    A Framework for Enhanced QoS Support in IEEE 802.11e Networks

    IEEE 802.11 based WLANs have became popular, but they can only provide best effort services and so they are poorly suitable for multimedia applications. Recently IEEE 802.11e standard has been proposed to support quality of service. The new standard introduces a so-called Hybrid Coordination Function containing two medium access mechanisms: contention-based channel access and controlled channel access. In this paper we propose a novel framework to better support QoS guarantees for multimedia applications. It comprises QoS Manager, Admission Control, Enhanced Scheduler, Predictor and Feedback System. The scheduler adopted supports real-time applications, variable packet sizes and variable bit rate traffic streams. We show that this framework is suitable to be used by applications requesting Application Level Contracts which will be translated in Resource Level Contracts to the scheduler subsystem. The QoS manager component is able to dynamically manage available resources under different load conditions

    Improving the QoS of IEEE 802.11e networks through imprecise computation

    IEEE 802.11e HCCA reference scheduler is based on fixed value parameters that do not adapt to traffic changes, thus quality of service (QoS) for multimedia applications is a challenge, especially in the case of variable bit rate (VBR) streams, that requires dynamic resource assignment. This paper is focused on immediate dynamic TXOP HCCA (IDTH) scheduling algorithm and its new evolution immediate dynamic TXOP HCCA plus (IDTH+). Their reclaiming mechanisms, refined by the monitoring of transmission duration, aim at overcoming the limits of fixed preallocation of resources by varying the stations transmission time and avoiding waste of resources. Simulations and theoretical analysis based on the imprecise computation model show that the integration of IDTH and IDTH+ can achieve improved network performance in terms of transmission queues length, mean access delay and packets drop rate, and to efficiently manage bursty traffic. Moreover, the performance improvements of IDTH+ with respect to IDTH are highlighted

    A communication platform demonstrator for new generation railway traffic management systems: Testing and validation

    Current rail traffic management and control systems cannot be easily upgraded to the new needs and challenges of modern railway systems because they do not offer interoperable data structures and standardized communication interfaces. To meet this need, the Horizon 2020 Shift2Rail OPTIMA project has developed a communication platform for testing and validating the new generation of traffic management systems (TMS), whose main innovative features are the interoperability of the data structures used, standardization of communications, continuous access to real-time and persistent data from heterogeneous data sources, modularity of components and scalability of the platform. This paper presents the main components, their functions and characteristics, then describes the testing and validation of the platform, even when federated with other innovative TMS modules developed in separate projects. The successful validation of the system has confirmed the achievement of the objectives set and allowed a new set of objectives to be defined for the reference platform for the railway TMS/Traffic Control systems

    Multi-contact planning and control for humanoid robots: Design and validation of a complete framework

    In this paper, we consider the problem of generating appropriate motions for a torque- controlled humanoid robot that is assigned a multi-contact loco-manipulation task, i.e., a task that requires the robot to move within the environment by repeatedly establishing and breaking multiple, non-coplanar contacts. To this end, we present a complete multi-contact planning and control framework for multi-limbed robotic systems, such as humanoids. The planning layer works offline and consists of two sequential modules: first, a stance planner computes a sequence of feasible contact combinations; then, a whole-body planner finds the sequence of collision-free humanoid motions that realize them while respecting the physical limitations of the robot. For the challenging problem posed by the first stage, we propose a novel randomized approach that does not require the specification of pre-designed potential contacts or any kind of pre-computation. The control layer produces online torque commands that enable the humanoid to execute the planned motions while guaranteeing closed-loop balance. It relies on two modules, i.e., the stance switching and reactive balancing module; their combined action allows it to withstand possible execution inaccuracies, external disturbances, and modeling uncertainties. Numerical and experimental results obtained on COMAN+, a torque-controlled humanoid robot designed at Istituto Italiano di Tecnologia, validate our framework for loco-manipulation tasks of different complexity

    Novel approach for validation of innovative modules for railway traffic management systems in a virtual environment

    To increase operational efficiency, resilience and capacity of the railway system, the development of modern railway traffic management system (TMS) has attracted more and more attention in recent years. To support the development and implementation of the next generation of TMS and related applications, advanced data collection, transmission and processing approaches, digitalised databases, and virtual validation platforms, etc., are required. In the context of the TMS development (addressed by Technology Demonstrator 2.9 of Shift2Rail Innovation Programme 2), this support is to be provided by a scalable, interoperable and standardised communication platform for internal and external communication between different subsystems, applications and clients. This paper outlines the approach of the ongoing OPTIMA project aimed to develop a communication platform demonstrator for railway TMS based on a novel Integration Layer (IL) and its various interfaces to entities including integration layer services, TMS service, rail business service, external services and operator workstations. Further detailed discussion in this paper relates to the approach to validating the communication platform demonstrator as a functional entity, and as a virtual testing environment to validate railway traffic management and other applications. The validation approach for the applications tested on the communication platform demonstrator is also presented. The results of future implementation of this validation approach will be used to assess the functionality of the communications platform demonstrator developed, and the initial TMS applications tested on it, and form an important step towards developing and implementing IL based communications platforms for future TMSs

    Omnidirectional Walking Pattern Generator Combining Virtual Constraints and Preview Control for Humanoid Robots

