    Ischemia with endovascular balloon during disarticulations and amputations of limbs

    Objective Limb disarticulation has been widely performed since the 18th century, especially in war surgery. Actually is infrequently done in orthopaedic and vascular surgery, and it is associated with a high mortality rate because of frequent comorbidities. Disarticulation usually is reserved for patients with malignant tumours or gangrene from severe artherosclerosis. During disarticulation, hemodynamic stability can be altered by hemorrhagic events in the femoral or humeral arteries. We propose an endovascular technique for proximal control of the artery to reduce blood loss during disarticulation. Our experience today is limited at hip disarticulation. Material and methods The vascular access was percutaneous at the common femoral artery of the healthy limb. A 6 French (Fr) introducer sheath was placed using the Seldinger technique. Under fluoroscopic control, with a portable vascular C-arm capable of digitally subtracter angiogram and roadmap angiography, a 0.035 inch hydrophilic guide wire was crossed aver into the opposite side iliac artery through a 5F contra angiographic catheter placed at the aortic bifurcation. After a diagnostic angiography the guide wire was replaced with an Amplatz 0.0035 inch, 260 cm long, super stiff guide wire. Then, a 7 9 20 mm Ultra-thinTM SDS balloon catheter was placed in the external iliac artery and systemic heparinization with 2500 UI was performed. The balloon catheter was inflated and femoral pulsation ceased immediately. After proximal, endovascular occlusion, hip disarticulation was accomplished without any hemorrhagic complication. At the end of procedure, the balloon was deflated and removed. Hemostasis of the surgical field completed the procedure. The femoral access in the healthy common femoral artery was controlled with a 6 Fr Angio-seal percutaneous hemostatic system. Results and discussion In hip disarticulation, hemostatic tourniquets cannot be used of the location of the operating field. Therefore, control of bleeding is a major issue in this procedure. Various techniques have been proposed, femoral vessels and nerves were attached before the disarticulation. The use of semi-compliant balloon catheters for endovascular occlusion avoids injury to the endothelium of the vessel wall during balloon inflation. However preoperative assessment, with color-duplex scanning and plain abdominal radiographs, is mandatory; coexisting atherosclerosis often is present especially in elderly patients, and severe wall calcification can lead to vessel rupture and retroperitoneal hematoma, or even balloon catheter rupture. Moreover, color-duplex scanning and radiographs will help in choosing the landing-zone for balloon inflation. Conclusions Endovascular balloon assistance is a simple, safe and effective technique in preventing major arterial bleeding during amputation or disarticulation and can be routinely used

    Composition and Epigenetic Markers of Heterochromatin in the Aphid Aphis nerii (Hemiptera: Aphididae).

    A detailed karyotype analysis of the oleander aphid Aphis nerii focusing on the distribution, molecular composition and epigenetic modifications of heterochromatin was done in order to better understand the structure and evolution of holocentric/holokinetic chromosomes in aphids. The female karyotype (2n = 8) consisted of 3 pairs of autosomes and a pair of X chromosomes that were the longest elements in the karyotype and carried a single, terminally located nucleolar organizer region. Males showed 2n = 7 chromosomes due to the presence of a single X chromosome. Heterochromatin was located in the X chromosomes only and consisted of 4 satellite DNAs that have been identified. A. nerii constitutive heterochromatin was enriched in mono-, di- and tri-methylated H3 histones and HP1 proteins but, interestingly, it lacked DNA methylation that was widespread in euchromatic chromosomal regions. These results suggest that aphid heterochromatin is assembled and condensed without any involvement of DNA methylation

    Poorly differentiated clusters (PDC) in colorectal cancer: Does their localization in tumor matter?

    Poorly differentiated clusters (PDC) are aggregates of at least five neoplastic cells lacking evidence of glandular differentiation. By definition, they can be present at the invasive front (peripheral PDC or pPDC) and within the tumor stroma (central PDC or cPDC). In colorectal cancer (CRC), PDC are considered adverse prognosticators and seem to reflect epithelial mesenchymal transition (EMT). In this study, we have investigated the immuno-expression of two EMT-related proteins, E-cadherin and β-catenin, in PDC of primary CRCs and matched liver metastases. pPDC always showed nuclear β-catenin staining and diffusely reduced/absence of E-cadherin expression as opposed cPDC which showed nuclear β-catenin immunoreactivity and E-cadherin expression in about 50% of cases. In addition, the pattern of β-catenin and E-cadherin expression differed between PDC and the main tumor, and between primary CRC and liver metastasis (LM), in a percentage of cases. A discordant pattern of β-catenin and E-cadherin expression between pPDC and cPDC, between main tumor and cPDC, and between primary CRC and LM, confirms that EMT is a dynamic and reversible process in CRC. On the overall, this suggests that pPDC and cPDC are biologically different. We may advocate that PDC develop at the tumor center (cPDC) and then some of them migrate towards the tumor periphery while progressively completing EMT process (pPDC). Based on these results, PDC presence and counting may have different prognostic relevance if the assessment is done at the invasive front of the tumor or in the intratumor stroma

    Agronegócio de Mato Grosso: uma análise insumo-produto.

