We have engaged in extensive research about the GVSU Holland campus and the Holland community in order to identify and respond to critical needs. We have come to understand that the campus needs to be a home for transformational and relevant education in order for it to best utilize its current assets and that it needs to do this work by extending an arm into the Holland community. In response to this cross-section of needs, we have innovated a purpose for the Holland campus. The process was demanding, and the documents are extensive, but the innovation is worth it. Enjoy

    Common Diagnoses among Refugee Populations: Linked Results with Statewide Hospital Discharge Database

    Background: According to the U.S. State Department’s Refugee Processing Center and the U.S. Census Bureau, in the fiscal year 2016, among all states in the United States, Nebraska resettled the highest number of refugees per capita. Objectives: The objectives of this study were to determine the most common reasons for refugees utilizing hospital services in Nebraska between January 2011 and September 2015, and to examine whether refugee patients had increased risks for adverse health conditions compared to non-refugee patients. Methods: Statewide linkage was performed between Nebraska Medicaid Program’s immigration data, and 2011–2015 Nebraska hospital discharge data inpatient and outpatient files. The linkage produced 3017, 5460, and 775 cases for emergency department visits, outpatient clinic visits, and inpatient care for the refugee sample, respectively. Findings: Refugee patients were at increased risk for a number of diagnoses or medical conditions, including pregnancy complications, abdominal pain, upper respiratory infections, viral infections, mood disorders, disorders of teeth and jaw, deficiency and anemia, urinary system disorders, headache, nausea and vomiting, limb fractures, spondylosis, essential hypertension, and uncomplicated diabetes mellitus. Conclusions: The findings suggest a greater emphasis on preventive healthcare, especially in areas of maternal health and perinatal outcomes, psychological counseling, screening for infectious diseases, nutrition and healthy eating, and oral health. Additionally, culturally appropriate measures to address prevention, health screening, and treatments should be adopted by health providers who care for refugees

    Foreground Separation and Constraints on Primordial Gravitational Waves with the PICO Space Mission

    PICO is a concept for a NASA probe-scale mission aiming to detect or constrain the tensor to scalar ratio rr, a parameter that quantifies the amplitude of inflationary gravity waves. We carry out map-based component separation on simulations with five foreground models and input rr values rin=0r_{in}=0 and rin=0.003r_{in} = 0.003. We forecast rr determinations using a Gaussian likelihood assuming either no delensing or a residual lensing factor AlensA_{\rm lens} = 27%. By implementing the first full-sky, post component-separation, map-domain delensing, we show that PICO should be able to achieve AlensA_{\rm lens} = 22% - 24%. For four of the five foreground models we find that PICO would be able to set the constraints r < 1.3 \times 10^{-4} \,\, \mbox{to} \,\, r <2.7 \times 10^{-4}\, (95\%) if rin=0r_{in}=0, the strongest constraints of any foreseeable instrument. For these models, r=0.003r=0.003 is recovered with confidence levels between 18σ18\sigma and 27σ27\sigma. We find weaker, and in some cases significantly biased, upper limits when removing few low or high frequency bands. The fifth model gives a 3σ3\sigma detection when rin=0r_{in}=0 and a 3σ3\sigma bias with rin=0.003r_{in} = 0.003. However, by correlating rr determinations from many small 2.5% sky areas with the mission's 555 GHz data we identify and mitigate the bias. This analysis underscores the importance of large sky coverage. We show that when only low multipoles 12\ell \leq 12 are used, the non-Gaussian shape of the true likelihood gives uncertainties that are on average 30% larger than a Gaussian approximation.Comment: 34 pages, 13 figures, published in JCA

