    Tailoring therapy for heart failure: the pharmacogenomics of adrenergic receptor signaling.

    Heart failure is one of the leading causes of mortality in Western countries, and β-blockers are a cornerstone of its treatment. However, the response to these drugs is variable among individuals, which might be explained, at least in part, by genetic differences. Pharmacogenomics is the study of genetic contributions to drug response variability in order to provide evidence for a tailored therapy in an individual patient. Several studies have investigated the pharmacogenomics of the adrenergic receptor system and its role in the context of the use of β-blockers in treating heart failure. In this review, we will focus on the most significant polymorphisms described in the literature involving adrenergic receptors and adrenergic receptor-related proteins, as well as genetic variations influencing β-blocker metabolism

    A signaling protocol for service function localization

    Current proposals for chaining service functions (SFs) do not address some critical management issues, such as the discovery of SF instances close to IP data paths. This information is crucial for deploying complex services both in large cloud networks, where SFs may be moved or replicated, and in the emerging fog/mobile edge computing systems. For this purpose, in this letter, we propose the distributed off-path signaling protocol. We show the protocol functions and demonstrate its scalability and effectiveness by experimental results

    Extensible Signaling Framework for Decentralized Network Management Applications

    The management of network infrastructures has become increasingly complex over time, which is mainly attributed to the introduction of new functionality to support emerging services and applications. To address this important issue, research efforts in the last few years focused on developing Software-Defined Networking solutions. While initial work proposed centralized architectures, their scalability limitations have led researchers to investigate a distributed control plane, with controller placement algorithms and mechanisms for building a logically centralized network view, being examples of challenges addressed. A critical issue that has not been adequately addressed concerns the communication between distributed decision-making entities to ensure configuration consistency. To this end, this paper proposes a signaling framework that can allow the exchange of information in distributed management and control scenarios. The benefits of the proposed framework are illustrated through a realistic network resource management use case. Based on simulation, we demonstrate the flexibility and extensibility of our solution in meeting the requirements of distributed decision-making processes

    Advanced Caching for Distributing Sensor Data through Programmable Nodes

    This paper shows an innovative solution for distributing dynamic sensor data by using distributed caches. Our proposal is based on the concepts of service modularization and virtualization of network nodes made available by the NetServ hosting environment, which has been defined and implemented with the aim of extending the functions of the network nodes. Through a lab experiment involving tens of nodes, we have demonstrated a significant performance improvements in term of traffic saving and download time in comparison with a legacy, Internet-based, approach. Beyond this performance improvements, the proposed solution holds also functional improvements, in terms of dynamic deployment and easy integration with services making use of sensor data.Comment: Accepted for publication in IEEE LANMAN 201

    Neuro-hormonal effects of physical activity in the elderly.

    Thanks to diagnostic and therapeutic advances, the elderly population is continuously increasing in the western countries. Accordingly, the prevalence of most chronic age-related diseases will increase considerably in the next decades, thus it will be necessary to implement effective preventive measures to face this epidemiological challenge. Among those, physical activity exerts a crucial role, since it has been proven to reduce the risk of cardiovascular diseases, diabetes, obesity, cognitive impairment and cancer. The favorable effects of exercise on cardiovascular homeostasis can be at least in part ascribed to the modulation of the neuro-hormonal systems implicated in cardiovascular pathophysiology. In the elderly, exercise has been shown to affect catecholamine secretion and biosynthesis, to positively modulate the renin-angiotensin-aldosterone system and to reduce the levels of plasma brain natriuretic peptides. Moreover, drugs modulating the neuro-hormonal systems may favorably affect physical capacity in the elderly.     Mechanical pull-out test of a new hybrid fixture-abutment connection: an in vitro study

