    Conflicting cultures - a street-ethnographic take on urban youth, unstructured socialization and territoriality

    "This article is about conflicting cultures among urban youth in a medium-sized Danish town called Lomby. At the central squares in Lomby different groups of children and young people gather around the newly established skater facility. Concentrating on a specific group of young boys, the pseudo-gang called the Thugz, the article illustrates how conflictual behaviour is a key component in the everyday lives of the various confronting groups present at the site. The analysis also depicts the Thugz' attitudes towards authorities like the SSP (a special Social Services, School and Police unit) and the police and in the summarizing part of the article, also the groups’ reflections on school."     Emerging roles of calcium-activated K channels and TRPV4 channels in lung oedema and pulmonary circulatory collapse

    It has been suggested that the transient receptor potential cation (TRP) channel subfamily V (vanilloid) type 4 (TRPV4) and intermediate conductance calcium-activated potassium (KCa3.1) channels contribute to endothelium-dependent vasodilation. Here, we summarize very recent evidence for a synergistic interplay of TRPV4 and KCa3.1 channels in lung disease. Among the endothelial Ca2+-permeable TRPs, TRPV4 is best characterized and produces arterial dilation by stimulating Ca2+-dependent nitric oxide synthesis and endothelium-dependent hyperpolarization. Besides these roles, some TRP channels control endothelial/epithelial barrier functions and vascular integrity, while KCa3.1 channels provide the driving force required for Cl- and water transport in some cells and most secretory epithelia. The three conditions, increased pulmonary venous pressure caused by left heart disease, high inflation pressure and chemically induced lung injury, may lead to activation of TRPV4 channels followed by Ca2+ influx leading to activation of KCa3.1 channels in endothelial cells ultimately leading to acute lung injury. We find that a deficiency in KCa3.1 channels protects against TRPV4-induced pulmonary arterial relaxation, fluid extravasation, haemorrhage, pulmonary circulatory collapse and cardiac arrest in vivo. These data identify KCa3.1 channels as crucial molecular components in downstream TRPV4 signal transduction and as a potential target for the prevention of undesired fluid extravasation, vasodilatation and pulmonary circulatory collapse

    Talking at cross purposes: Negotiation of the sick role with a MUS patient as a real time social process

    Medically unexplained symptoms (MUS) challenge our perception of illness. As a consequence, a MUS-patient’s legitimised entry into the sick role, as defined by Parsons, poses a problem for physicians and patients alike. The encounter between patients with MUS and physicians therefore can be frustrating for both parties. The present study is a single case analysis of such an encounter in primary care. Using the microanalytical method of conversation analysis, this study aims to provide a contextualised description of both the patient’s and the physician’s different orientations to the purpose of the encounter. This approach locates the source of the frustration in the participants’ incompatible interactional projects. The patient is oriented towards the emotional and life world aspects of her situation. She is pursuing recognition, not only of the destructive effect that the symptoms have on all aspects of her life, but also of herself as a morally sound person. The physician, on the contrary, is oriented towards avoiding to psychologise the patient and keeping the focus on reaching an agreement about future clinical action. The physician uses ancillary questions to refocus the emotionally loaded contributions from the patient and maintain structured progression. Consequently, physician and patient are talking at cross purposes, and it is argued that this is a structurally facilitated pitfall of the conventional format of primary care visits that physicians are trained to adhere to
