    Nel 2017, la Consulta aveva confinato i laureati in scienze motorie a meri operatori di supporto all’attività sportiva, dichiarando la loro esclusione dalle attività professionali volte alla rieducazione e alle attività preventive adattate, che potevano svolgere solo i fisioterapisti. Sono diverse le norme che hanno invece affermato la centralità del laureato in scienze motorie nella pianificazione della prevenzione, fino all’istituzione di nuove figure professionali, che sono strategiche nel contesto della riapertura dei centri sportivi a seguito delle limitazioni dovute al Covid-19, causa dello stile di vita sedentario che ha indotto le persone ad abbandonare le attività rivolte al loro benessere. Lo studio si pone come un necessario spunto interdisciplinare volto a far ripartire la società in sicurezza senza rinunciare ad uno stile di vita sano, obiettivo che può essere raggiunto lasciando il giusto spazio ai professionisti della attività motorie, anche grazie al loro inquadramento come professionisti dal d.lgs 36/2021

    Ranking among the most prevalent of shoulder disorders, lesions involving shoulder rotator cuff strike million of patients in the United States at different points in their lives. Despite the fact that rotator cuff disease is a very common cause of pain and disability of the upper arm, a high proportion of patients is asymptomatic and unaware of performing daily living activities despite of a shoulder lesion. Given these drawbacks, surgeons are trying exploring whether conservative treatment is a viable option for the management of these injuries. This study presents a rationale to consider nonoperative treatment an effective option when dealing with patients with rotator cuff tears. © 2011 by Lippincott Williams & Wilkins

    Osteolytic lesion of greater tuberosity in calcific tendinitis of the shoulder

    Hypothesis: This study investigated tuberosity osteolysis, an uncommon and frequently misdiagnosed form of calcific tendinitis of the shoulder, and evaluated its effects on clinical and surgical outcomes. Materials and methods: A total of 126 patients with calcific tendinitis studied with radiographs, ultrasound, and magnetic resonance images (MRIs) were divided into groups positive and negative for tuberosity osteolysis and treated by arthroscopy. Follow-up evaluation was at 2 years, using the Constant score. Results: Tuberosity osteolysis was associated with significantly lower Constant scores, both before and after surgical treatment. Clinical and imaging findings exhibited a significant correlation. A 100% correlation was found between arthroscopy and MRI findings of tuberosity osteolysis compared with 90% with radiographs. Conclusion: Imaging and functional data indicate that calcific tendinitis of the rotator cuff with tuberosity osteolysis is a distinctive form of calcific tendinitis that should be considered in clinical and surgical practice.     Prevalence of shoulder discomfort in paraplegic subjects

    Introduction: The annual incidence of acute spinal cord injury, as reported in the National Spinal Cord Injury Statistical Center, was estimated to be approximately 40 cases per million, or approximately 11.000 new cases in the United States each year (1). In subjects with spinal cord injury, overstress of the upper extremities occurs not only during daily life activities, but also utilizing wheelchair. The aim of this study is to determine the prevalence of shoulder discomfort in subjects affected by paraplegia living in our country area and evaluate the associated risk factors negatively affecting the shoulder function. Material and Method: A computerized database search for paraplegic patients was performed at the Department of Orthopedic Surgery, Trauma Surgery and Rehabilitation Medicine. Among the 54 selected paraplegic patients, 47 accepted to participate at the study. Once the patients were contacted, the medical records were reviewed to obtain information regarding shoulder, elbow and spine region pain and discomfort. Results: The 60% of all the patients who answered to the questionnaire complain shoulder pain, 90% bilaterally, and the 30% referred elbow pain, 20% bilaterally. 77% complains back pain, involving particularly lumbar region. Only 2% of this population undergoes a shoulder surgery treatment for cuff tear and shoulder disability. Discussion and Conclusion: The present study is the first in our country that gives an overview of prevalence of upper limb discomfort in paraplegic patients.We have shown the interplay between shoulder elbow and spine with pain, age of patients and type of wheel chair utilized. The previous factors can be corrected and upper limb discomfort relieved. © Mattioli 1885

    Predisposing factors for recurrent shoulder dislocation after arthroscopic treatment

    Background: Arthroscopic repair of anterior dislocation of the shoulder can fail. We hypothesized that patients who are at higher risk for redislocation following repair could be recognized preoperatively on the basis of their clinical history. The purpose of the present study was to identify the risk factors for recurrence in a community-based population of patients with traumatic unidirectional instability that was treated with a single arthroscopic technique. Methods: From January 2000 to December 2003, 625 patients with anterior unidirectional instability were managed with an arthroscopic Bankart technique, and 385 met the criteria for inclusion in the study. Demographic data were collected, and clinical follow-up was performed at three, six, twelve, twenty-four, and thirty-six months. Results: At thirty-six months, thirty-one patients (8.1%) had experienced a redislocation; the rate was 13.3% among patients who were twenty-two years of age and younger and 6.3% among older patients. Age at the time of the first dislocation, male sex, and the time from the first dislocation until surgery were significant risk factors for recurrence (p < 0.05 for all). Conclusions: Patients who are more likely to have a redislocation following arthroscopic repair of an anterior shoulder dislocation can be identified preoperatively on the basis of sex, age, and the time from the first dislocation to surgery.     Esporte e atividade física na idade avançada: incidência nas alterações do equilíbrio

    Introdução: O presente trabalho foi conduzido com o objetivo de pesquisar os efeitos que o esporte e a atividade física podem exercer nas alterações do equilíbrio estático nas pessoas idosas. Materiais e Métodos: Para o estudo foram recrutadas 30 pessoas saudáveis, de ambos os sexos e de idade entre 65 e 75 anos. Os sujeitos que resultaram capazes preencheram o questionário Minnesota sobre a atividade física praticada e fizeram 6 testes diferentes na plataforma estabilométrica, dependendo das indicações do Smart Balance Master da Neurocom International. Os dados foram submetidos ao teste, de tipo não paramétrico, de Wilcoxon e Mann-Whitney. P < 0,05 foi considerado significativo. Discussão: A análise dos dados não evidenciou diferenças significativas na comparação entre os grupos (0,1 < p< 0,9). Não obstante isso é importante evidenciar que, em quase todas as variáveis examinadas, os valores mostram claras diferenças em favor da população esportiva (nos sujeitos esportivos: redução média SUP= 0,05 ± 0,07cm2 ; redução média X= 0,07 ± 0,06cm; redução média Y= 0,06 ± 0,09cm). Isso nos leva a afirmar que a prática de atividade esportiva, na idade avançada, tem um papel não indiferente nas capacidades de anutenção do equilíbrio estático, se comparada com a atividade físic

    Shoulder arthroplasty in alkaptonuric arthropathy: A clinical case report and literature review

    Alkaptonuria is a rare hereditary metabolic disease of autosomal recessive inheritance, resulting from deficiency of the enzyme homogentisic acid oxidase. The term ''alkaptonuria'' was first used in 1859 by Boedeker to describe a patient's urinary reducing compound, and in 1866, Virchow coined the term ''ochronosis'' due to typical yellow pigmentation. Deposition of this pigment in articular cartilage leads to ochronotic arthropathy, the most incapacitating complication of alkaptonuria. We report a rare case of shoulder ochronotic arthritis, treated with total shoulder arthroplasty, achieving a successful long-term clinical and radiological outcomes. \ua9 2012 Springer-Verlag