    O objetivo deste trabalho foi avaliar o agronegócio de Mato Grosso, quanto à sua estrutura produtiva, e sua importância tanto para o estado quanto para a economia do País. O agronegócio é um setor estratégico e pode atualmente ser considerado o grande condicionante do desempenho da economia brasileira. Mato Grosso destaca-se como importante polo agropecuário do País, e as características climáticas ideais para a atividade são naturais no estado. Esses fatores, quando associados à modernização das práticas agrícolas, inovações tecnológicas e políticas setoriais, proporcionam altos níveis de produção e produtividade. Por meio dos modelos de insumo-produto, foi possível estimar a importância relativa das principais atividades do agronegócio mato-grossense nos contextos regional e nacional. Os setores diretamente relacionados ao agronegócio respondem por 27,6% do valor adicionado de Mato Grosso. Porém, sua importância vai a 34,8%, quando se consideram as relações de compra e venda que o setor de agronegócio mantém com os demais setores da economia. O trabalho apresenta também os indicadores da complexidade da economia estadual, como índices de ligações e multiplicadores de produto, valor adicionado e emprego. Os principais resultados indicam que diversas atividades diretamente relacionadas ao agronegócio se destacam em importância relativa, dentro da estrutura produtiva do estado, para todos os indicadores econômicos calculados a partir da matriz de insumo-produto. Os resultados referentes aos multiplicadores indicam que diversas atividades relacionadas ao agronegócio desempenham papel importante na economia mato-grossense pela capacidade de aumento da produção e da geração de emprego e de valor adicionado

Through the input-output models has been possible to estimate the importance of gross domestic product generated by the agribusiness family. The results show that agribusiness family production in Rio Grande do Sul is of high importance in GDP not only in agribusiness, but in the overall state economy

    Emergence of a HER2-amplified clone during disease progression in an ALK-rearranged NSCLC patient treated with ALK-inhibitors: A case report

    Anaplastic lymphoma kinase tyrosine kinase inhibitors (ALK-TKIs) are the standard treatment for advanced ALK-positive non-small cell lung cancer (NSCLC) allowing survivals up to 5 years. However, duration of responses is limited by the almost certain occurrence of drug resistance. Here, we report a case of a never smoker, 59-year-old female with metastatic ALK-positive adenocarcinoma, solid and signet ring patterns, who developed resistance to alectinib, a second-generation ALK-TKI, mediated by HER2 gene amplification. The patient received 22 months of crizotinib as first-line and subsequently 1-year of alectinib therapy. A study of resistance mechanism was performed with next generation sequencing (NGS) on tissue re-biopsy. A HER2-amplified emerging clone was identified by NGS in a liver metastasis and confirmed by fluorescent in situ hybridization (FISH) analysis. The resistant clone was detectable 2 months before disease progression in plasma cell-free DNA (cfDNA) using digital droplet PCR (ddPCR) copy number variation (CNV) assay and it was retrospectively traced in rare cells of the lung primary by FISH. To our best knowledge, this is first evidence of HER2 gene amplification as a resistance mechanism to ALK-TKI in a NSCLC. Future strategies against oncogene-addicted NSCLC might benefit of combined drug treatments, such as ALK and HER2 inhibition

    Comparação Entre O Agronegócio Familiar Do Rio Grande Do Sul E Do Brasil

    This goal of this paper is to measure and to make an analysis of the level of the familiar agricultural agribusiness in the state of Rio Grande do Sul, and comparing it with the one for the Brazilian economy as a whole, for the period from 1995 to 2003. Using input-output models it was possible to estimate the importance of the Gross Domestic Product of the familiar agribusiness in the national economy. The results show that around 1/3 of the Brazilian agribusiness comes from the agricultural production done by the familiar agriculture, the numbers are even more stricken for the state of Rio Grande do Sul, where 1/2 of the agribusiness GDP comes from the familiar agriculture. It was also noticed that the recent development of the familiar agriculture and of its links has been very positive, suppressing, the growth rates found in the non-familiar segment
