The authors focused on three perspectives: access, equity, and achievement about students\u27 academic success in their K-12 educational experiences and how the quality of their education and test scores impacted their college acceptance rate, respectively. The three perspectives are specific to the works of Dr. Asa G. Hilliard and Dr. Barbara A. Sizemore, both of whom were passionate about the academic success and progression of students of African descent. Their many works embody a fight against standardized testing for students, especially those of color, and work toward a more sophisticated knowledge assessment.     eRegistries: Electronic registries for maternal and child health

    Background: The Global Roadmap for Health Measurement and Accountability sees integrated systems for health information as key to obtaining seamless, sustainable, and secure information exchanges at all levels of health systems. The Global Strategy for Womenā€™s, Childrenā€™s and Adolescentā€™s Health aims to achieve a continuum of quality of care with effective coverage of interventions. The WHO and World Bank recommend that countries focus on intervention coverage to monitor programs and progress for universal health coverage. Electronic health registries - eRegistries - represent integrated systems that secure a triple return on investments: First, effective single data collection for health workers to seamlessly follow individuals along the continuum of care and across disconnected cadres of care providers. Second, real-time public health surveillance and monitoring of intervention coverage, and third, feedback of information to individuals, care providers and the public for transparent accountability. This series on eRegistries presents frameworks and tools to facilitate the development and secure operation of eRegistries for maternal and child health. Methods: In this first paper of the eRegistries Series we have used WHO frameworks and taxonomy to map how eRegistries can support commonly used electronic and mobile applications to alleviate health systems constraints in maternal and child health. A web-based survey of public health officials in 64 low- and middleincome countries, and a systematic search of literature from 2005ā€“2015, aimed to assess country capacities by the current status, quality and use of data in reproductive health registries. Results: eRegistries can offer support for the 12 most commonly used electronic and mobile applications for health. Countries are implementing health registries in various forms, the majority in transition from paperbased data collection to electronic systems, but very few have eRegistries that can act as an integrating backbone for health information. More mature country capacity reflected by published health registry based research is emerging in settings reaching regional or national scale, increasingly with electronic solutions. 66 scientific publications were identified based on 32 registry systems in 23 countries over a period of 10 years; this reflects a challenging experience and capacity gap for delivering sustainable high quality registries. Conclusions: Registries are being developed and used in many high burden countries, but their potential benefits are far from realized as few countries have fully transitioned from paper-based health information to integrated electronic backbone systems.     Fire-Environment Analysis: An Example of Army Garrison Camp Williams, Utah

    The planning of fuel treatments for ecological or societal purposes requires an in-depth understanding of the conditions associated with the occurrence of free-burning fire behavior for the area of concern. Detailed fire-environment analysis for Army Garrison Camp Williams (AGCW) in north-central Utah was completed as a prerequisite for fuel treatment planning, using a procedure that could be generally applied. Vegetation and fuels data, topographic and terrain features, and weather and climate data, were assessed and integrated into predictive fuel models to aid planning. A fire behavior fuel model map was developed from biophysical variables, vegetation type, and plot survey data using random forests, and resulted in an overall classification rate of 72%. The predominate vegetation type-fuel complex was grass, followed by lesser amounts of Gambel oak, Wyoming big sagebrush and Utah juniper. The majority of AGCW is mountainous in nature, characterized by slopes less than 40% in steepness with slightly more northerly and easterly aspects than south and west, and elevations that ranged from 1650 to 1950 m above mean sea level. Local fire weather data compiled from the three nearest remote automated weather stations indicated that average temperature maxima (32ā—¦C) and relative humidity minima (12%) usually occurred between 1400 to 1500 h daily, and from July to August, seasonally. The semi-arid climate at AGCW, coupled with the corresponding preponderance of flashy fuel types and sloping terrain, constitutes a formidable fire environment in which to plan for mitigating against adverse fire behavior