    Implant abutment connection was described among the main causes of peri-implant bone resorption. The aim of this in vitro study was to test the coupling capacity, the surface modification of a new hybrid connection and the influence of repeated connection activations caused during the main clinical and laboratory phases. A total of 40 implant-abutment screw retained systems with 10 degrees-conical and internal hexagon connection were tested. The connection was screwed, fixed to the universal test machine, removed the screw and a pull-out test was performed. Test was repeated five times in succession. Also Scanning Electron Microscopy (SEM) was used to detect microscopically surface modification. Analysis of variance and Tukey tests were used for the statistical analysis. Pull-out test reveals a mean value of 131.35 +/- 16.52 Newton Centimeter (N center dot cm). For each single activation, results from first to fifth were: 113.9 +/- 13.02, 126.1 +/- 12.81, 138.11 +/- 15.15, 138.8 +/- 11.90 and 140 +/- 12.99 N center dot cm. A statistically significant difference between the measurements and an increase in the removal force was shown. The collected data supports the use of this new type of connection, resulting in a very strong interface between implant and abutment. Also, repeated activation of connection can promote a better coupling of the implant-abutment interface

    Directional Receivers for Diffusion-Based Molecular Communications

    The particle motion in diffusion-based molecular communication systems is typically modeled by using Brownian processes. In particular, this model is used to characterize the propagation of signal molecules after their release from the transmitter. This motion cannot include directionality in the propagating signal and translates into omnidirectional broadcast communications. In order to make such molecular communications system suitable for supporting communications protocols at the molecular scales, we propose to improve the receiver capabilities by introducing a form of directionality while receiving biological signals. Inspired by the directionality introduced in electromagnetic communications by means of directional antennas, we designed a nanomachine receiver having directionality properties. Our aim is to increase the average concentration of signal molecules, also referred to as carriers, in the area around the receiver surface. In this way, it is possible to increase the signal strength at the receiver. For this purpose, we propose to use a purely reflecting shell to be placed at a configurable distance from the receiver surface. The shape of the shell can be modeled as either a spherical cap or a cylinder with an empty basis. The presence of this surface causes a number of signal molecules to remain trapped in a region close to the receiver surface for a sufficiently long time. In this way, the probability of assimilating additional carriers by the compliant receptors present on the receiver surface increases. By means of an extensive simulation campaign, we identified the most suitable configuration able to provide a significant advantage with respect to those not adopting the proposed solution. The resulting approach can be regarded as an enabler of protocols for diffusive molecular communications taking advantage of directionality properties at the receiver site. It can result in an increased communication range or in improved capabilities of discriminating signals of coexisting molecular communication systems

    Microbiological and biochemical effectiveness of an antiseptic gel on the bacterial contamination of the inner space of dental implants: a 3-month human longitudinal study.

    Microbial penetration inside the implant's internal cavity produces a bacterial reservoir that is associated with an area of inflamed connective tissue facing the fixture-abutment junction. The aim of this clinical trial is to evaluate the effectiveness of a 1% chlorhexidine gel on the internal bacterial contamination of implants with screw-retained abutments and on the level of AST secreted in peri-implant crevicular fluid. Twenty-five patients (aged 29 to 58 years) each received one implant. Three months after the end of the restorative treatment, and immediately after a clinical and radiographic examination and the abutment removal, microbiological samples were obtained from the internal part of each fixture and biochemical samples were collected by peri-implant sulci. The patients were then divided into two groups: the control (CG; n=10) and test (TG; n=15) groups. The CG had the abutment screwed into place and the crown cemented without any further intervention. In contrast, before the abutment placement and screw tightening, the TG had the internal part of the fixture filled with a 1% chlorhexidine gel. Three months later, the same clinical, microbiological and biochemical procedures were repeated in both groups. Total bacterial count, specific pathogens and AST activity were detected. The clinical parameters remained stable throughout the study. From baseline to the 3-month examination, the total bacterial counts underwent a significant reduction only in the TG. In contrast, the AST activity showed a significant increase in the CG. The administration of a 1% chlorhexidine gel appears to be an effective method for the reduction of bacterial colonization of the implant cavity and for safeguarding the health status of peri-implant tissue over a 3-month administration period
