    Lichenoid reaction: case report

    Lichenoid lesions are mucocutaneous disease of chronic inflammatory origin. Can produce side effects to drugs, dental materials, affecting 0.5 to 1% of the world population mainly in women in the fourth decade of life. The history and meticulous clinical servation may clarify the reactive nature, but the clinical diagnosis will only be conclusive when associated with tests such as histopathology. Therefore the objective of the study is to report a case of a woman of 55 years old, complaining of "white spots sore". Reported being allergic to metals, chocolate, acidic foods and certain types of clothes. From accurate intraoral clinical examination, white plaques were found in the dorsum of the tongue, hard palate and buccal mucosa, inaccurate and rough limits, and ulcerations throughout the labial edge and upper palate. Histopathology revealed a lichenoid reaction. Was instituted as pharmacological treatment, the use of systemic corticosteroids. Clinical follow-up of one year showed reduction of clinical symptoms, allowing to conclude that the clinical follow-up of this disease is necessary since its dubious malignant potential and diagnostic difficulty. Thus, early recognition of this disease allows instituting appropriate treatment with relief of anxiety of the patient and their family.As lesões liquenoides são doenças mucocutâneas de origem inflamatória crônica. Podem produzir efeitos colaterais a medicamentos e a materiais odontológicos, acometendo de 0,5 a 1% da população mundial predominantemente em mulheres na quarta década de vida. A anamnese e a observação clínica meticulosa podem esclarecer a natureza reacional, porém o diagnóstico clínico só será conclusivo quando associado a ex- ames complementares como o histopatológico. O objetivo do presente trabalho é o de relatar um caso clínico de uma pa- ciente de 55 anos de idade, do sexo feminino, queixando-se de “manchas brancas doloridas”. Relatou ser alérgica a metais, chocolates, alimentos ácidos e certos tipos de roupas. A partir de acurado exame clínico intrabucal, foi encontrado placas brancas no dorso de língua, palato duro e mucosa jugal, limites imprecisos e rugosas, além de ulcerações ao longo de todo o rebordo vestibular superior e palato. O exame histopatológico revelou reação liquenoide. Foi instituído como tratamento farmacológico, o uso de corticosteroide sistêmico. O acompan- hamento clínico de um ano revelou redução do quadro clínico, permitindo concluir que o acompanhamento clínico dessa doença é necessário visto seu controverso potencial de malignização e a dificuldade diagnóstica. Assim, o reconhecimento precoce desta doença permite instituir tratamento adequado, com alívio da ansiedade do paciente e de seus familiares

    Antimicrobial resistance of aerobes and facultative anaerobes isolated from the oral cavity

    OBJECTIVES: This study evaluated the resistance to antimicrobials of aerobes and facultative anaerobes isolated from patients wearing complete dentures, patients with gingivitis and periodontitis, and periodontally health subjects. MATERIAL AND METHODS: Three hundred and four isolates were tested. The minimal inhibitory concentrations of the drugs were evaluated through the agar dilution method using Mueller-Hinton agar. RESULTS: The most active antimicrobial drugs were the carbapenems (meropenem and imipenem), and resistance to these drugs was restrict to 1.6-2.3% of the isolates, as well as ciprofloxacin and rifampin. Microbial resistance to ampicillin, amoxicillin/clavulanic acid, cefoxitin, cephalothin, amikacin, chloramphenicol and nalidixic acid was particularly high. In most cases, the resistance to β-lactams was mediated by the production of hydrolytic enzymes, especially in gram-negative enteric rods, while enterococci did not evidence production of these enzymes. The association amoxicillin/clavulanic acid was not effective in 28.3% of the tested isolates. CONCLUSIONS: The results of this investigation confirmed that the oral cavity of patients with periodontitis and gingivitis, and particularly edentulous patients wearing complete dentures, could harbor microorganisms with several antimicrobial resistance markers, and these microorganisms are frequently implicated in multiresistant, systemic, oral or nosocomial infections

    Fusobacterium nucleatum is indigenous of the human oral cavity and has been involved in different infectious processes. The production of bacteriocin-like substances may be important in regulation of bacterial microbiota in oral cavity. The ability to produce bacteriocin-like substances by 80 oral F. nucleatum isolates obtained from periodontal patients, healthy individuals and Cebus apella monkeys, was examinated. 17.5% of all tested isolates showed auto-antagonism and 78.8% iso- or hetero-antagonism. No isolate from monkey was capable to produce auto-inhibition. In this study, the antagonistic substances production was variable in all tested isolates. Most of the F. nucleatum showed antagonistic activity against tested reference strains. These data suggest a possible participation of these substances on the oral microbial ecology in humans and animals. However, the role of bacteriocins in regulating dental plaque microbiota in vivo is discussed.<br>Fusobacterium nucleatum é indígena da cavidade oral humana e tem sido envolvido em diferentes processos infecciosos. A produção de substâncias semelhantes a bacteriocinas pode ser importante na regulação da microbiota bacteriana da cavidade oral. A capacidade de produzir substâncias tipo bacteriocina de 80 isolados de F. nucleatum orais, obtidos de pacientes com doença periodontal, indivíduos sadios e macaco Cebus apella, foi avaliada. 17,5% de todos os isolados mostrou auto-antagonismo e 78,8% iso- ou hetero-antagonismo. Nenhum isolado de macaco foi capaz de produzir auto-inibição. Neste estudo, a produção de substâncias antagonístas foi variável em todos os isolados testados. A maioria dos F. nucleatum mostrou atividade antagonísta para as cepas de referência testadas. Esses dados sugerem a possível participação dessas substâncias sobre a ecologia microbiana em humanos e animais.
