    Welcoming Remarks

    In this transcript, first, Mr. Michael Cooper, President of the Association of the Bar of the City of New York offers welcoming remarks to the conference, describing briefly the work of the Association’s and its Special Commission on Campaign Finance Reform and the focus of the conference, specifically what can be done in the field of federal campaign finance reform based on what has been done at the state and local levels. Second, chair of the Campaign Finance Board, Father O’Hare, addressed the conference focusing on the non-partisan Board of the Campaign Finance Program in New York City as a model for overcoming the impediment to federal reform, that lawmakers view the problem in partisan terms. Father O’Hare elaborated on the purpose of the conference, briefly described new amendments to the New York City Program, an overview of the conference’s programs and speakers, and finally, a brief introduction for the first speaker, Mr. Fritz Schwarz. This transcript was taken at From the Ground Up: Local Lessons for National Reform, a national conference on campaign finance reform held on November 9, 1998, sponsored by the New York City Campaign Finance Board and the Association of the Bar of the City of New York

    Colorectal cancer screening of high-risk populations: A national survey of physicians

    <p>Abstract</p> <p>Background</p> <p>The incidence of colorectal cancer can be decreased by appropriate use of screening modalities. Patients with a family history of colon cancer and of African-American ethnicity are known to be at higher risk of developing colorectal cancer. We aimed to determine if there is a lack of physician knowledge for colorectal cancer screening guidelines based on family history and ethnicity. Between February and April 2009 an anonymous web-based survey was administered to a random sample selected from a national list of 25,000 internists, family physicians and gastroenterologists. A stratified sampling strategy was used to include practitioners from states with high as well as low CRC incidence. All data analyses were performed following data collection in 2009.</p> <p>Results</p> <p>The average knowledge score was 37 ± 18% among the 512 respondents. Gastroenterologists averaged higher scores compared to internists, and family physicians, <it>p </it>= 0.001. Only 28% of physicians correctly identified the screening initiation point for African-Americans while only 12% of physicians correctly identified the screening initiation point and interval for a patient with a family history of CRC. The most commonly cited barriers to referring high-risk patients for CRC screening were "patient refusal" and "lack of insurance reimbursement."</p> <p>Conclusions</p> <p>There is a lack of knowledge amongst physicians of the screening guidelines for high-risk populations, based on family history and ethnicity. Educational programs to improve physician knowledge and to reduce perceived barriers to CRC screening are warranted to address health disparities in colorectal cancer.</p

    Inhibitor of Apoptosis Proteins as Novel Targets in Inflammatory Processes

    Objective: Inhibitor of apoptosis proteins (IAPs), such as X-linked or cellular IAP 1/2 (XIAP, cIAP1/2), are important regulators of apoptosis. IAP antagonists are currently under clinical investigation as anticancer agents. Interestingly, IAPs participate in the inflammation-associated TNF receptor signaling complex and regulate NFκB signaling. This raises the question about the role of IAPs in inflammation. Here, we investigated the anti-inflammatory potential of IAP inhibitors and the role of IAPs in inflammatory processes of endothelial cells. Methods and Results: In mice, the small molecule IAP antagonist A-4.10099.1 (ABT) suppressed antigen-induced arthritis, leukocyte infiltration in concanavalin A-evoked liver injury, and leukocyte transmigration in the TNFα-activated cremaster muscle. In vitro, we observed an attenuation of leukocyte– endothelial cell interaction by downregulation of the intercellular adhesion molecule-1. ABT did not impair NFκB signaling but decreased the TNFα-induced activation of the TGF-β–activated kinase 1, p38, and c-Jun N-terminal kinase. These effects are based on the proteasomal degradation of cIAP1/2 accompanied by an altered ratio of the levels of membrane-localized TNF receptor-associated factors 2 and 5. Conclusion: Our results reveal IAP antagonism as a profound anti-inflammatory principle in vivo and highlight IAPs as important regulators of inflammatory processes in endothelial cells

    Two glaciers and one sedimentary sink: the competing role of the Aare and the Valais glaciers in filling an overdeepened trough inferred from provenance analysis