    Evolution MegaLab: a case study in citizen science methods

    1. Volunteers have helped in scientific surveys of birds and other organisms for decades, but more recently, the use of the Internet has enormously widened the opportunity for citizen science and greatly increased its practice. There is now a need to share experience of which methods work and which do not. Here, we describe how we planned and executed the Evolution MegaLab, one of the largest surveys of polymorphism in wild species so far undertaken. 2. The aim of the Evolution MegaLab was to exploit the occasion of Charles Darwin’s double centenary in 2009 to mobilize the widest possible section of the general public in Europe to help survey shell polymorphism in the banded snails Cepaea nemoralis and Cepaea hortensis. These data were then compared with historical records to detect evolutionary change that may have taken place in the decades between samples. 3. Records of polymorphism in over 7000 populations sampled throughout the natural range of the two species were captured from published and unpublished sources and added to an online database. These data could be explored by the general public via a Google Maps interface on the project website (http://evolutionmegalab.org). The website contained a welcome page that explained what evolution is and how recent changes in climate, and the abundance of predatory birds (song thrushes Turdus philomelos) might have caused an evolutionary change in the shell patterns of banded snails. 4. A network of collaborators in 15 European countries was formed, with each country responsible for translating the website and associated materials, recruiting volunteers and raising any funds required locally. A total of 6461 users registered with the site, and 7629 records were submitted. We used an online quiz to train users and to test their ability, to recognize the correct snails and their morphs. Every user received automated, immediate feedback that compared their data with nearby records fromthe historical database. 5.     Metabolic Targeting of Lactate Efflux by Malignant Glioma Inhibits Invasiveness and Induces Necrosis: An In Vivo Study

    Glioblastoma multiforme (GBM) are the most malignant among brain tumors. They are frequently refractory to chemotherapy and radiotherapy with mean patient survival of approximately 6 months, despite surgical intervention. The highly glycolytic nature of glioblastomas describes their propensity to metabolize glucose to lactic acid at an elevated rate. To survive, GBMs efflux lactic acid to the tumor microenvironment through transmembrane transporters denoted monocarboxylate transporters (MCTs). We hypothesized that inhibition of MCT function would impair the glycolytic metabolism and affect both glioma invasiveness and survival. We examined the effect on invasiveness with α-cyano-4-hydroxy-cinnamic acid (ACCA, 4CIN, CHCA), a small-molecule inhibitor of lactate transport, through Matrigel-based and organotypic (brain) slice culture invasive assays using U87-MG and U251-MG glioma cells. We then conducted studies in immunodeficient rats by stereotaxic intracranial implantation of the glioma cells followed by programmed orthotopic application of ACCA through osmotic pumps. Effect on the implanted tumor was monitored by small-animal magnetic resonance imaging. Our assays indicated that glioma invasion was markedly impaired when lactate efflux was inhibited. Convection-enhanced delivery of inhibitor to the tumor bed caused tumor necrosis, with 50% of the animals surviving beyond the experimental end points (3 months after inhibitor exhaustion). Most importantly, control animals did not display any adverse neurologic effects during orthotopic administration of ACCA to brain through programmed delivery. These results indicate the clinical potential of targeting lactate efflux in glioma through delivery of small-molecule inhibitors of MCTs either to the tumor bed or to the postsurgical resection cavity

    AcrB drug-binding pocket substitution confers clinically relevant resistance and altered substrate specificity.

    The incidence of multidrug-resistant bacterial infections is increasing globally and the need to understand the underlying mechanisms is paramount to discover new therapeutics. The efflux pumps of Gram-negative bacteria have a broad substrate range and transport antibiotics out of the bacterium, conferring intrinsic multidrug resistance (MDR). The genomes of pre- and posttherapy MDR clinical isolates of Salmonella Typhimurium from a patient that failed antibacterial therapy and died were sequenced. In the posttherapy isolate we identified a novel G288D substitution in AcrB, the resistance-nodulation division transporter in the AcrAB-TolC tripartite MDR efflux pump system. Computational structural analysis suggested that G288D in AcrB heavily affects the structure, dynamics, and hydration properties of the distal binding pocket altering specificity for antibacterial drugs. Consistent with this hypothesis, recreation of the mutation in standard Escherichia coli and Salmonella strains showed that G288D AcrB altered substrate specificity, conferring decreased susceptibility to the fluoroquinolone antibiotic ciprofloxacin by increased efflux. At the same time, the substitution increased susceptibility to other drugs by decreased efflux. Information about drug transport is vital for the discovery of new antibacterials; the finding that one amino acid change can cause resistance to some drugs, while conferring increased susceptibility to others, could provide a basis for new drug development and treatment strategies
