Doctoral: A smartphone-based decision support tool for the early detection of oral potentially malignant disorders
Oral potentially malignant disorders can be defined as mucosal lesions and conditions with an increased risk of malignant transformation. Oral potentially malignant disorders are a significant health burden, and they are often diagnosed late due to scant attention to routine dental practice and the low number of specialized oral medicine centres. This report summarizes the DoctOral experience, a research initiative, providing a free smartphone-based decision support tool for the general medical/dental practitioner; the tool is based on the clinical appearance of oral lesions. Captured, oral pictures can be immediately examined via interactive decision trees and constructed on the smartphone. Such decision trees are expressed in standard formats, and they are readily accessible for facilitating the completion of a hypothetical diagnostic path. Since October 2017 the DoctOral mobile app has been downloaded by 10Kâ+âusers, achieving a score of 4.8 out of 5. DoctOral also supports an unfolding joint initiative, called DoctOralAI: this involves selecting reference images, with which to create an open-source model, and perform a Case-Based Reasoning method, both of which are combined with machine learning. The DoctOral mobile app has revolutionized oral pathology by providing dental students and professionals with an interactive platform for recognizing and diagnosing oral lesions
Improving the cancer adult patient support network (iCAN): a pilot study on a communication model and modified focus group
BackgroundMany consider that cancer has the greatest impact of any disease in the world, and it can drastically limit patients' quality of life. Combating such a life-threatening disease can pose many challenges to daily life, highlighted by demonstrating the need to discuss one's health status within a focus group and encourage treatment compliance.Aimthe purposes of this study were to share the authors' experience of a modified focus group in an Oral Medicine Unit, termed "Improving Cancer Adult Patients Support Network" (iCAN), and to evaluate how effective communication could improve patients' quality of life and empower them by virtue of enhanced knowledge and an awareness of cancer management.Methodsthe paper adhered to the COREQ checklist regarding its reporting procedures. The iCAN format was precisely reproduced four times with four groups, consisting of 12 adult male and female patients with solid cancers. They discussed several main topics relating to cancer treatment, as chosen by a majority of the participants. Four specialists were involved in the discussion of the selected topics The iCAN format was faithfully reproduced during each meeting, with the participants in the roles of moderator and health specialists. Finally, a satisfaction questionnaire was administered.Resultsthe most reliable results demonstrated a marked change in lifestyle and eating habits in more than 50% of participants. More than 80% were unaware of the side effects of cancer treatments in general and the oral mucosa in particular. Each meeting reported a maximum degree of satisfaction experienced by the participants.ConclusioniCAN focus group meetings appear to have facilitated a process of narrative interviewing, thereby improving the doctor-patient relationship underlying the humanization of the care process
Reduction of MRONJ risk after exodontia by virtue of ozone infiltration: A randomized clinical trial
Introduction: Exodontia is commonly considered as a risk factor for the development of medication-related osteonecrosis of the jaw (MRONJ) in individuals exposed to bone modifying agents. This study was aimed at assessing the efficiency and safety of a gaseous oxygen-ozone mixture as an adjuvant to a standard exodontia to reduce the risk of MRONJ development. Methods: A randomized, open-label, phase II, single-center clinical trial involving 117 patients at risk of MRONJ was conducted. The study protocol tested injections of an oxygen-ozone mixture in the post-extraction site. Participants were randomly assigned to two groups: oxygen-ozone therapy, and standard tooth extraction protocol. Post-extraction wound healing was assessed using the Inflammatory Proliferative Remodeling (IPR) Wound Healing Scale. Results: The oxygen-ozone therapy group exhibited a significant improvement in wound healing post-extraction during the inflammatory and proliferative phases, as indicated by the IPR scale scores at 3-5 days (p = 0.006) and 14 days (p < 0.001) respectively. Conclusion: Oxygen-ozone therapy shows promise in improving post-extraction healing in patients at risk of MRONJ. Future studies with larger sample sizes and multicenter collaborations are recommended to confirm the validity of these findings and explore the long-term efficacy of ozone therapy
Paediatric Sleep Questionnaire for Obstructive Sleep Apnoea Syndrome Screening: Is Sleep Quality Worthy of Note?
