    Enabling Collaborative eHealth Research using Web 2.0 Tools

    In this paper, we describe two Web 2.0 based systems designed to facilitate and enhance collaborative eHealth research activities. Using a combination of Forums, Wikis and connectivity to 3rd party social networking systems, we have designed systems to support collaborative document creation (including editing, reviewing and publication), dissemination of material to relevant communities, discussion of ideas, and sharing of opinions. The ECDC Field Epidemiology Manual Wiki and Medicine Support Unit Online Forums are presented herein, including an overview to the system architectures, and user interaction models. We present our planned methods of evaluation, focusing on the ability to measure successful and sustainable community involvement

    Prevalence of frailty and prefrailty among community-dwelling older adults in low-income and middle-income countries: a systematic review and meta-analysis

    OBJECTIVE: To systematically review the research conducted on prevalence of frailty and prefrailty among community-dwelling older adults in low-income and middle-income countries (LMICs) and to estimate the pooled prevalence of frailty and prefrailty in community-dwelling older adults in LMICs. DESIGN: Systematic review and meta-analysis. PROSPERO registration number is CRD42016036083. DATA SOURCES: MEDLINE, EMBASE, AMED, Web of Science, CINAHL and WHO Global Health Library were searched from their inception to 12 September 2017. SETTING: Low-income and middle-income countries. PARTICIPANTS: Community-dwelling older adults aged ≥60 years. RESULTS: We screened 7057 citations and 56 studies were included. Forty-seven and 42 studies were included in the frailty and prefrailty meta-analysis, respectively. The majority of studies were from upper middle-income countries. One study was available from low-income countries. The prevalence of frailty varied from 3.9% (China) to 51.4% (Cuba) and prevalence of prefrailty ranged from 13.4% (Tanzania) to 71.6% (Brazil). The pooled prevalence of frailty was 17.4% (95% CI 14.4% to 20.7%, I²=99.2%) and prefrailty was 49.3% (95% CI 46.4% to 52.2%, I²=97.5%). The wide variation in prevalence rates across studies was largely explained by differences in frailty assessment method and the geographic region. These findings are for the studies with a minimum recruitment age 60, 65 and 70 years. CONCLUSION: The prevalence of frailty and prefrailty appears higher in community-dwelling older adults in upper middle-income countries compared with high-income countries, which has important implications for healthcare planning. There is limited evidence on frailty prevalence in lower middle-income and low-income countries

    Association between frailty and disability among rural community-dwelling older adults in Sri Lanka: a cross-sectional study

    OBJECTIVE: We examined the association between frailty and disability in rural community-dwelling older adults in Kegalle district of Sri Lanka. DESIGN: A population-based cross-sectional study. PARTICIPANTS: A total of 746 community-dwelling adults aged ≥60 years. PRIMARY AND SECONDARY OUTCOME MEASURES: Frailty was assessed using the Fried phenotype. Disability was operationalised in terms of having one or more activity limitation/s in instrumental activities of daily living (IADL) and basic activities of daily living (BADL). RESULTS: The median age of the sample was (median 68; IQR 64-75) years and 56.7% were female. 15.2% were frail and 48.5% were prefrail. The prevalence of ≥1 IADL limitations was high, 84.4% among frail adults. 38.7% of frail adults reported ≥1 BADL limitations. Over half of frail older adults (58.3%) reported both ≥1 physical and cognitive IADL limitations. Being frail decreased the odds of having no IADL limitations, and was associated with a higher count of IADL limitations. No significant association was found between prefrailty and number of IADL limitations. CONCLUSIONS: The prevalence of ≥1 IADL limitations was high among rural community-dwelling frail older adults. Findings imply the greater support and care required for rural Sri Lankan frail older adults to live independently in the community

    Building Integrated Vegetation Systems and their Sustainability Aspects; A Literature Review

