    Instrumental delivery: a comparative study in a tertiary care hospital

    Background: To compare maternal and neonatal outcomes of vacuum versus forceps application in assisted vaginal delivery.Methods: Women in labor with vertex presentation were delivered by indicated/propylactic vacuum or forceps. A total of 500 cases were included in this retrospective study. Maternal and neonatal morbidity were compared in terms of perineal lacerations, episiotomy extension, post-partum hemorrhage, Apgar score, neonatal jaundice, perinatal mortality, NICU admissions etc. Chi square test was used to analyze the data.Results: Maternal morbidity was significant in the forceps group. With regards to neonatal morbidity, in NICU admissions, statistically significant difference was noted.Conclusions: Vacuum and forceps should remain appropriate tools in the modern obstetrics. However, ventouse may be chosen first (if there is no fetal distress) as it is significantly less likely to injure the mother and decrease NICU admissions

    Study of histopathological patterns of endometrium in abnormal uterine bleeding

    Background: Abnormal uterine bleeding (AUB) is the commonest presenting symptom in gynaecology out- patient department. Endometrial sampling could be effectively used as the first diagnostic step in AUB. This study was done to evaluate histopathology of endometrium for identifying the endometrial causes of AUB. And also, to observe the incidence of various endometrial pathology in different age groups presenting with abnormal uterine bleeding.Methods: A one-year prospective study conducted in the department of obstetrics and gynaecologist in A. J. institute of medical sciences and research centre which included 200 cases of clinically diagnosed AUB patients. Histopathological examination of endometrial biopsies specimens was done, followed by clinical correlation.Results: The most common age group presenting with AUB was 41-50 years (43.84%). The commonest pattern in these patients was normal cycling endometrium (42.32%). The commonest pathology irrespective of the age group was disordered proliferative pattern (26.15%). Other causes identified atrophic endometrium (11.5%), benign endometrial polyp (5.38%), endometrial hyperplasia (5.41%), carcinomas (0.79%) and chronic endometritis (1.54%).Conclusions: The knowledge of endometrial pattern in a case of AUB helps to decide a plan of management. Endometrial sampling plays a very important role in management of AUB, especially in the age group of more than 40 years; i.e., the peri and post-menopausal age groups, where incidence of malignant or pre malignant conditions was noted to be the highest

    A modified particle swarm optimization algorithm to enhance MPPT in the PV array

    Due to the growing demand for electrical power, the researchers are trying to fulfill this demand by considering different ways of renewable energy resource as existing energy resources failed to do so. The solar energy from the sun is freely available, and by using photovoltaic (PV) cell power can be generated. However, it depends on rays fall on the PV cell, climatic condition. Thus, to enhance the efficiency of the photovoltaic (PV) systems, maximum power point tracking (MPPT) of the solar arrays is needed.The output of solar arrays mainly depends on solar irradiance and temperature. The mismatch phenomenon takes place due to partial shade, and it causes to the power output, which brings the incorrect operation of traditional MPP tracker. In this shaded condition, PV array exhibits multiple extreme points. In general, under this scenario, the MPPT approaches fail to judge the MPP, and it leads to low efficiency. The conventional approaches of PSO based algorithms can able to track the MPP under shading condition. However, the optimization process leads to issues in tracking speed. Thus, there a need for an efficient MPPT system which can track MPPT effectively in shaded condition? Hence, the proposed manuscript presents a modified Particle Swarm Optimization (PSO) algorithm is introduced to enhance the tracking speed as well as performance. The outcomes of the proposed system are compared with the traditional PSO system and are found that the tracking speed of MPP, accuracy, and efficiency is improved

    A cost effective computational design of maximum power point tracking for photo-voltaic cell

    Maximum Power Point Tracking (MPPT) is one of the essential controller operations of any Photo-Voltaic (PV) cell design. Developing an efficient MPPT system includes a significant challenge as there are various forms of uncertainty factors that results in higher degree of fluctuation in current and voltage in PV cell. After reviewing existing system, it has been found that there is no presence of any benchmarked model to ensure a better form of computational model. Hence, this paper presents a novel and very simple design of MPPT without using any form of complex design mechanism nor including any form of frequently used iterative approach. The proposed model is completely focused on developing an algorithm that takes the input of voltage (open circuit), current (short circuit), and max power in order to obtain the peak power to be extracted from the PV cells. The study outcome shows faster response time and better form of analysis of current-voltage-power for given state of PV cells