    Combination therapy with two antimicrobial agents is superior to monotherapy in severe community-acquired pneumonia, and recent data suggest that addition of a macrolide as the second antibiotic might be superior to other combinations. This observation requires confirmation in a randomised control trial, but this group of antibiotics have pleiotropic effects that extend beyond bacterial killing. Macrolides inhibit bacterial cell-to-cell communication or quorum sensing, which not only might be an important mechanism of action for these drugs in severe infections but may also provide a novel target for the development of new anti-infective drugs

    A series of half-sandwich arene ruthenium complexes containing bidentate thiosemicarbazone ligands have been synthesized and their biological activity investigated. The compounds have the general formula [(6-p-cymene)Ru(R-ATSC)Cl]X (ATSC = 9-anthraldehyde thiosemicarbazone and R = H, CH3 and C6H5). The crystal structure of [(6-p-cymene) Ru(MeATSC)Cl]Cl have been determined and represents the first structurally characterized areneā€“ruthenium half-sandwich complex with a thiosemicarbazone ligand. The complexes show good cytotoxic profiles against MCF-7 and MDA-MB-231 (breast adenocarcinoma) as well as HCT 116 and HT-29 (colorectal carcinoma) cell lines. [Refer to PDF for graphical abstract

    The MH1 domain of Smad3 interacts with Pax6 and represses autoregulation of the Pax6 P1 promoter

    Pax6 transcription is under the control of two main promoters (P0 and P1), and these are autoregulated by Pax6. Additionally, Pax6 expression is under the control of the TGFĪ² superfamily, although the precise mechanisms of such regulation are not understood. The effect of TGFĪ² on Pax6 expression was studied in the FHL124 lens epithelial cell line and was found to cause up to a 50% reduction in Pax6 mRNA levels within 24ā€‰h. Analysis of luciferase reporters showed that Pax6 autoregulation of the P1 promoter, and its induction of a synthetic promoter encoding six paired domain-binding sites, were significantly repressed by both an activated TGFĪ² receptor and TGFĪ² ligand stimulation. Subsequently, a novel Pax6 binding site in P1 was shown to be necessary for autoregulation, indicating a direct influence of Pax6 protein on P1. In transfected cells, and endogenously in FHL124 cells, Pax6 co-immunoprecipitated with Smad3 following TGFĪ² receptor activation, while in GST pull-down experiments, the MH1 domain of Smad3 was observed binding the RED sub-domain of the Pax6 paired domain. Finally, in DNA adsorption assays, activated Smad3 inhibited Pax6 from binding the consensus paired domain recognition sequence. We hypothesize that the Pax6 autoregulatory loop is targeted for repression by the TGFĪ²/Smad pathway, and conclude that this involves diminished paired domain DNA-binding function resulting from a ligand-dependant interaction between Pax6 and Smad3

    Predicting the costs of managing patients with chronic obstructive pulmonary disease

    SummaryThe economic consequences of chronic obstructive pulmonary disease (COPD) are considerable, although the factors that best predict costs are largely unknown. This study used a population-based cohort to identify the clinical factors during an index year that were most predictive of increased direct medical costs in the subsequent year, and to develop a predictive model that described the cost variations in COPD.The medical records of 2116 patients enrolled in one regional health system who had COPD and healthcare resource utilisation data for 1998 and 1999, were abstracted for information about symptoms, smoking history, chronic illnesses, and pulmonary function data. All inpatient, outpatient and pharmacy utilisation data for each subject for 1999 were extracted from the database. Total costs for each individual were transformed to a log scale. Potential causes of cost variability (predictor variables) were defined and classified into sets (or domains). Multiple linear regression models were fitted for each domain.The study demonstrated that severity of airflow obstruction, as assessed by FEV1% predicted, is a significant but weak predictor of future healthcare resource utilisationā€”prior hospitalisation and home oxygen use, the presence of comorbid conditions and symptoms of dyspnoea are better predictors of costs. Those interested in the economic benefits of new COPD treatments and disease management programs need to carefully account for these factors