    Abstract:There is a growing need of sustainable building development all over the world. It aims to decrease the adverse effects to the environment due to urbanization and escalating population growth. Since the building construction is considered as one of the main concerns, the priority was given to mitigate the negative impact to the environment. Therefore, incorporating sustainable elements and techniques to the buildings to regain the land loss due to construction activities in cities is currently practicing. Adding various vegetation types through different approaches, to obtain the expected results of better living condition around the building is called as building integrated vegetation systems (BIV). Vertical gardening systems, vertical farms, constructed green roofs and roof farms can be stated as main categories of BIV systems. However, there is a paucity of published critical reviews on such systems and therefore, this study is an attempt to review the overall sustainability aspects of BIV systems including environmental sustainability, economic sustainability and social sustainability. This study consists with a critical review of 114 research publications from relevant journals and online scientific databases. Finally, the identified sustainability aspects of each BIV systems were analyzed to select the best option in terms of greening a building which can be recommended for the implementations in future. Mainly, the importance of moving towards the sustainable solution which meets the food needs through BIV is finally discussed. Finally, it can be concluded that by incorporating green architecture with smart agriculture, we can expect green, healthy and productive cities which fulfill the main requirements of sustainable cities. Though there are many challenges to overcome, maintaining good management practices will give better output. Out of the 114 literature selected for this study, only 8 research papers were discussed about the drawbacks and the limitations of the BIV systems which is still having paucity of information.     Predicting the effects of deep brain stimulation using a reduced coupled oscillator model

At present, it involves administering a train of pulses with constant frequency via electrodes implanted into the brain. New ‘closed-loop’ approaches involve delivering stimulation according to the ongoing symptoms or brain activity and have the potential to provide improvements in terms of efficiency, efficacy and reduction of side effects. The success of closed-loop DBS depends on being able to devise a stimulation strategy that minimizes oscillations in neural activity associated with symptoms of motor disorders. A useful stepping stone towards this is to construct a mathematical model, which can describe how the brain oscillations should change when stimulation is applied at a particular state of the system. Our work focuses on the use of coupled oscillators to represent neurons in areas generating pathological oscillations. Using a reduced form of the Kuramoto model, we analyse how a patient should respond to stimulation when neural oscillations have a given phase and amplitude, provided a number of conditions are satisfied. For such patients, we predict that the best stimulation strategy should be phase specific but also that stimulation should have a greater effect if applied when the amplitude of brain oscillations is lower. We compare this surprising prediction with data obtained from ET patients. In light of our predictions, we also propose a new hybrid strategy which effectively combines two of the closed-loop strategies found in the literature, namely phase-locked and adaptive DBS

As a solution to this, two hybrid Napier cultivars; CO-5 and Sampoorna were recently introduced and have been described as superior yielding to previous cultivars of CO-3 (Pennisetum perpureum × P. typhodium) and CO-4 (Pennisetum glaucum × P. purpureum).     Stationary phase expression of the arginine biosynthetic operon argCBH in Escherichia coli

    BACKGROUND: Arginine biosynthesis in Escherichia coli is elevated in response to nutrient limitation, stress or arginine restriction. Though control of the pathway in response to arginine limitation is largely modulated by the ArgR repressor, other factors may be involved in increased stationary phase and stress expression. RESULTS: In this study, we report that expression of the argCBH operon is induced in stationary phase cultures and is reduced in strains possessing a mutation in rpoS, which encodes an alternative sigma factor. Using strains carrying defined argR, and rpoS mutations, we evaluated the relative contributions of these two regulators to the expression of argH using operon-lacZ fusions. While ArgR was the main factor responsible for modulating expression of argCBH, RpoS was also required for full expression of this biosynthetic operon at low arginine concentrations (below 60 μM L-arginine), a level at which growth of an arginine auxotroph was limited by arginine. When the argCBH operon was fully de-repressed (arginine limited), levels of expression were only one third of those observed in ΔargR mutants, indicating that the argCBH operon is partially repressed by ArgR even in the absence of arginine. In addition, argCBH expression was 30-fold higher in ΔargR mutants relative to levels found in wild type, fully-repressed strains, and this expression was independent of RpoS. CONCLUSION: The results of this study indicate that both derepression and positive control by RpoS are required for full control of arginine biosynthesis in stationary phase cultures of E. coli

