    Efficacy of a program of prevention and control for methicillin-resistant Staphylococcus aureus infections in an intensive-care unit

    Methicillin-resistant Staphylococcus aureus (MRSA) is endemic in most Brazilian Hospitals, and there are few studies which show the efficacy of control measures in such situations. This study evaluated intensive care unit (ICU) patients, in two years divided in control, intervention and post-intervention group. Intervention measures: hands-on educational programs for healthcare workers; early identification of MRSA infected or colonized patients, labeled with a bed-identification tag for contact isolation; nasal carriers, patients, and healthcare professionals treated with topical mupirocin for five days. The hospital infection rates in the control period were compared to the ones in the post-intervention period. Hospital infection rates were found by means of the NNISS methodology The incidence coefficients of MRSA hospital infection (monthly average of 1,000 pts/day) in the control, intervention and post-intervention groups were respectively: 10.2, 5.1 and 2.5/1,000 pts/day (p<0.001) and MRSA-originated bloodstream infections were 3.6, 0.9 and 1.8/1,000 central venous catheter/day (p=0.281). Nasal colonization in both intervention and post-intervention periods was of 30.9% and 22.1% among the hospitalized patients, respectively 54.4% and 46.1% of whom were already MRSA-positive when admitted to the unit. In the intervention period, most of those MRSA infected patients (76.2%) were nasal carrier. Mortality rates were, respectively 26.6%; 27.3% and 21.0% (p<0.001). Nasal carriers, both patients (93.7%) and healthcare professionals (88.2%), were successfully treated with topical mupirocin. Intervention measures for the prevention and control of MRSA infections in ICUs, have been efficient in the reduction of the bloodstream and MRSA-originated hospital infections incidence, and reduced the overall mortality rate significantly

    Epidemiologic studies of chagas' disease in the urban zone of Teresina. State of Piauí, northeastern Brazil

    The triatomine species Rhodnius nasutus and Triatoma pseudomaculata were captured on palm trees Orbignya martiana "babaçu ", in the urban zone of Teresina. This kind of palm tree is largely distributed in Piauí State. The predominant species was R. nasutus; the young in stars predominated. The infestation index of palm trees and the infection index of triatomines by flagellates were 96.0 ana 29.1%, respectively. Marsupiais, bats and a rodent were captured in palm trees. The flagellates found in both triatomines ana marsupiais were morphologically and biologically indistinguishable from Trypanosoma cruzi. Forty seven percent (481/1,025) of triatomines were found concentrated in six palm trees where marsupiais circulated. Of the total of 1,025 triatomines 230 (22%) were infected by flagellates and 53.0% (123/230) of these infected triatomines were present in the same six palm trees. No evidence of triatomine domiciliation or human transmission was observed in the houses in the vicinity of palm trees. The results suggest that marsupiais play an important role in the life-cycle of T. cruzi in this region. The natural focus of Chagas' disease, demonstrated in the present study could represent a potential epidemiological threat

    Effect of Tamoxifen and Raloxifene on the Proliferative Activity of the Breast Epithelium in Premenopausal Women

    Objectives To compare the effects of tamoxifen and raloxifene on the proliferative activity of normal breast tissue in premenopausal women as measured by Ki-67/MIB-1 expression. Study Design A total of 48 women with benign breast nodules and a recommendation for surgical removal of the lesion took part in this study. They were randomized to use tamoxifen or raloxifene for 22 days, after which they were submitted to surgery. During the surgical procedure, a 1-cm fragment of normal breast tissue was removed to study Ki-67 expression. Results The mean percentage ratios between immunolabeled and non-labeled cells were 2.02 ± 1.09 and 3.13 ± 3.23 for the tamoxifen and raloxifene groups, respectively. There was no statistically significant difference between the tamoxifen ( n = 16) and raloxifene ( n = 14) groups in relation to the immunohistochemical analysis of Ki-67 ( P = 0.205). Conclusion The results of this study showed no difference between tamoxifen and raloxifene with respect to the potential of these drugs to reduce the proliferative activity of the normal breast epithelium in premenopausal women

    Fabrication of microenvironments with different geometrical features for cell growth studies

    Studies of cell development in artificial microenvironments can contribute to understanding a series of physiological mechanisms that might be influenced by geometrical features of the microenvironment itself. In this work we applied two-photon polymerization to fabricate three-dimensional microenvironments composed of a matrix arrangement of microstructures (circular and square cross-sections pillars) with distinct spacing. Such microenvironments were used to evaluate the growth of Michigan Cancer Foundation-7 (MCF-7) cells that are commonly used as a model system to investigate fundamental aspects of the tumor biology. Our results reveal that the cell density decreases as the distance between structures in the environment is increased.
