    Knowledge of HIV infection, risk perception, and sexual behaviour of undergraduates. May female medical students act as peer educators?

    This paper is aimed to verify whether the knowledge and beliefs of female medical freshmen about HIV infection, their personal risk perception, and their sexual behaviour differs from their female peers, in view of the possibility of female medical student-based peer education. A purposive, theoretical quota sampling method was used to recruit the target population. A self-administered anonymous questionnaire was delivered to both female medical and non medical freshmen during March 2004 (n = 266: 124 medical students and 142 non medical students) in Catania (Sicily). The data were analysed for the whole sample and for the two groups of students individually. The Chi-square test was used to compare data from the two groups of students. Results showed that knowledge and risk perception about HIV infection were higher for medical students when compared with non medical students. Moreover, a lower rate of sexually active medical students and a higher rate of condom use was found among them. Since female medical students seem more sensitive to risk perception and aware of healthier lifestyles, they could be useful in peer sexual education and appropriate prevention programmes against HIV infection

    Plasmid-mediated multiple antibiotic resistance of Escherichia coli in crude and treated wastewater used in agriculture.

    A total of 273 Escherichia coli isolates from raw and treated municipal wastewaters were investigated to evaluate the frequency and persistence of antibiotic resistance and to detect the occurrence of conjugative R plasmids and integrons. The highest resistance rates were against ampicillin (22.71%), tetracycline (19.41%), sulfamethoxazole (16.84%) and streptomycin (14.28%). Multiple antibiotic resistance was present in 24.17% of the isolates. Several multiple antibiotic-resistant isolates proved to be able to transfer en bloc their resistance patterns by conjugative R plasmids with different molecular sizes and restriction profiles. Class 1 integrons of 1 or 1.5 kbp were found in 5 out of 24 representative multiresistant E. coli isolates. Although wastewater treatments proved to be effective in eliminating Salmonella spp. and in reaching WHO microbiological standards for safe use of wastewater in agriculture, they were ineffective in reducing significantly the frequency of plasmid-mediated multiple antibiotic resistance in surviving E. coli. Since multiple antibiotic-resistant bacteria carrying integrons and conjugative R plasmids can constitute a reservoir of antibiotic-resistance genes in wastewater reclaimed for irrigation, risks for public health should be considered. Bacterial strains carrying R plasmids and integrons could contaminate crops irrigated with reclaimed wastewater and transfer their resistances to the consumers' intestinal bacteria

    Helicobacter pylori and Epstein-Barr virus infection in gastric diseases: Correlation with IL-10 and IL1RN polymorphism.

    Introduction: Helicobacter pylori and Epstein-Barr virus (EBV) infection have recently 23 been shown to be associated with gastric diseases. Polymorphisms in genes encoding 24 cytokines such as interleukin 10 (IL-10) and interleukin 1 Receptor (IL-1RN) influence 25 cytokine secretion levels and appear to contribute to the risk of developing gastroduodenal 26 diseases. 27 To our knowledge, this is the first preliminary study to address the association of 28 coinfection with H. pylori and EBV and their correlation with genetic predisposition in the 29 development of gastric diseases. 30 Methods: Gastric biopsy samples of 96 patients with different gastric diseases were used. 31 Results: Our results showed that the rate of co-infection was higher in patients with 32 gastric cancer than in patients with normal gastric mucosa, active chronic gastritis and 33 MALT lymphoma. As regards the characterization of H. pilory strains, the 34 polymorphism s1m1i1 of vacA gene was more frequent in patients with MALT 35 Lymphoma in comparison to others, while the polymorphism s2m2i2 was most 36 frequent in patients with normal gastric mucosa. In addition, patients who tested 37 positivefor the cagA gene were more frequently those affected with gastric cancer than 38 those with inactive chronic gastritis. Similarly, the patients with oipA gene ON were more 39 frequently those with gastric cancer than those with inactive chronic gastritis. 40 Conclusion: According to our analysis, there was no correlation between coinfection 41 and polymorphisms in genes encoding IL-10 and IL-1RN. We conclude that various 42 factors can be involved in the development of gastric diseases

    Persistent oral and urinary Candida spp. carriagein Italian HIV-seropositive asymptomatic subjects

    The aim of the present study was to ascertain frequency and persistence of Candida spp. oral and urinary carriage in asymptomatic, HAART-naive HIV-seropositive subjects who had not undergone therapy with antimycotic drugs, and whose CD4+ lymphocyte count was greater than 200/?l. Oral carriage was the most common Candida spp. carriage (63.0% of the subjects), while candiduria was more rarely observed (6.5%). C. albicans was recovered from the majority of the subjects examined (56.5%), followed by C. krusey (4.3%), C. tropicalis (2.2%) and C. dubliniensis (2.2%). C. albicans was also isolated from two urinary carriers (4.3%) and C. glabrata from another one (2.2%). The same C. albicans clone was repeatedly isolated from 14 out of 15 oral carriers while the same clone of C. dubliniensis was repeatedly isolated from one carrier, as shown by the persistence of RAPD fingerprint of serial isolates during one year of follow-up. Since persistence of Candida spp. carriage may influence the development of clinical candidiasis in immunocompromised hosts, monitoring of the carrier status could be useful for preventing clinical thrush in HIV-seropositive subjects

