    Secreted miR-210-3p as non-invasive biomarker in clear cell renal cell carcinoma

    The most common subtype of renal cell carcinoma (RCC) is clear cell RCC (ccRCC). It accounts for 70-80% of all renal malignancies representing the third most common urological cancer after prostate and bladder cancer. The identification of non-invasive biomarkers for the diagnosis and responsiveness to therapy of ccRCC may represent a relevant step-forward in ccRCC management. The aim of this study is to evaluate whether specific miRNAs deregulated in ccRCC tissues present altered levels also in urine specimens. To this end we first assessed that miR-21-5p, miR-210-3p and miR-221-3p resulted upregulated in ccRCC fresh frozen tissues compared to matched normal counterparts. Next, we evidenced that miR-210-3p resulted significantly upregulated in 38 urine specimens collected from two independent cohorts of ccRCC patients at the time of surgery compared to healthy donors samples. Of note, miR- 210-3p levels resulted significantly reduced in follow-up samples. These results point to miR-210-3p as a potential non-invasive biomarker useful not only for diagnosis but also for the assessment of complete resection or response to treatment in ccRCC management

    Multiparametric MRI of the bladder: inter-observer agreement and accuracy with the Vesical Imaging-Reporting and Data System (VI-RADS) at a single reference center

    Objectives: To evaluate accuracy and inter-observer variability using Vesical Imaging-Reporting and Data System (VI-RADS) for discrimination between non-muscle invasive bladder cancer (NMIBC) and muscle-invasive bladder cancer (MIBC). Methods: Between September 2017 and July 2018, 78 patients referred for suspected bladder cancer underwent multiparametric MRI of the bladder (mpMRI) prior to transurethral resection of bladder tumor (TURBT). All mpMRI were reviewed by two radiologists, who scored each lesion according to VI-RADS. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were calculated for each VI-RADS cutoff. Receiver operating characteristics curves were used to evaluate the performance of mpMRI. The Ƙ statistics was used to estimate inter-reader agreement. Results: Seventy-five patients were included in the final analysis, 53 with NMIBC and 22 with MIBC. Sensitivity and specificity were 91% and 89% for reader 1 and 82% and 85% for reader 2 respectively when the cutoff VI-RADS > 2 was used to define MIBC. At the same cutoff, PPV and NPV were 77% and 96% for reader 1 and 69% and 92% for reader 2. When the cutoff VI-RADS > 3 was used, sensitivity and specificity were 82% and 94% for reader 1 and 77% and 89% for reader 2. Corresponding PPV and NPV were 86% and 93% for reader 1 and 74% and 91% for reader 2. Area under curve was 0.926 and 0.873 for reader 1 and 2 respectively. Inter-reader agreement was good for the overall score (Ƙ = 0.731). Conclusions: VI-RADS is accurate in differentiating MIBC from NMIBC. Inter-reader agreement is overall good. Key Points: • Traditionally, the local staging of bladder cancer relies on transurethral resection of bladder tumor. • However, transurethral resection of bladder tumor carries a significant risk of understaging a cancer; therefore, more accurate, faster, and non-invasive staging techniques are needed to improve outcomes. • Multiparametric MRI has proved to be the best imaging modality for local staging; therefore, its use in suitable patients has the potential to expedite radical treatment when necessary and non-invasive diagnosis in patients with poor fitness

    Accuracy of magnetic resonance imaging to identify pseudocapsule invasion in renal tumors

