    Surgical preferences of patients at risk of hip fractures: hemiarthroplasty versus total hip arthroplasty

    BACKGROUND: The optimal treatment of displaced femoral neck fractures in patients over 60 years is controversial. While much research has focused on the impact of total hip arthroplasty (THA) and hemiarthroplasty (HA) on surgical outcomes, little is known about patient preferences for either alternative. The purpose of this study was to elicit surgical preferences of patients at risk of sustaining hip fracture using a novel decision board. METHODS: We developed a decision board for the surgical management of displaced femoral neck fractures presenting risks and outcomes of HA and THA. The decision board was presented to 81 elderly patients at risk for developing femoral neck fractures identified from an osteoporosis clinic. The participants were faced with the scenario of sustaining a displaced femoral neck fracture and were asked to state their treatment option preference and rationale for operative procedure. RESULTS: Eighty-five percent (85%) of participants were between the age of 60 and 80 years; 89% were female; 88% were Caucasian; and 49% had some post-secondary education. Ninety-three percent (93%; 95% confidence interval [CI], 87-99%) of participants chose THA as their preferred operative choice. Participants identified several factors important to their decision, including the perception of greater walking distance (63%), less residual pain (29%), less reoperative risk (28%) and lower mortality risk (20%) with THA. Participants who preferred HA (7%; 95% CI, 1-13%) did so for perceived less invasiveness (50%), lower dislocation risk (33%), lower infection risk (33%), and shorter operative time (17%). CONCLUSION: The overwhelming majority of patients preferred THA to HA for the treatment of a displaced femoral neck fracture when confronted with risks and outcomes of both procedures on a decision board

    A randomised controlled trial to evaluate the efficacy of a 6 month dietary and physical activity intervention for prostate cancer patients receiving androgen deprivation therapy

    <p>Abstract</p> <p>Background</p> <p>Treatment with Androgen Deprivation Therapy (ADT) for prostate cancer is associated with changes in body composition including increased fat and decreased lean mass; increased fatigue, and a reduction in quality of life. No study to date has evaluated the effect of dietary and physical activity modification on the side-effects related to ADT. The aim of this study is to evaluate the efficacy of a 6-month dietary and physical activity intervention for prostate cancer survivors receiving ADT to minimise the changes in body composition, fatigue and quality of life, typically associated with ADT.</p> <p>Methods</p> <p>Men are recruited to this study if their treatment plan is to receive ADT for at least 6 months. Men who are randomised to the intervention arm receive a home-based tailored intervention to meet the following guidelines a) ≥ 5 servings vegetables and fruits/day; b) 30%-35% of total energy from fat, and < 10% energy from saturated fat/day; c) 10% of energy from polyunsaturated fat/day; d) limited consumption of processed meats; e) 25-35 gm of fibre/day; f) alcoholic drinks ≤ 28 units/week; g) limited intake of foods high in salt and/or sugar. They are also encouraged to include at least 30 minutes of brisk walking, 5 or more days per week. The primary outcomes are change in body composition, fatigue and quality of life scores. Secondary outcomes include dietary intake, physical activity and perceived stress. Baseline information collected includes: socio-economic status, treatment duration, perceived social support and health status, family history of cancer, co-morbidities, medication and supplement use, barriers to change, and readiness to change their health behaviour. Data for the primary and secondary outcomes will be collected at baseline, 3 and 6 months from 47 intervention and 47 control patients.</p> <p>Discussion</p> <p>The results of this study will provide detailed information on diet and physical activity levels in prostate cancer patients treated with ADT and will test the feasibility and efficacy of a diet and physical activity intervention which could provide essential information to develop guidelines for prostate cancer patients to minimise the side effects related to ADT.</p> <p>Trial registration</p> <p>ISRCTN trial number ISCRTN75282423</p

    Music recommendation according to human motion based on kernel CCA-based relationship

    In this article, a method for recommendation of music pieces according to human motions based on their kernel canonical correlation analysis (CCA)-based relationship is proposed. In order to perform the recommendation between different types of multimedia data, i.e., recommendation of music pieces from human motions, the proposed method tries to estimate their relationship. Specifically, the correlation based on kernel CCA is calculated as the relationship in our method. Since human motions and music pieces have various time lengths, it is necessary to calculate the correlation between time series having different lengths. Therefore, new kernel functions for human motions and music pieces, which can provide similarities between data that have different time lengths, are introduced into the calculation of the kernel CCA-based correlation. This approach effectively provides a solution to the conventional problem of not being able to calculate the correlation from multimedia data that have various time lengths. Therefore, the proposed method can perform accurate recommendation of best matched music pieces according to a target human motion from the obtained correlation. Experimental results are shown to verify the performance of the proposed method

    Conditions for criticality by uranium deposition in water-saturated geological formations

    This study focuses on neutronic analysis to examine the criticality conditions for uranium depositions in geological formations resulting from geological disposal of damaged fuels from Fukushima Daiichi reactors. MCNP models are used to evaluate the neutron multiplication factor (keff) and critical mass for various combinations of host rock and geometries. It has been observed that the keff for the deposition become greater with (1) smaller concentrations of neutron-absorbing materials in the host rock, (2) larger porosity of the host rock, (3) heterogeneous geometry of the deposition, and (4) greater mass of uranium in the deposition. This study has revealed that the planar fracture geometry applied in the previous criticality safety assessment for geological disposal would not necessarily yield conservative results against the homogeneous uranium deposition