    ASSESSMENT OF THE COMPOSITION AND THE VOLUME OF BIOGAS LIBERATION FROM AGRICULTURAL WASTES

    Study was conducted in the Faculty of Agriculture, University of Ruhuna, Mapalana toassess the composition and the volume of biogas liberation pattern from differentagricultural raw materials.In the Sri Lankan batch type biogas generator (volume 6m\ first batch of straw (lOOOkg)was filled in September 1999 and second and third batch in October 2000, April 2001respectively. It was revealed that average gas production from the digester was 1.0 m3/daywith a peak value of 1.8m3/day. Total gas production during four and half month period ofdigestion was 166.85 m3. The maximum average methane percentage and the average lagphase during the digestion were 50% and 20 days respectively.In order to assess the biogas liberation pattern and composiuon of each material, alaboratory test unit was designed using a 20 L plastic gallon as the digester andexperiment was conducted using straw, cow dung and market garbage (vegetable) as rawmaterials. Gas volumes, pH value of slurry and methane percentage were measured whilemaintaining the temperature between 35°C-37°C using electric bulbs.N,P,K compositions, CIN ratio and dry matter percentages of different raw materials wereanalyzed. Total biogas liberated per unit weight of dry matter was 8.15 L, 58.7 Land15.797L for dung, vegetable and straw respectively. The duration of digestion period was55-60 days. Average methane percentages of the liberated gases were 53%, 54% and 50%for dung, vegetable and straw respectively. It was revealed that market garbage had thehighest digestibility value, which had the highest gas production with high methanepercentage.

    Drought increases heat tolerance of leaf respiration in Eucalyptus globulus saplings grown under both ambient and elevated atmospheric [CO₂] and temperature

    Climate change is resulting in increasing atmospheric [CO₂], rising growth temperature (T), and greater frequency/severity of drought, with each factor having the potential to alter the respiratory metabolism of leaves. Here, the effects of elevated atmospheric [CO₂], sustained warming, and drought on leaf dark respiration (R(dark)), and the short-term T response of R(dark) were examined in Eucalyptus globulus. Comparisons were made using seedlings grown under different [CO₂], T, and drought treatments. Using high resolution T-response curves of R(dark) measured over the 15-65 °C range, it was found that elevated [CO₂], elevated growth T, and drought had little effect on rates of R(dark) measured at T <35 °C and that there was no interactive effect of [CO₂], growth T, and drought on T response of R(dark). However, drought increased R(dark) at high leaf T typical of heatwave events (35-45 °C), and increased the measuring T at which maximal rates of R(dark) occurred (Tmax) by 8 °C (from 52 °C in well-watered plants to 60 °C in drought-treated plants). Leaf starch and soluble sugars decreased under drought and elevated growth T, respectively, but no effect was found under elevated [CO₂]. Elevated [CO₂] increased the Q₁₀ of R(dark) (i.e. proportional rise in R(dark) per 10 °C) over the 15-35 °C range, while drought increased Q₁₀ values between 35 °C and 45 °C. Collectively, the study highlights the dynamic nature of the T dependence of R dark in plants experiencing future climate change scenarios, particularly with respect to drought and elevated [CO₂].This work was funded by the Australian Research Council (ARC FT0991448, DP1093759, and CE140100008, to OKA; and DP0879531, to DTT).     Prevalence of frailty in rural community-dwelling older adults in Kegalle district of Sri Lanka: a population-based cross-sectional study

    OBJECTIVE: Our main objective was to describe the prevalence and associated sociodemographic factors of frailty and pre-frailty in rural community-dwelling older adults in Kegalle district of Sri Lanka. DESIGN: Community-based cross-sectional study. SETTING: The study was conducted in rural areas of Kegalle district in Sri Lanka. PARTICIPANTS: A total of 746 community-dwelling older adults aged ≥60 years were included in the study. RESULTS: The prevalence of frailty and pre-frailty in rural Kegalle district was 15.2% (95% CI 12.3% to 18.6%) and 48.5% (95% CI 43.8% to 53.2%), respectively. We found a strong association between age and both frailty and pre-frailty. There were strong associations between longest-held occupation and frailty and education level and pre-frailty. CONCLUSIONS: The prevalence of frailty in this rural Sri Lankan older population was high compared with high-income and upper middle-income countries. The profile of health and social care services in Sri Lanka needs to address frailty and its consequences