    This paper presents a novel omnidirectional walking pattern generator for bipedal locomotion combining two structurally different approaches based on the virtual constraints and the preview control theories to generate a flexible gait that can be modified on-line. The proposed strategy synchronizes the displacement of the robot along the two planes of walking: the zero moment point based preview control is responsible for the lateral component of the gait, while the sagittal motion is generated by a more dynamical approach based on virtual constraints. The resulting algorithm is characterized by a low computational complexity and high flexibility, requisite for a successful deployment to humanoid robots operating in real world scenarios. This solution is motivated by observations in biomechanics showing how during a nominal gait the dynamic motion of the human walk is mainly generated along the sagittal plane. We describe the implementation of the algorithm and we detail the strategy chosen to enable omnidirectionality and on-line gait tuning. Finally, we validate our strategy through simulation experiments using the COMAN + platform, an adult size humanoid robot developed at Istituto Italiano di Tecnologia. Finally, the hybrid walking pattern generator is implemented on real hardware, demonstrating promising results: the WPG trajectories results in open-loop stable walking in the absence of external disturbances

    Treatment of squamous cell carcinoma of the anal canal: A new strategies with anti-EGFR therapy and immunotherapy

    The incidence of squamous cell carcinoma of the anal canal (SCAC) is increasing in both sexes but the standard treatment remains that of 20 years ago. However, interesting data have recently emerged on the use of anti-epidermal growth factor receptor (EGFR) agents and immunotherapy in advanced disease. Thus, new avenues of research are opening up that will hopefully lead to more effective therapeutic strategies. We provide an overview of the latest studies published on this tumor and discuss the possible future therapeutic options for combination therapy, anti-EGFR treatment and radiotherapy

    Melanoma metastasis mimicking gastric cancer: a challenge that starts from diagnosis

    The gastrointestinal tract is an uncommon site of metastasis in melanoma. However, when the primary melanoma cannot be found, the diagnosis of gastric melanoma by endoscopic biopsy is problematic mainly because some tumors are amelanotic and do not contain melanin granules detectable by microscopy. A 56-year-old Caucasian man with melanoma was referred to us following an initial histopathological diagnosis via gastroscopy of poorly differentiated primary gastric carcinoma. A computerized tomography (CT) scan showed metastatic disease and on the basis of this information we started palliative chemotherapy. However, the atypical presentation of the disease with subcutaneous metastases prompted us to make a more in-depth evaluation. Immunohistochemical evaluation modified the diagnosis to melanoma. After only one cycle of chemotherapy, treatment was changed to dabrafenib + trametinib, which was better tolerated and initially induced a partial response. The patient is currently in good clinical condition 20 months after diagnosis. Our case report highlights the difficulty in diagnosing melanoma of the gastrointestinal tract and indicates the need for pathologists and clinicians to consider such a possibility when they are faced with a diagnosis of poorly differentiated gastric cancer and unusual sites of metastasis

    Is the outpatient management of acute diverticulitis safe and effective? A systematic review and meta-analysis

    Background: In Western countries, the incidence of acute diverticulitis (AD) is increasing. Patients with uncomplicated diverticulitis can undergo a standard antibiotic treatment in an outpatient setting. The aim of this systematic review was to assess the safety and efficacy of the management of acute diverticulitis in an outpatient setting. Methods: A literature search was performed on PubMed, Scopus, Embase, Central and Web of Science up to September 2018. Studies including patients who had outpatient management of uncomplicated acute diverticulitis were considered. We manually checked the reference lists of all included studies to identify any additional studies. Primary outcome was the overall failure rates in the outpatient setting. The failure of outpatient setting was defined as any emergency hospital admission in patients who had outpatient treatment for AD in the previous 60 days. A subgroup analysis of failure was performed in patients with AD of the left colon, with or without comorbidities, with previous episodes of AD, in patients with diabetes, with different severity of AD (pericolic air and abdominal abscess), with or without antibiotic treatment, with ambulatory versus home care unit follow-up, with or without protocol and where outpatient management is a common practice. The secondary outcome was the rate of emergency surgical treatment or percutaneous drainage in patients who failed outpatient treatment. Results: This systematic review included 21 studies including 1781 patients who had outpatient management of AD including 11 prospective, 9 retrospective and only 1 randomized trial. The meta-analysis showed that outpatient management is safe, and the overall failure rate in an outpatient setting was 4.3% (95% CI 2.6%-6.3%). Localization of diverticulitis is not a selection criterion for an outpatient strategy (p 0.512). The other subgroup analyses did not report any factors that influence the rate of failure: previous episodes of acute diverticulitis (p = 0.163), comorbidities (p = 0.187), pericolic air (p = 0.653), intra-abdominal abscess (p = 0.326), treatment according to a registered protocol (p = 0.078), type of follow-up (p = 0.700), type of antibiotic treatment (p = 0.647) or diabetes (p = 0.610). In patients who failed outpatient treatment, the majority had prolonged antibiotic therapy and only few had percutaneous drainage for an abscess (0.13%) or surgical intervention for perforation (0.06%). These results should be interpreted with some caution because of the low quality of available data. Conclusions: The outpatient management of AD can reduce the rate of emergency hospitalizations. This setting is already part of the common clinical practice of many emergency departments, in which a standardized protocol is followed. The data reported suggest that this management is safe if associated with an accurate selection of patients (40%); but no subgroup analysis demonstrated significant differences between groups (such as comorbidities, previous episode, diabetes). The main limitations of the findings of the present review concern their applicability in common clinical practice as it was impossible to identify strict criteria of failure