    Biofilm production in Staphylococcus epidermidis strains isolated from the skin of hospitalized patients: Genetic and phenotypic characteristics

    A major virulence factor of Staphylococcus epidermidis is its ability to form biofilms, permitting it to adhere to a surface and, in turn, to form a mucoid layer on polymer surfaces. Multiple factors have been found to influence bacterial attachment. Currently, this bacterium is commonly associated with hospital infections as a consequence of its ability to colonize, albeit accidentally, medical devices. This study investigated the genetic and phenotypic formation of biofilm in 105 S. epidermidis strains isolated from the skin of hospitalized patients. Fifty-eight of these strains were positive for the mecA gene (MRSE) and 47 were found to be negative (MSSE). Genetic characterizations were performed for the detection of the mecA, icaADBC, atlE, aap, bhp, IS256 and agr groups by PCR. Biofilm production was examined by culturing the strains in TBS medium and TBS with 0.5 and 1% respectively of glucose, and a semiquantitative assay on tissue culture plates was used. Although a molecular analysis estimate of detailed biofilm formation is costly in terms of time and complexity, a semiquantitative assay can be proposed as a rapid and cheap diagnostic method for initial screening to discover virulent strains. We confirmed a close correlation between genetic and phenotypic characteristics, highlighting the fact that, when S. epidermidis isolates were cultured in TSB with 1% of glucose, an increase in biofilm production was observed, as confirmed by positivity for the ica locus by molecular analysis

    Preliminary Indoor Radon Measurements Near Faults Crossing Urban Areas of Mt. Etna Volcano (Italy)

    The slopes of Etna are crossed by numerous active faults that traverse various towns and villages. These faults pose a two-fold problem for the local people: on one hand, they cause frequent damage to houses and breakage of roads, while on the other they constitute a preferential route for the rising of crustal and sub-crustal gases, including radon, toward the surface. Various recent studies on the volcano confirm a high level of radon degassing measured both in the soil (> 10,000 Bq/m3), and inside homes (> 2,000 Bq/m3). For this reason, we felt the need to deepen our knowledge on the radon present in the Etnean area, focusing in particular on indoor radon pollution that, as widely recognized, is among the main causes of cancer largely (but not exclusively) of the respiratory system. Firstly, since 2005 we made a broad surface survey that revealed very high radon emissions from soils near active faults on Etna. Typical background soil activity on Etna were <1,000 Bq/m3, whereas in areas of stronger soil degassing, activity values up to ~60,000 Bq/m3 were measured. Furthermore, since late 2015 we have performed continuous indoor radon monitoring inside seven houses, some of which located close to degassing faults on the eastern, southern and south-western flanks of the volcano. Indoor radon concentration varied according to the season of the year, but above all, they changed according to the geology and tectonic setting of the substratum of the monitored houses. In one case, indoor radon concentration reached 3,549 Bq/m3 and remained > 1,000 Bq/m3 for several consecutive months, highlighting a potential health problem for those living in such environments. In other cases, the construction features of the houses and/or the materials used seemed to play an important role in the mitigation of indoor radon accumulation, even in the presence of intensely degassing soils. These preliminary data demonstrate the need to deepen the studies, extending indoor radon measurements to other urban areas, in order to monitor the health hazard for the Etna population, amounting to about one million people

    Fourth case of louse-borne relapsing fever in Young Migrant, Sicily, Italy, December 2015. Mini Review Article

    Objectives Currently louse-borne relapsing fever (LBRF) is primarily found in limited endemic foci in Ethiopia, Somalia and Sudan; no case of imported LBRF has been reported in Europe in the 9 years prior to 2015. The aim of our paper is to describe a new case of imported LBRF detected in Sicily, Italy, and to review all cases reported in migrants arrived in Europe in the last 10 years. Study design Mini review of all published cases of louse-borne relapsing fever in Europe in the last 10 years. Methods A computerized search without language restriction was conducted using PubMed combining the terms ‘(louse-borne relapsing fever or LBRF or recurrentis) and (refugee or Europe or migrant)’ without limits. Furthermore, the ‘Ahead-of-Print Articles’ of the top 10 journals (ranked by Impact factor – Web of Science) of Infectious diseases and of Epidemiology were checked. Results Our search identified 26 cases of LBRF between July and October 2015 in migrants recently arrived in Europe: 8 had been described in Italy; 1 in Switzerland; 2 in the Netherlands; 15 in Germany. We describe data regarding the clinical characteristics, diagnostic methods, therapy and outcome of these patients and of the new case. Conclusions LBRF by Borrelia recurrentis should be considered among the clinical hypotheses in migrants presenting with fever, headache, chills, sweating, arthralgia, myalgia, dizziness, nausea and vomiting
