Combined Assessment of Arterial and Venous Anatomy in Lower Limb Ischaemia Using a Single Contrast-Enhanced Magnetic Resonance Scan
AbstractContrast-Enhanced Magnetic Resonance Angiography (CE-MRA) is a non-invasive investigative modality for patients with lower limb arterial insufficiency. When infra-inguinal arterial reconstruction is indicated autologous great saphenous vein is the conduit of choice and adequacy of the vein is often assessed by an additional Duplex ultrasound.We evaluated whether single, high-resolution steady state MR imaging with blood pool contrast agent could generate a sufficiently informative assessment of both venous and arterial anatomy to plan surgery potentially avoiding the need for an additional venous duplex.There was good correlation between CE-MRA, venous duplex and subsequent operative findings
'Atypical' bacteria are a common cause of community-acquired pneumonia in hospitalised adults
Objectives. To assess the proportion of cases of community· acquired pneumonia caused by 'atypical' bacteria, inclUding the recently discovered Chlamydia pneumoniae, and to compare the clinical, radiographic and laboratory features of patients with and without 'atypical' bacteria.Methods. A prospective serological study was carried out on consecutive adult pneumonia patients from July 1987 to July 1988. Acute and convalescent sera were tested in batches for antibodies against Legionella pneumophila serogroup 1, C. pneumoniae, Chlamydia psittaci, Coxiella burnetii (phase-2 antigen) and Mycoplasma pneumoniae (lgG and IgM). Records and chest radiographs were examined retrospectively.Results. Acute and convalescent sera were available from 113 patients. The records of 4 patients could not be traced and 17 patients did not fulfil the inclusion criteria. Thirty-two of these 92 patients (35,9%) were found to be infected with 'atypical' bacteria. The two most common organisms were C. pneumoniae (20,7%) and L. pneumophila (8,7%). There. were no differences in the clinical and radiographic features of patients with and without 'atypical' bacteria. Clinicians prescribed erythromycin or tetracyclines with equal frequency in the two groups.Conclusions. 'Atypical' bacteria, especially C. pneumoniae, are a common cause of community-acquired pneumonia in adults in South Africa. This is the first demonstration of an aetiological role of C. pneumoniae in this country. We confirmed the finding of other studies that there are no clinical, radiographic or laboratory features characteristic of 'atypical' bacterial infection in hospitalised patients. This has major implications for therapy, as these organisms respond to erythromycin and tetracyclines, but· not to β-lactam antibiotics
t1 signal measurements in pediatric brain findings after multiple exposures to gadobenate dimeglumine for imaging of nonneurologic disease
BACKGROUND AND PURPOSE: Signal intensity increases possibly suggestive of gadolinium retention have recently been reported on unenhanced T1-weighted images of the pediatric brain following multiple exposures to gadolinium-based MR contrast agents. Our aim was to determine whether T1 signal changes suggestive of gadolinium deposition occur in the brains of pediatric nonneurologic patients after multiple exposures to gadobenate dimeglumine. MATERIALS AND METHODS: Thirty-four nonneurologic patients (group 1; 17 males/17 females; mean age, 7.18 years) who received between 5 and 15 injections (mean, 7.8 injections) of 0.05 mmol/kg of gadobenate during a mean of 2.24 years were compared with 24 control patients (group 2; 16 males/8 females; mean age, 8.78 years) who had never received gadolinium-based contrast agents. Exposure to gadobenate was for diagnosis and therapy monitoring. Five blinded readers independently determined the signal intensity at ROIs in the dentate nucleus, globus pallidus, pons, and thalamus on unenhanced T1-weighted spin-echo images from both groups. Unpaired t tests were used to compare signal-intensity values and dentate nucleus–pons and globus pallidus–thalamus signal-intensity ratios between groups 1 and 2. RESULTS: Mean signal-intensity values in the dentate nucleus, globus pallidus, pons, and thalamus of gadobenate-exposed patients ranged from 366.4 to 389.2, 360.5 to 392.9, 370.5 to 374.9, and 356.9 to 371.0, respectively. Corresponding values in gadolinium-based contrast agent–naive subjects were not significantly different ( P > .05). Similarly, no significant differences were noted by any reader for comparisons of the dentate nucleus–pons signal-intensity ratios. One reader noted a difference in the mean globus pallidus–thalamus signal-intensity ratios (1.06 ± 0.006 versus 1.02 ± 0.009, P = .002), but this reflected nonsignificantly higher T1 signal in the thalamus of control subjects. The number of exposures and the interval between the first and last exposures did not influence signal-intensity values. CONCLUSIONS: Signal-intensity increases potentially indicative of gadolinium deposition are not seen in pediatric nonneurologic patients after multiple exposures to low-dose gadobenate