    The extent and distribution of glaciers on the Swiss Plateau during the Last Glacial Maximum (LGM) can be determined from the geological record. However, similar reconstructions for the glaciations that preceded the LGM are far more difficult to be made due to the destruction of suitable sedimentary records through recurring glaciations or due to the inaccessibility of preserved records. Here, we explored Quaternary sediments that were deposited during the Marine Isotope Stage (MIS) 8 glaciation at least around 250 ka, and which were recovered in a drilling that was sunk into an overdeepened bedrock trough west of Bern (Switzerland). We analyzed the sediment bulk chemical composition of the deposits to investigate the supply of the material to the area by either the Aare Glacier, the Saane Glacier, or the Valais Glacier, and we complement this investigation with the results of heavy mineral analyses and geochemical information from detrital garnet. The potential confluence of the Valais and the Aare glaciers in the Bern area makes this location ideal for such an analysis. We determined the sediment bulk chemical signal of the various lithological units in the central Swiss Alps where the glaciers originated, which we used as endmembers for our provenance analysis. We then combined the results of this fingerprinting with the existing information on the sedimentary succession and its deposition history. This sedimentary suite is composed of two sequences, Sequence A (lower) and Sequence B (upper), both of which comprise a basal till that is overlain by lacustrine sediments. The till at the base of Sequence A was formed by the Aare Glacier. The overlying lacustrine deposits of an ice-contact lake were mainly supplied by the Aare Glacier. The basal till in Sequence B was also formed by the Aare Glacier. For the lacustrine deposits in Sequence B, the heavy mineral and garnet geochemical data indicate that the sediment was supplied by the Aare and the Saane glaciers. We use these findings for a paleogeographic reconstruction. During the time when Sequence A and the basal till in Sequence B were deposited, the Aare Glacier dominated the area. This strongly contrasts with the situation during the LGM, when the Aare Glacier was deflected by the Valais Glacier towards the northeast. The Valais Glacier was probably less extensive during MIS 8, but it was potentially presentin the area, and it could have been essential for damming a lake in which the material supplied by the Aare and the Saane glaciers accumulated. In conclusion, combining provenance with sedimentological data, we could document how sediment was supplied to the investigated overdeepened basin during the MIS 8 glacial period and how glaciers were arranged in a way that was markedly different from the LGM

    Intraluminal Pressure Modulates Vascular Contractility of Perfused Mesenteric Resistance Arteries: Altered Response in Hypertension

    Intraluminal pressure may affect vascular contractility in both normotension and hypertension. To test this hypothesis, we studied mesenteric resistance arteries from normotensive humans as well as normotensive (WKY) and spontaneously hypertensive (SHR) rats (internal diameter 214 ± 27, 201 ± 6, and 172 ± 6 μm, mean ± SEM at 10 mm Hg). Vessels were mounted on glass cannulas and perfused in organ chambers filled with buffer solution at intraluminal pressures of 10 to 120 mm Hg; vasomotion was measured using a video dimension analyzer. Under baseline conditions (10 mm Hg), wall thickness was 36 ± 4 μm in humans, 32 ± 4 μm in WKY, and 47 ± 2 μm in SHR (P < .001). With increasing pressure, the diameter of human vessels increased up to 25 mm Hg and remained constant at higher pressures. In contrast, resistance arteries of normotensive and hypertensive rats exhibited an almost linear increase in diameter over the whole pressure range. In SHR, the pressure-diameter relationship was much flatter than that of WKY, indicating reduced compliance. In human arteries, the contraction to KCl was maximal at 25 mm Hg and averaged 40 ± 6%. Both above and below 25 mm Hg, the response declined to a minimum of 17 ± 2% at 120 mm Hg (P < .01). Similar results were obtained in WKY rats. In contrast, the contractile response in SHR remained maximal over the entire pressure range studied (65 ± 5%). Thus, intraluminal pressure profoundly affects vascular reactivity of resistance arteries; low pressure augments and high pressure reduces the contractile response in normotensive human and rat resistance arteries, whereas this pressure-dependent modulation of vascular reactivity is lost in the SHR.     Study protocol for the Anesthesiology Control Tower—Feedback Alerts to Supplement Treatments (ACTFAST-3) trial: A pilot randomized controlled trial in intraoperative telemedicine [version 1; referees: 2 approved]

    Background: Each year, over 300 million people undergo surgical procedures worldwide. Despite efforts to improve outcomes, postoperative morbidity and mortality are common. Many patients experience complications as a result of either medical error or failure to adhere to established clinical practice guidelines. This protocol describes a clinical trial comparing a telemedicine-based decision support system, the Anesthesiology Control Tower (ACT), with enhanced standard intraoperative care. Methods: This study is a pragmatic, comparative effectiveness trial that will randomize approximately 12,000 adult surgical patients on an operating room (OR) level to a control or to an intervention group. All OR clinicians will have access to decision support software within the OR as a part of enhanced standard intraoperative care. The ACT will monitor patients in both groups and will provide additional support to the clinicians assigned to intervention ORs. Primary outcomes include blood glucose management and temperature management. Secondary outcomes will include surrogate, clinical, and economic outcomes, such as incidence of intraoperative hypotension, postoperative respiratory compromise, acute kidney injury, delirium, and volatile anesthetic utilization. Ethics and dissemination: The ACTFAST-3 study has been approved by the Human Resource Protection Office (HRPO) at Washington University in St. Louis and is registered at clinicaltrials.gov (NCT02830126). Recruitment for this protocol began in April 2017 and will end in December 2018. Dissemination of the findings of this study will occur via presentations at academic conferences, journal publications, and educational materials

    Calcium carbonate dissolution rates in hydrothermal vent fields of the Guaymas Basin