    Purpose: To evaluate accuracy of MRI in detecting renal tumor pseudocapsule (PC) invasion and to propose a classification based on imaging of PC status in patients with renal cell carcinoma. Methods: From January 2017 to June 2018, 58 consecutive patients with localized renal cell carcinoma were prospectively enrolled. MRI was performed preoperatively and PC was classified, according to its features, as follows: MRI-Cap 0 (absence of PC), MRI-Cap 1 (presence of a clearly identifiable PC), MRI-Cap 2 (focally interrupted PC), and MRI-Cap 3 (clearly interrupted and infiltrated PC). A 3D image reconstruction showing MRI-Cap score was provided to both surgeon and pathologist to obtain complete preoperative evaluation and to compare imaging and pathology reports. All patients underwent laparoscopic partial nephrectomy. In surgical specimens, PC was classified according to the renal tumor capsule invasion scoring system (i-Cap). Results: A concordance between MRI-Cap and i-Cap was found in 50/58 (86%) cases. ρ coefficient for each MRI-cap and iCap categories was: MRI-Cap 0: 0.89 (p < 0.0001), MRI-Cap1: 0.75 (p < 0.0001), MRI-Cap 2: 0.76 (p < 0.0001), and MRI-Cap3: 0.87 (p < 0.0001). Sensitivity, specificity, positive predictive value, negative predictive value, and AUC were: MRI-Cap 0: Se 97.87% Spec 83.3%, PPV 95.8%, NPV 90.9%, and AUC 90.9; MRI-Cap 1: Se 77% Spec 95.5%, PPV 83.3%, NPV 93.5%, and AUC 0.86; MRI-Cap 2- iCap 2: Se 88% Spec 90%, PPV 79%, NPV 95%, and AUC 0.89; MRI-Cap 3: Se 94% Spec 95%, PPV 88%, NPV 97%, and AUC 0.94. Conclusions: MRI-Cap classification is accurate in evaluating renal tumor PC features. PC features can provide an imaging-guided landmark to figure out where a minimal margin could be preferable during nephron-sparing surgery

    Modeling, Optimization, and Techno-Economic Analysis of Bipolar Membrane Electrodialysis for Direct Air Capture Processes

    Bipolar membrane electrodialysis (BPMED) could allow for the complete electrification of direct air capture (DAC) technologies. In this work, we have modeled and optimized two DAC processes based on different electrodialysis cell designs. The technical assessment has been complemented by a detailed economic analysis, showing the advantages but also the current shortcomings of this technology and pathways for advancement. A minimum energy demand of 24 MJ kgCO2–1 has been estimated for the base-case scenario, a result comparable to what has been reported for other liquid-scrubbing DAC technologies. Several solutions to further abate power consumption have been reviewed, with the most promising case providing a 29% reduction. Membrane cost and performance are currently the main limiting factors. In a scenario where cheaper membranes with better performance are assumed to be available, total costs below $250 tonCO2–1 may be feasible, making BPMED a viable fully electrified alternative to other technologies requiring natural gas

    More atypical than atypical Alzheimer's disease phenotypes: a Treviso Dementia (TREDEM) registry case report

    Background: A 57-year-old right-handed man was admitted to the Treviso Memory Clinic due to the presence of memory forgetfulness, repetition of the same questions, episodes of confusion, initial difficulties in performing complex tasks and easy distraction over the past two years, as well as recurrent and never-happened-before car accidents.Objective: We report a peculiar case of an early onset Alzheimer's disease (AD) with an unusual symptomatology, apparently not fitting in any of the categorized atypical forms of AD nor being representative of a typical amnestic AD.Methods: The patient underwent a neuropsychological, structural, and metabolic cerebral evaluation by MRI and F-18-FDG PET, together with the search for cerebral amyloid (amyloid PET), a genetic testing for dementia related genes and the dosage of CSF protein biomarkers of neurodegenerative conditions.Results: We observed a convergence of predominant frontal (dysexecutive, verbal disinhibition) and posterior (visuospatial) features of cognitive impairment. Structural MRI sequences showed subarachnoid spaces of the vault enlarged in the frontoparietal region with anterior and posterior cortical atrophy. The hippocampus appeared preserved. The 18F-FDG PET scans showed hypometabolism in the prefrontal, lateral temporal, posterior parietal, and occipital regions bilaterally. The F-18-Flutemetamol scan showed a diffused uptake of the amyloid tracer at the cerebral cortex. CSF biomarkers were compatible with Alzheimer's disease (AD).Conclusion: This case report presented with clinical phenotypic aspects atypical of AD, both frontal and posterior, never described as concomitant in the most accredited criteria for atypical AD, and appeared therefore more atypical than each of the atypical AD phenotypes already reported

