Evaluation of antianginal channel- and receptor blockers in a rat model of coronary arteriogenesis
Zielsetzung: Ziel der Arbeit war es die, bereits in reinen Infarkt-und
Infarkt-Reperfusionsmodellen untersuchte, kardioprotektive Wirkung von
antianginös wirkenden Kanal-und Rezeptorblockern erstmals auch in einem
koronaren Arteriogenesemodell der Ratte zu evaluieren. Methoden: Das
repetitiv-okklusive Modell fand Anwendung in 32 männlichen Sprague-Dawley-
Ratten. Die Tiere wurden zunächst in 5 Gruppen eingeteilt: repetitiv-
okklusives Protokoll (ROP), SHAM (Kontrollgruppe), ROP + Metoprolol (METO:
250mg/kg/d p.o.), ROP + Verapamil (VERA: 7,5mg/kg/d p.o.), und ROP + Ranolazin
(RANO: 47mg/kg/d p.o.). Allen Tieren wurde in einer Initialoperation ein
pneumatischer Miniokkluder auf die Left anterior descending artery (LAD)
implantiert. Die Gruppen mit Ausnahme der SHAM-Gruppe durchliefen anschließend
über 10 Tage das ROP. Mit Beginn des Protokolls wurde die erste
Medikamentendosis appliziert. Am 12. postoperativen Tag erfolgten die
experimentelle Infarktsetzung mittels 90-minütiger Okklusion sowie eine
20-minütige Reperfusionsphase. Zur Evaluation der medikamentösen Wirkung
wurden die Infarktgrößen bestimmt, elektrokardiographische Daten ausgewertet
und immunhistochemische Färbungen angefertigt. Ergebnisse: Bei der Analyse der
durchschnittlichen Infarktvolumina zeigten alle untersuchten Gruppen ein
durchschnittlich größeres Infarktareal. Obwohl die Infarktvolumina der SHAM-
und RANO-Gruppe doppelt so groß wie die der ROP-Gruppe ausfielen, waren die
Unterschiede statistisch nicht signifikant (SHAM: p = 0,089; RANO: p = 0,062).
Bei der Auswertung der elektrokardiographischen Daten zeigte die RANO-Gruppe
eine signifikant höhere ST-Hebung im Vergleich zur ROP-Gruppe. Trotz höherer
ST-Hebungen im Vergleich zur ROP-Gruppe waren die Ergebnisse der SHAM- und
VERA-Gruppe nicht signifikant. Die METO-Gabe hatte keinen Einfluss auf das
Ausmaß der ST-Hebung. Bei der Analyse des prozentualen R-Amplitudenverlustes
zeigten sowohl die SHAM- als auch die VERA-Gruppe signifikant höhere Werte als
die ROP-Gruppe. Die anderen Gruppen zeigten keinen Einfluss auf den
R-Amplitudenverlust. Sowohl die VERA- als auch die RANO- Gruppe zeigten ein
häufigeres Auftreten von höhergradigen Herzrhythmusstörungen als die ROP-
Gruppe. In der METO-Gruppe zeigte sich kein Einfluss auf die
Herzrhythmusstörungen. Schlussfolgerung: Für keines der untersuchten
Medikamente ließ sich im koronaren Arteriogenesemodell der Ratte eine
kardioprotektive Wirkung nachweisen. Insbesondere die Medikamente mit einem
Einfluss auf die Calciumhomöostase zeigten die Tendenz zu einem negativen
Effekt im repetitiv-okklusiven Modell.Objective: Our goal was to evaluate for the first time the effect of
antianginal channel- and receptor-blockers in a rat model of coronary
arteriogenesis. Approach and Results: First 32 Sprague-Dawley rats were
divided into 5 groups: repetitive-occlusive protocol (ROP), SHAM, ROP +
Metoprolol, ROP + Verapamil, ROP + Ranolazin. Subsequently each group except
for the SHAM-group had to undergo the repetitive-occlusive protocol for ten
days. The experimental infarct was initiated by an occlusion of 90 minutes
followed by a reperfusion of 20 minutes. Analysing the average infarct volume
indicated within each group the average infarct area was greater compared to
that of the ROP-group. Though the infarct volumes of the SHAM- and RANO-group
were twice the volume of the ROP-group the differences were not statistically
significant. Evaluation of the electrocardiographic data showed that the RANO-
group had significantly higher ST-segment elevation than the ROP-group.
Despite higher ST-segment elevation compared to the ROP-group the results of
the SHAM- and VERA-group were not significant. Administration of METO had no
impact on the extent of ST-segment elevation. Analysis of the percentage
R-amplitude loss resulted in significantly higher loss of the SHAM- and Vera-
group compared to the ROP-group. There was no effect on R-amplitude loss in
any of the other groups. Both VERA- and RANO-group were more frequently
affected by high-grade cardiac arrythmias than the ROP-group. Administration
of METO had no effect on cardiac arrythmias. Conclusion: None of the
medication examined showed protective effect in the rat model of coronary
arteriogenesis. Particularly drugs affecting Calcium Homeostasis tended to
have negative effects in the repetitive-occlusive model

BACKGROUND:In the presence of a coronary occlusion, pre-existing small collateral vessels (arterioles) develop into much larger arteries (biological bypasses) that have the potential to allow a certain level of perfusion distal to the blockage. Termed arteriogenesis, this phenomenon proceeds via a complex combination of events, with nitric oxide (NO) playing an essential role. The aim of this study was to investigate the effects of supplemental administration of NO donors, i.e., short-acting nitroglycerin (NTG) or slow-release pelleted isosorbide dinitrate (ISDN), on collateral development in a repetitive coronary artery occlusion model in rats. METHODS:Coronary collateral growth was induced via a repetitive occlusion protocol (ROP) of the left anterior descending coronary artery (LAD) in rats. The primary endpoints were the histological evaluation of rat heart infarct size and ST-segment elevation (ECG-analysis) upon final permanent occlusion of the LAD (experimentally induced myocardial infarction). The effects of NTG or ISDN were also evaluated by administration during 5 days of ROP. We additionally investigated whether concomitant application of NTG can compensate for the anti-arteriogenic effect of acetylsalicylic acid (ASA). RESULTS:After 5 days of ROP, the mean infarct size and degree of ST-elevation were only slightly lower than those of the SHAM group; however, after 10 days of the protocol, the ROP group displayed significantly less severe infarct damage, indicating enhanced arteriogenesis. Intermittent NTG application greatly decreased the ST-elevation and infarct size. The ISDN also had a positive effect on arteriogenesis, but not to the same extent as the NTG. Administration of ASA increased the infarct severity; however, concomitant dosing with NTG somewhat attenuated this effect. CONCLUSION:Intermittent treatment with the short-acting NTG decreased the size of an experimentally induced myocardial infarct by promoting coronary collateral development. These new insights are of great relevance for future clinical strategies for the treatment of occlusive vascular diseases