    Analysis of bivalve shell fragments that were embedded in epoxy blocks, mounted on titanium stakes, and deployed by DSRV Alvin at 5 sites in the Southern Trough of the Guaymas Basin (27°00′N, 111°24.55′W; depth 2012 m) indicates significant variation of calcium carbonate dissolution in in situ exposures of more than 900 days. Arrays of shell fragments of six bivalve species (i.e., Bathymodiolus thermophilus, Calyptogena magnifica, Calyptogena sp., Corbicula fluminea, Crassostrea virginica and Mytilus edulis) were positioned −17 cm, −7 cm and −2.5 cm below the sediment-water interface and 2.5 cm, 7 cm and 17 cm above the sediment-water interface in hydrothermal vent fields of the basin. Maximum dissolution rates for both calcite (mean = 86 μm/yr) and aragonite (mean = 312 μm/yr) were found in epoxy blocks located at the deepest point sampled in the sediment column (depth = 17 cm). Minimum dissolution rates of calcite and aragonite were found 7 cm (mean = 26 μm/yr) and 2.5 cm (mean = 96 μm/yr) above the sediment-water interface, respectively. Intermediate rates of dissolution were recorded 17 cm above the sediment-water interface (mean = 40 μm/yr for calcite and 126 μm/yr for aragonite). Mean rates of aragonite dissolution ranged from 59 μm/yr (site 5; clam area) to 227 μm/yr (site 3; clam area), and those of calcite dissolution ranged from 13 μm/yr (site 3; clam area) to 94 μm/yr (site 4; bacterial mat area). Dissolution rates were consistently highest in the bacterial mat area (site 4; mean = 94 μm/yr for calcite and 223 μm/yr for aragonite). Rates of calcium carbonate dissolution reported here for hydrothermal vent fields of the Guaymas Basin compare favorably with those of Rose Garden (Galapagos Rift) and 21N (East Pacific Rise) hydrothermal vent sites. These results have important implications for assessing biological rate processes in deep-sea hydrothermal vent environments

    From glacial erosion to basin overfill: a 240 m-thick overdeepening–fill sequence in Bern, Switzerland

    We drilled a 210 m-thick succession of Quaternary sediments and extended it 30 m upsection with information that we collected from an adjacent outcrop. In the 240 m-thick succession we identified 12 different lithofacies, grouped them into five facies assemblages, and distinguished two major sedimentary sequences. A sharp contact at 103 m depth cuts off cross-beds in sequence A and separates them from the overlying horizontal beds in sequence B. Although the lowermost facies assemblage of each sequence includes a till deposited during a period of ice cover, the two tills differ from each other. In particular, the till at the base of sequence A is dominated by large clasts derived from the underlying Molasse bedrock, whereas the till at the base of sequence B has no such Molasse components. Furthermore, the till in sequence A bears evidence of glaciotectonic deformation. Both tills are overlain by thick assemblages of subaqueous, most likely glaciolacustrine and lacustrine facies elements. The cross-bedded and steeply inclined sand, gravel, and diamictic beds of sequence A are interpreted as deposits of density currents in a subaqueous ice-contact fan system within a proglacial lake. In contrast, the lacustrine sediments in sequence B are considered to record a less energetic environment where the material was most likely deposited in a prodelta setting that gradually developed into a delta plain. Towards the top, sequence B evolves into a fluvial system recorded in sequence C, when large sediment fluxes of a possibly advancing glacier resulted in a widespread cover of the region by a thick gravel unit. Feldspar luminescence dating on two samples from a sand layer at the top of sequence B provided uncorrected ages of 250.3 ± 80.2 and 251.3 ± 59.8 ka. The combination of these ages with lithostratigraphic correlations of sedimentary sequences encountered in neighboring scientific drillings suggests that sequence B was deposited between Marine Isotope Stage 8 (MIS 8; 300–243 ka) and MIS 7 (243–191 ka).     Metallicities of M Dwarf Planet Hosts from Spectral Synthesis

    We present the first spectroscopic metallicities of three M dwarfs with known or candidate planetary mass companions. We have analyzed high resolution, high signal-to-noise spectra of these stars which we obtained at McDonald Observatory. Our analysis technique is based on spectral synthesis of atomic and molecular features using recently revised cool-star model atmospheres and spectrum synthesis code. The technique has been shown to yield results consistent with the analyses of solar-type stars and allows measurements of M dwarf [M/H] values to 0.12 dex precision. From our analysis, we find [M/H] = -0.12, -0.32, and -0.33 for GJ 876, GJ 436, and GJ 581 respectively. These three M dwarf planet hosts have sub-solar metallicities, a surprising departure from the trend observed in FGK-type stars. This study is the first part of our ongoing work to determine the metallicities of the M dwarfs included in the McDonald Observatory planet search program.Comment: 13 pages, 2 figures, accepted for publication in ApJ

    Algorithmic Bias in Machine Learning Based Delirium Prediction

    Although prediction models for delirium, a commonly occurring condition during general hospitalization or post-surgery, have not gained huge popularity, their algorithmic bias evaluation is crucial due to the existing association between social determinants of health and delirium risk. In this context, using MIMIC-III and another academic hospital dataset, we present some initial experimental evidence showing how sociodemographic features such as sex and race can impact the model performance across subgroups.
