    Fistula Campaigns-Are They of Any Benefit?

    Objective Evaluation of the problems encountered during a voluntarily fistula campaign in a regional hospital of Niger (Africa) Materials and Methods Women underwent basic gynecological examination, methylene blue testing, and/or direct cystoscopy as necessary According to their clinical condition, women were informed and surgical options offered as appropriate Operations were performed under spinal or epidural anesthesia Immediate postoperative outcomes were followed during the stay of the surgical team in the country Results A total of 62 women were examined and 11 had causes of incontinence other than obstetric fistula In 9 8% of the women, severe local infection precluding any surgical intervention was evident In 58 8% of patients, the trigonal region and/or urethra were irreversibly damaged A proportion of patients (9 8%) with large lesions and intact urethra that were offered vaginal layered closure refused the intervention Of the women that were operated on (21 6%), six underwent vaginal layered closure with Martius fat flap and five women underwent a combined abdomino vaginal approach Conclusion It is extremely difficult to meet the needs of this global problem with short term programs and volunteers Directing these efforts to specialist fistula centers and creating reliable scientific evidence should be the main goal [Taiwan J Obstet Gynecol 2010,49(3) 291-296

    Andersen Tawil Sendromunda Anestezi Uygulaması

    Andersen Tawil sendromu kardiyak disritmi, periyodik paralizi ve gelişim deformiteleriyle karakterize olan nadir görülen genetik bir hastalıktır. Sendromda gözlenen kardiyak bulgular ventriküler aritmi (%84), uzun QT sendromu (%50), anormal TU dalga paterni (%73) ve ani kardiyak arresttir (%10). Periyodik paraliziler sıklıkla hipokalemide görülmekte ve bu durumda kardiyak aritmi sıklığı artmaktadır. Fiziksel görünümde ise kısa boy, hipertelorizm, hafif bilateral ptosis, düşük kulak seviyesi, protrude alın, hipoplastik mandibula ve sindaktili bildirilmektedir. Bu olgu ile Andersen Tawil sendromu tanısı bilinen bir hastanın rinoplasti ameliyatı sırasındaki anestezi uygulamasını sunmayı amaçladık

    Technical efficiency and total factor productivity growth in the Hazelnut Agricultural Sales Cooperatives Unions in Turkey

    This study measures the production efficiencies and total factor productivity changes of Hazelnut Agricultural Sales Cooperatives Unions (HASCUs) in Turkey over the period 2004-2008. Turkey produces %78.2 hazelnut production of world and gives service to 233,820 farmers in the Black Sea Region of Turkey. To measure production efficiencies of these units, Data Envelopment Analysis (DEA) Approach and Malmquist Productivity Indexes are used. In the our efficiency analysis, the data of 50 HASCUs is considered but only the data of 37 cooperative units is found appropriate for the DEA. The findings of our study show that the average annual technical efficiency scores of HASCUs change between 0.841 and 0.938. It has also been observed that there are average annual %1.3 improvements in technical efficiency, 3% regress in technical change, and %1.7 decreases in the total factor productivity of the HASCUs over the period 2004-2008. © Association for Scientific Research

    Baseline clinical characteristics and patient profile of the TURKMI registry: Results of a nation-wide acute myocardial infarction registry in Turkey

    Objective: The TURKMI registry is designed to provide insight into the characteristics, management from symptom onset to hospital discharge, and outcome of patients with acute myocardial infarction (MI) in Turkey. We report the baseline and clinical characteristics of the TURKMI population. Methods: The TURKMI study is a nation-wide registry that was conducted in 50 centers capable of percutaneous coronary intervention selected from each EuroStat NUTS region in Turkey according to population sampling weight, prioritized by the number of hospitals in each region. All consecutive patients with acute MI admitted to coronary care units within 48 hours of symptom onset were prospectively enrolled during a predefined 2-week period between November 1, 2018 and November 16, 2018. Results: A total of 1930 consecutive patients (mean age, 62.0±13.2 years; 26.1% female) with a diagnosis of acute MI were prospectively enrolled. More than half of the patients were diagnosed with non-ST elevation MI (61.9%), and 38.1% were diagnosed with ST elevation MI. Coronary angiography was performed in 93.7% and, percutaneous coronary intervention was performed in 73.2% of the study population. Fibrinolytic therapy was administered to 13 patients (0.018%). Aspirin was prescribed in 99.3% of the patients, and 94% were on dual antiplatelet therapy at the time of discharge. Beta blockers were prescribed in 85.0%, anti-lipid drugs in 96.3%, angiotensin converting enzyme inhibitors in 58.4%, and angiotensin receptor blockers in 7.9%. Comparison with European countries revealed that TURKMI patients experienced MI at younger ages compared with patients in France, Switzerland, and the United Kingdom. The most prevalent risk factors in the TURKMI population were hypercholesterolemia (60.2%), hypertension (49.5%), smoking (48.8%), and diabetes (37.9%). Conclusion: The nation-wide TURKMI registry revealed that hypercholesterolemia, hypertension, and smoking were the most prevalent risk factors. TURKMI patients were younger compared with patients in European Countries. The TURKMI registry also confirmed that current treatment guidelines are largely adopted into clinical cardiology practice in Turkey in terms of antiplatelet, anti-ischemic, and anti-lipid therapy. ©Copyright 2020 by Turkish Society of Cardiolog
