUMS EDC based on accreditation standards. Method: The present study is an Institutional self-evaluation study based on the national accreditation standards of EDCs (2012). Data were gathered using an open-ended questionnaire developed on the basis of the SWOT format. A directional content analysis applied to analyze the data. Results: In total, 84 point of strengths, 87 weaknesses, 15 opportunities, 24 threats and also 99 recommendations for quality improvement were reported. The most important strengths of the center were the existence of an established mechanism regarding research process in education and scholarship of education, holding various faculty development courses and training standardized patient. The most important weaknesses were the lack of specified procedures in some areas such as monitoring the planning and reviewing of educational programs in the field of educational programs and evaluation of empowerment courses. Conclusion: The present evaluation results will be useful in directing future policies of TUMS EDC such as revising its strategic planning. We hope that the current experience can be helpful for administrators in EDCs in the Ministry of Health and Medical Education and also other Medical Sciences Universities

    The association between waterpipe smoking and gastroesophageal reflux disease

    Background: Unlike cigarettes, there is little information about the association between other tobacco products and the risk of gastroesophageal reflux disease (GERD) and esophageal adenocarcinoma. Methods: We used the baseline data from the Pars Cohort Study conducted in southern Iran. In 2012, 9264 local residents between 40 and 75 years old were enrolled, with detailed information about lifestyle, including duration and frequency of tobacco use. GERD was defined based on questions assessing heartburn and regurgitation in the past 12 months, frequency and severity. Associations were calculated by logistic regression models adjusted for age, sex, education, cigarettes and body mass index. Results: In the study, 25.4% of the participants had severe GERD (interfering with participants' routines), 25.1% had frequent GERD (at least once a week) and 11.2% had both severe and frequent GERD, all more common among women (p < 0.001); 45.6% of women and 28.3% of men smoked waterpipes. Among people not using medications against reflux symptoms, there was an association between waterpipe smoking and severe [odds ratio (OR) = 1.18; 95% confidence interval (CI):1.04-1.35], frequent (OR=1.16; 95% CI: 1.02-1.32) and severe and frequent reflux (OR=1.30; 95% CI: 1.08-1.56). The associations increased with the duration of use, intensity and cumulative waterpipe-years, reaching an OR of 1.44 (95% CI: 1.12-1.86) for severe and frequent reflux in those who had smoked more than 48 waterpipe-years. There was effect modification by sex, and all the associations were only seen among women. Conclusion: The increasing trend in the association between cumulative waterpipe use and reflux disease among women is particularly important given the growing waterpipe tobacco epidemic in many populations. \ua9 The Author 2017