Obstructive sleep apnoea syndrome (OSAS) is the most severe condition on the spectrum
of sleep-related breathing disorders (SRBDs). The Paediatric Sleep Questionnaire (PSQ) is one of
the most used and validated screening tools, but it lacks the comprehensive assessment of some
determinants of OSAS, specifically anamnestic assessment and sleep quality. This study aims to
assess the accuracy of some specific items added to the original PSQ, particularly related to the
patientâs anamnestic history and to the quality of sleep, for the screening of OSAS in a paediatric
population living in Sicily (Italy). Fifteen specific items, divided into âanamnesticâ and ârelated to
sleep qualityâ were added to the original PSQ. The whole questionnaire was administered via a
digital form to the parents of children at 4 schools (age range: 3â13 years). For each item, sensitivity
and specificity, positive and negative predictive values, and positive and negative likelihood ratios
were calculated. The highest sensitivity (80.0, 95% CI: 28.4; 99.5), in combination with the highest
specificity (61.1, 95% CI: 35.7; 82.7), was found for the Item 32 (âassumption of bizarre or abnormal
positions during sleepâ). This item was found statistically significant for predicting the occurrence of
OSAS in children (p-value â¤0.003). The study demonstrates the accuracy of specific items related to
sleep quality disturbance for the preliminary assessment of the disease. Although these results should
be validated on a larger sample of subjects, they suggest that including the factors discriminating
sleep quality could further increase the efficiency and accuracy of PSQ
Oropharyngeal Adverse Events to Drugs and Vaccines: Pharmacovigilance Data From Italy (2019â2021)
Objectives: The aim of this study was to perform a descriptive analysis of oropharyngeal adverse events (AEs) related to drugs and/or vaccines in order to provide useful information for clinicians. Methods: Data related to three regions of Italy were analyzed from 2019 to 2021 by using the National Pharmacovigilance database. Results: Among overall 67,384 cases, 2773 (4.1%) reported at least one oropharyngeal AE. Most cases referred to females (71.0%) and adults (70.8%). The majority of cases were reported as not serious (68.4%) and the outcome was mainly positive (73.5%). The cases related to drugs (52.2%) were slightly more than those related to vaccines (47.8%), the latter nearly completely represented by COVID-19 vaccines. Among 3324 oropharyngeal AEs, the most commonly reported were oropharyngeal conditions (65.9%). The most reported AEs related to vaccines were paresthesia oral and oropharyngeal pain, whereas the most reported AEs related to drugs were throat tightness and angioedema. A marked under-reporting of osteonecrosis of the jaw (2.9%) was observed, despite this risk was well documented in the same country. Conclusions: This analysis suggested an under-reporting of oropharyngeal AEs and the need to better train dentists, dental hygienists, and also general practitioners
Intratumor Heterogeneity of ALK-Rearrangements and Homogeneity of EGFR-Mutations in Mixed Lung Adenocarcinoma
BACKGROUND:
Non Small Cell Lung Cancer is a highly heterogeneous tumor. Histologic intratumor heterogeneity could be 'major', characterized by a single tumor showing two different histologic types, and 'minor', due to at least 2 different growth patterns in the same tumor. Therefore, a morphological heterogeneity could reflect an intratumor molecular heterogeneity. To date, few data are reported in literature about molecular features of the mixed adenocarcinoma. The aim of our study was to assess EGFR-mutations and ALK-rearrangements in different intratumor subtypes and/or growth patterns in a series of mixed adenocarcinomas and adenosquamous carcinomas.
METHODS:
590 Non Small Cell Lung Carcinomas tumor samples were revised in order to select mixed adenocarcinomas with available tumor components. Finally, only 105 mixed adenocarcinomas and 17 adenosquamous carcinomas were included in the study for further analyses. Two TMAs were built selecting the different intratumor histotypes. ALK-rearrangements were detected through FISH and IHC, and EGFR-mutations were detected through IHC and confirmed by RT-PCR.
RESULTS:
10/122 cases were ALK-rearranged and 7 from those 10 showing an intratumor heterogeneity of the rearrangements. 12/122 cases were EGFR-mutated, uniformly expressing the EGFR-mutated protein in all histologic components.
