    Lack of correlation between Ki-67 labelling index and tumor size of anterior pituitary adenomas

    AIMS AND BACKGROUND: The Ki-67 is a nuclear antigen detected by the monoclonal antibody MIB-1 and its Labeling Index (LI) is considered a marker of normal and abnormal cell proliferation. Pituitary adenomas are generally well differentiated neoplasms, even if in about one third of cases they are invasive of surrounding tissues. The aim of this study is to evaluate the correlation between Ki-67 labelling index and tumor size of pituitary adenomas extimated by means CT and MRI and confirmed at operation. METHODS: Using the monoclonal antibody MIB-1, we evaluated the expression of Ki-67 in 121 anterior pituitary adenomas consecutively operated on in a 48-month period. RESULTS: In relation to neuroradiological (CT and MRI) and surgically verified tumor size, we identified 24 microadenomas, 27 intrasellar macroadenomas, 34 intra-suprasellar macroadenomas, and 36 intra-supra-parasellar macroadenomas. The adenomas were non-infiltrating (76 cases) and infiltrating (45 cases) adenomas. The wall of the cavernous sinus (CS) was infiltrated in 18 cases. Forty-eight adenomas were non-functioning and 73 functioning. The overall mean ± SD Ki-67 LI was 2.72 ± 2.49% (median 1.6). It was 2.59 ± 1.81 in microadenomas, 2.63 ± 3.45 in intrasellar macroadenomas, 1.91 ± 2.11 in intra-suprasellar macroadenomas, and 3.29 ± 5.45 in intra-supra-parasellar macroadenomas (p = 0.27). It was 3.73 ± 5.13% in infiltrating and 2.03 ± 2.41% in non-infiltrating adenomas (p = 0.02), and 5.61 ± 7.19% in CS-infiltrating versus 2.09 ± 2.37% in CS-non-infiltrating adenomas (p = 0.0005). CONCLUSIONS: Our preliminary results seem to exclude significative correlations between Ki-67 LI and tumor size of anterior pituitary adenomas, even if this index can be considered a useful marker in the determination of the infiltrative behaviour of these tumors

    Tracing the fate of atmospheric nitrate deposited onto a forest ecosystem in Eastern Asia using Δ17O

    The stable isotopic compositions of nitrate in precipitation (wet deposition) and groundwater (spring, lake, and stream water) were determined for the island of Rishiri, Japan, so as to use the 17O anomalies (Δ17O) to trace the fate of atmospheric nitrate that had deposited onto the island ecosystem, which is a representative background forest ecosystem for eastern Asia. The deposited nitrate had large 17O anomalies with Δ17O values ranging from +20.8‰ to +34.5‰ (n = 32) with +26.2‰ being the annual average. The maximum Δ17O value of +34.5‰, obtained for precipitation on the 23rd to 24th of February 2007, was an extraordinarily large value among values for all samples of precipitation in Rishiri. Most nitrate in the sample might have been produced via NO3 radical in a highly polluted air mass that had been supplied from megacities on the eastern coast of the Asian continent. On the other hand, nitrate in groundwater had small Δ17O values ranging from +0.9‰ to 3.2‰ (n = 19), which corresponds to an mixing ratio of atmospheric nitrate to total nitrate of (7.4±2.6)%. Comparing the inflow and outflow of atmospheric nitrate in groundwater within the island, we estimated that the direct drainage accounts for (8.8±4.6)% of atmospheric nitrate that has deposited on the island and that the residual portion has undergone biological processing before being exported from the forest ecosystem

    FRI0349 PSORIATIC ARTHRITIS AND CENTRAL OBESITY: STRONG ASSOCIATION WITH FUNCTIONAL DISABILITY AND A WORSE QUALITY OF LIFE

    Background: Psoriatic arthritis (PsA) is a chronic inflammatory arthritis asso- ciated with comorbidities like obesity, metabolic syndrome, and cardiovascular disease. Adipose tissue leads to a pro-inflammatory status in obese subjects. For this reason, central obesity may determine a worsening in both disability index or quality of life in PsA patients treated with biologic agents. Objectives: Our study aimed to evaluate the relationship between central obe- sity and disability index or the impact of the disease on quality of life in a real- world sample of PsA patients. Methods: A cross-sectional study was conducted. Patients with PsA were enrolled at the PsA clinic at the ARNAS Civico in Palermo (Italy) from March 2018 to December 2019. Clinical, pharmacological, anthropometric, labo- ratoristic variables, and patient-reported outcomes, including the Health Assessment Questionnaire (HAQ) and Psoriatic Arthritis Impact of Disease (PsAID) were evaluated. STATA 14.1 was used to perform statistical analysis. Results: A total of 143 outpatients aged 55.6 (47.7-63.7) affected by PsA, according to CASPAR criteria, were consecutively evaluated. The average years of illness were 10.8 (9.5-12.1). Patients were treated with biological therapy (81.3%), DMARDS (41.6%), small molecules (9.9%), or their combinations. Both sexes were equally represented. 71.9% of enrolled patients had central obesity (64.9% men and 78.1% women) with an average waist circumference of 104.2 (101.8 - 106.6) for women and 103.6 (100.0 -107.2) for men. Average HAQ was 1.05 (0.92 - 1.19), and data analysis showed 50.3% of patients with normal-mild functional disability, 30.1% moderate to severe disability, and 19.6% severe to very severe disability [Fig 1]. 51,7% of the sample had a high impact of the dis- ease on life, according to the PsAID questionnaire [Fig 2]. A strong association was observed between functional disability measured by HAQ &gt;2 and central obesity [OR (95% CI) 16.94 (2.22 - 129.48); p &lt; 0.006]. Moreover, data analysis showed an association between high impact of disease on life (PsAID &gt;4) and central obesity [OR (95% CI) 3.33 (1.56 - 7.13); p&lt;0,002].Conclusion: Our study demonstrated a high association between functional dis- ability studied subjectively using the HAQ, the impact of the disease on patients’ quality of life using PsAID, and central obesity in Sicilian outpatients affected by PsA. Data suggest that therapeutic goals should not be focused on treatment but also on waist circumference reduction in order to reduce inflammation and improve patients’ functional ability and quality of life