    Study on post partum intrauterine contraceptive device practices and causes for discontinuation of PPIUCD at follow up in a tertiary hospital

    Background: Post partum contraception is the best evidence based intervention in prevention of pregnancy and abortion related maternal morbidity and mortality in the developing countries. The unmet need for contraception among women in the postpartum period can be effectively fulfilled by post partum insertion of IUCD, in a single visit under the Government scheme of providing free maternity services during institutional delivery. Despite optimal efforts by family planning program, very few couples are opting for spacing methods and lack of awareness has resulted in discontinuation of family planning methods particularly PPIUCD. This study is designed to study the practices of PPIUCD and causes for discontinuation at follow up in our hospital. The objectives of the study were study PPIUCD practices at Vanivilas hospital; causes for discontinuation of PPIUCD.Methods: Prospective study done at Vanivilas hospital attached to Bangalore Medical College and Research Institute from January 2014 to December 2014. Women admitted and delivered at VVH, were counselled. CuT 380A was inserted in accepters who fulfilled the Medical Eligibility Criteria and had no contraindications for PPIUCD. They were followed up till June 2016.Results: There were 2072 PPIUCD insertions in one year, out of which 1244 were post placental, 139 were in immediate postpartum and 689 were intra caesarean insertions. Fifty four (54) women discontinued PPIUCD during follow up .Main causes for removal were menstrual abnormalities (19), pain abdomen (13), wanting sterilisation procedure (12) and marital disharmony.Conclusions: PPIUCD is an effective, safe, reversible method of long term contraception with high reported expulsion and low perforation rate, compared to interval insertion. More research is needed in the field of PPIUCD to enhance awareness and acceptance in the community. Awareness and counselling the eligible couples during ante natal care can improve acceptance and compliance of PPIUCD continuation rates

    Laparoscopy as a diagnostic tool in evaluation of female factors in infertility

    Background: Infertility leads to considerable personal suffering and disruption of family life. According to United Nations "Reproductive health is a state of complete physical mental and social well-being and not merely the absence of disease or infirmity in all matters relating to the reproductive system and to its functions and processes". The objective of present study was to find out different causes of female infertility with diagnostic laparoscopy and their comparative frequency in primary and secondary infertility.Methods: It is a prospective study conducted on all infertile women and they underwent diagnostic laparoscopy for primary and secondary infertility during the study period. Couples who had not lived together for at least 12 months, and those with male factor infertility were excluded. Data were collected on a proforma, and analyzed on SPSS package for windows version 10. Frequencies were calculated for laparoscopic findings regarding primary and secondary infertility.Results: Fifty infertile women underwent laparoscopy during the study period, 35 (70%) had primary infertility while 15 (30%) secondary infertility. 10 (28.5%) patients with primary and 3 (20%) patients with secondary infertility had no visible abnormality. The common finding was tubal blockage in 10 (28.5%) and 5 (33.3%) cases of primary and secondary infertility respectively. 9 (25.7%) cases of primary infertility were detected as polycystic ovaries (PCO) and 2 (13.3%) in cases of secondary infertility. Endometriosis was found in 1 case with primary infertility and 2 (13.3%) cases with secondary infertility. Fibroid was found in 3 (8.57%) and 1 (6.6%) cases of primary and secondary infertility respectively.Conclusions: Most common causes responsible for infertility were tubal occlusion and polycystic ovary. Infertile couple should be thoroughly investigated. Laparoscopy in infertility can be used for a definitive diagnosis

    Prospective study of contraceptive knowledge among the patients seeking medical termination of pregnancy in 1st and 2nd trimester in a tertiary health care