CONCLUSION:
Our data suggests that EGFR-mutations is generally homogeneously expressed. On the contrary, ALK-rearrangement showed an intratumor heterogeneity in both mixed adenocarcinomas and adenosquamous carcinomas. The intratumor heterogeneity of ALK-rearrangements could lead to a possible impact on the therapeutic responses and the disease outcomes
Dupilumab in the treatment of severe uncontrolled chronic rhinosinusitis with nasal polyps (CRSwNP): A multicentric observational Phase IV real-life study (DUPIREAL)
Background
Chronic rhinosinusitis with nasal polyps (CRSwNP) is associated with significant morbidity and reduced health-related quality of life. Findings from clinical trials have demonstrated the effectiveness of dupilumab in CRSwNP, although real-world evidence is still limited.
Methods
This Phase IV real-life, observational, multicenter study assessed the effectiveness and safety of dupilumab in patients with severe uncontrolled CRSwNP (nâ=â648) over the first year of treatment. We collected data at baseline and after 1, 3, 6, 9, and 12âmonths of follow-up. We focused on nasal polyps score (NPS), symptoms, and olfactory function. We stratified outcomes by comorbidities, previous surgery, and adherence to intranasal corticosteroids, and examined the success rates based on current guidelines, as well as potential predictors of response at each timepoint.
Results
We observed a significant decrease in NPS from a median value of 6 (IQR 5â6) at baseline to 1.0 (IQR 0.0â2.0) at 12âmonths (pâ<â.001), and a significant decrease in Sino-Nasal Outcomes Test-22 (SNOT-22) from a median score of 58 (IQR 49â70) at baseline to 11 (IQR 6â21; pâ<â.001) at 12âmonths. Sniffin' Sticks scores showed a significant increase over 12âmonths (pâ<â.001) compared to baseline. The results were unaffected by concomitant diseases, number of previous surgeries, and adherence to topical steroids, except for minor differences in rapidity of action. An excellent-moderate response was observed in 96.9% of patients at 12âmonths based on EPOS 2020 criteria.
Conclusions
Our findings from this large-scale real-life study support the effectiveness of dupilumab as an add-on therapy in patients with severe uncontrolled CRSwNP in reducing polyp size and improving the quality of life, severity of symptoms, nasal congestion, and smell

L'odontoiatra si trova sempre piuĚ frequentemente a dover affrontare un aspetto particolarmente critico della sua pratica e cioeĚ le reazioni avverse ai farmaci. Tra queste, quelle che colpiscono le ghiandole salivari meritano un'attenzione speciale a causa della varietaĚ di segni, dei sintomi e dei farmaci coinvolti nelle diverse reazioni avverse. Le ghiandole salivari, essenziali per la produzione di saliva, giocano un ruolo cruciale non solo nella digestione e nella protezione dei denti e delle mucose orali, ma anche nel mantenimento dell'equilibrio microbico e nella facilitazione della percezione gustativa. Pertanto, qualsiasi alterazione indotta da farmaci a queste ghiandole puoĚ compromettere seriamente queste funzioni essenziali, causando secchezza della bocca, alterazioni del gusto, e predisponendo a infezioni orali, difficoltaĚ nella masticazione e nella deglutizione, noncheĚ a un deterioramento della salute dentale e sistemica. Per quanto riguarda poi la funzione gustativa, essa puoĚ essere compromessa da numerosi farmaci, i quali possono alterare o inibire la percezione dei sapori. Negli ultimi anni, si eĚ assistito a un significativo aumento delle reazioni avverse ai farmaci (ADR) in ambito medico; questo fenomeno eĚ attribuibile a diversi fattori, tra cui l'incremento dell'uso di farmaci sia prescritti sia auto-somministrati, e l'introduzione sul mercato di nuovi farmaci. Infatti, il rapporto annuale del 2022 sull'uso dei farmaci in Italia, redatto dall'Osservatorio nazionale sull'impiego dei medicinali dell'Agenzia Italiana del Farmaco (AIFA), ha registrato un aumento del 6,0% nella spesa farmaceutica nazionale complessiva rispetto all'anno precedente, raggiungendo i 34,1 miliardi di euro