Bethe ansatz solution of the closed anisotropic supersymmetric U model with quantum supersymmetry
The nested algebraic Bethe ansatz is presented for the anisotropic
supersymmetric model maintaining quantum supersymmetry. The Bethe ansatz
equations of the model are obtained on a one-dimensional closed lattice and an

Gaussian Wavefunctional Approach in Thermofield Dynamics
The Gaussian wavefunctional approach is developed in thermofield dynamics. We
manufacture thermal vacuum wavefunctional, its creation as well as annihilation
operators,and accordingly thermo-particle excited states. For a
(D+1)-dimensional scalar field system with an arbitrary potential whose Fourier
representation exists in a sense of tempered distributions, we calculate the
finite temperature Gaussian effective potential (FTGEP), one- and
two-thermo-particle-state energies. The zero-temperature limit of each of them
is just the corresponding result in quantum field theory, and the FTGEP can
lead to the same one of each of some concrete models as calculated by the

Challenges in delivering computed tomography coronary angiography as the first-line test for stable chest pain
Objective The National Institute for Health and Care Excellence (NICE) clinical guidelines ‘chest pain of recent onset: assessment and diagnosis’ (update 2016) state CT coronary angiography (CTCA) should be offered as the first-line investigation for patients with stable chest pain. However, the current provision in the UK is unknown. We aimed to evaluate this and estimate the requirements for full implementation of the guidelines including geographical variation. Ancillary aims included surveying the number of CTCA-capable scanners and accredited practitioners in the UK. Methods The number of CTCA scans performed annually was surveyed across the National Health Service (NHS). The number of percutaneous coronary interventions performed for stable angina in the NHS in 2015 was applied to a model based on SCOT-HEART (CTCA in patients with suspected angina due to coronary heart disease: an open-label, parallel-group, multicentre trial) data to estimate the requirement for CTCA, for full guideline implementation. Details of CTCA-capable scanners were obtained from manufacturers and formally accredited practitioner details from professional societies. Results An estimated 42 340 CTCAs are currently performed annually in the UK. We estimate that 350 000 would be required to fully implement the guidelines. 304 CTCA-capable scanners and 198 accredited practitioners were identified. A marked geographical variation between health regions was observed. Conclusions This study provides insight into the scale of increase in the provision of CTCA required to fully implement the updated NICE guidelines. A small specialist workforce and limited number of CTCA-capable scanners may present challenges to service expansion
Thermal and magnetic properties of integrable spin-1 and spin-3/2 chains with applications to real compounds
The ground state and thermodynamic properties of spin-1 and spin-3/2 chains
are investigated via exactly solved su(3) and su(4) models with physically
motivated chemical potential terms. The analysis involves the Thermodynamic
Bethe Ansatz and the High Temperature Expansion (HTE) methods. For the spin-1
chain with large single-ion anisotropy, a gapped phase occurs which is
significantly different from the valence-bond-solid Haldane phase. The
theoretical curves for the magnetization, susceptibility and specific heat are
favourably compared with experimental data for a number of spin-1 chain
compounds. For the spin-3/2 chain a degenerate gapped phase exists starting at
zero external magnetic field. A middle magnetization plateau can be triggered
by the single-ion anisotropy term. Overall, our results lend further weight to
the applicability of integrable models to the physics of low-dimensional
quantum spin systems. They also highlight the utility of the exact HTE method.Comment: 38 pages, 15 figure