    Background: A rapid population growth is a burden on the resources of many developing countries. Unregulated fertility, which contributes to such situations compromise the economic development and political stability.Methods: It was a prospective, observational study over a period of 12 months at Vani Vilas hospital, attached to Bangalore Medical College and Research Institute. All patients seeking medical termination of pregnancy in I and II trimester in tertiary health care were evaluated for contraceptive knowledge.Results: Majority of the patients were from 25 – 30 years, educated, and had married life of 5 years, multipara, with no relationship with religion. The major reason for seeking termination of pregnancy was V i.e. failure of contraception. Intrauterine devices are the most popular method of contraception. There were various sources for knowledge.Conclusions: This study concludes that there is considerable level of knowledge and awareness about contraception in the above studied women but actual practice of those methods is low

    PHYTOCHEMICAL COMPOSITION, ANTIOXIDANT AND IN-VITRO ANTI-INFLAMMATORY ACTIVITY OF ETHANOL EXTRACT OF RUTA GRAVEOLENS L. LEAVES

    Objective: The uses of medicinal herbs in prevention and treatment of numerous diseases attracting attention of scientists worldwide. It is necessary to bring vital healthcare to the people and the better substitute for various infectious diseases. Ruta graveolens is traditionally used as a medicinal plant as well as a flavoring agent in food. In the present study R. graveolens leaves extracts were evaluated for phytochemical and anti-inflammatory activity.Methods: The phytochemical, antioxidant (DPPH), ferrous reducing antioxidant property, Anti-lipid peroxidation and superoxide scavenging activity of extracts were assayed by spectrophotometerically. Anti-inflammatory activities; Human plural fluid phospholipase A2 (HPF-PLA2) inhibition by egg yolk plate method and lipoxigenase (LOX) inhibition were assayed by IN VITRO method.Results: The phenolics were significantly higher than the flavonoids, saponins and alkaloids contents in all the extracts. Among extracts of R. graveolens, ethanol and water extracts exhibits more phytochemicals. The ethanol extract had the highest antioxidant activity followed by the aqueous extract. The ethanol extract shows highest DPPH free radical scavenging, ferric reducing, superoxide scavenging and anti-lipid peroxidation activity and IC50 was 3.27±0.03, 3.58±0.05, 3.87±0.04 and 4.77±0.04 µg/µl respectively at dose-dependent manner. Further ethanol and water extracts were subjected for LOX inhibition in the concentration dependent manner and IC50 values are 4.25±0.05, 5.15±0.05 µg/µl for 5-LOX and 4.15±0.04, 4.66±0.05 µg/µl for 15-LOX respectively.Conclusion: The result shows R. graveolens has strong antioxidant property and anti-inflammatory activity.     A prospective study of role of doppler in pregnancy and the perinatal outcome

    Background: The development of doppler ultrasonographic technology has provided an opportunity to obtain a qualitative and quantitative assessment of maternal and foetal circulation using a non-invasive method. It has been proved by many studies that doppler has a very important role in screening of high-risk pregnancies. Objective of this study was to evaluate the role of colour doppler study in normal and high-risk pregnancy in relation to perinatal outcome.Methods: A prospective study was done including 75 women with high risk pregnancy and 75 normal pregnant women during the period October 2018 to September 2019 in hospitals attached to Bangalore Medical College and Research Institute. Doppler examination was done after recording patients’ history, clinical examination and ultrasound. Results were analysed and conclusions were made.Results: Out of the 22 patients with PIH, 20 patients had abnormal umbilical artery S/D ratio and all 22 had abnormal MCA PI. Out of 12 patients with diabetes, 10 had abnormal umbilical artery S/D ratio. All the patients with IUGR had abnormal umbilical artery S/D ratio and abnormal MCA PI.Conclusions: Colour doppler flow velocimetry done repeatedly can predict adverse foetal events with a great degree of accuracy

    Content-Split Block Search Algorithm Based High Efficiency Video Coding

    690-693In this paper, the video streaming generation in H.265 using novel technique based on content split block (CSB) search algorithm is presented. The proposed algorithm exploits the Inter and Intra prediction through motion estimation and compensation (IPME) encoded to use four different QPs: 22, 27, 32, and 37, during the redundancy analysis in order to improve the quality of video frame encoded. The proposed algorithm exhibits the useful property of block structure based on content-tree representation for each and every frame to IPME coded without affecting either the bit rate of video stream and perceptual quality of the video frame. The proposed Search algorithm improves the visual quality of coded video frame and reduces the blocking artefacts of video frame passed through multi-stages of H.265