    Syngas quality in fluidized bed gasification of biomass: comparison between olivine and K-feldspar as bed materials

    The relevance of selecting an appropriate bed material in fluidized bed gasification is a crucial aspect that is often underestimated. The ideal material should be economical, resistant to high temperatures and have small chemical interaction with biomass. However, often only the first of such three aspects is considered, neglecting the biomass–bed interaction effects that develop at high temperatures. In this work, olivine and K-feldspar were upscale-tested in a prototype fluidized bed gasifier (FBG) using arboreal biomass (almond shells). The produced syngas in the two different tests was characterized and compared in terms of composition (H2, CH4, CO, CO2, O2) and fate of contaminants such as volatile organic compounds (VOCs), tar and metals.. Moreover, the composition of olivine and K-feldspar before and after the biomass gasification process has been characterized. The aim of this work is to show which advantages and disadvantages there are in choosing the most suitable material and to optimize the biomass gasification process by reducing the undesirable effects, such as heavy metal production, bed agglomeration and tar production, which are harmful when syngas is used in internal combustion engines (ICE). It has been observed that metals, such as Ni, Cu, Zn, Cd, Sn, Ba and Pb, have higher concentrations in the syngas produced by using olivine as bed material rather than K-feldspar. In particular, heavy metals, such as Pb, Cu, Cd, Ni and Zn, show concentrations of 61.06 mg/Nm3, 15.29 mg/Nm3, 17.97 mg/Nm3, 37.29 mg/Nm3 and 116.39 mg/Nm3, respectively, compared to 23.26 mg/Nm3, 11.82 mg/Nm3, 2.76 mg/Nm3, 24.46 mg/Nm3 and 53.07 mg/Nm3 detected with K-feldspar. Moreover, a more hydrogen-rich syngas when using K-feldspar was produced (46% compared to 39% with olivine)

    Project. Oxidative stress (OS) is enhanced in hemodialysis (HD) patients. Lipid peroxidation and oxidative damage to glycids, proteins and nucleic acids are main consequences of OS and are associated to increased cardiovascular risk. Vitamin E and Glutathione Peroxidase (GSH-Px) represent main antioxidant systems in human cells. Selenium (Se), bound to the active sites of GSH-Px, plays a critical role in this antioxidant defense system. Statins are widely used and extensively investigated in the prevention of cardiovascular disease, notably in high-risk subjects. Several studies suggest that statins show antioxidant effects, protecting low-density lipoproteins from oxidation. Aim of our study was to compare serum Se concentration in ESRD patients on maintenance HD and in homogeneous healthy subjects and to investigate whether chronic assumption of statins may interfere with serum Se concentration in HD patients. Procedure. A total of 103 HD patients and 69 healthy subjects were enrolled; HD patients were then divided into patients who were not treated with statins (group A) and patients who assumed statins since six months at least (group B). Serum Se was determined by atomic absorption spectrometry. Results. Serum Se was significantly lower in HD patients of group A compared to healthy subjects (81.65±19.66mcg/L Vs. 96.47±15.62mcg/L, p<0.0040). However, in HD patients who assumed statins serum Se was significantly higher than in HD patients who did not. (111.83±18.82mcg/L Vs. 81.65±19.66mcg/L, p<0.0001). Conclusions. our results suggest that in HD patients chronic assumption of statins is related to a higher availability of active antioxidant agents and to reduced oxidative stress

    Fog Detection Based on Meteosat Second Generation-Spinning Enhanced Visible and InfraRed Imager High Resolution Visible Channel

    In this study, the Meteosat Second Generation (MSG)-Spinning Enhanced Visible and Infrared Imager (SEVIRI) High Resolution Visible channel (HRV) is used in synergy with the narrow band MSG-SEVIRI channels for daytime fog detection. A new algorithm, named MSG-SEVIRI SatFog, has been designed and implemented. MSG-SEVIRI SatFog provides the indication of the presence of fog in near real time and at the high spatial resolution of the HRV channel. The HRV resolution is useful for detecting small scale daytime fog that would be missed in the MSG-SEVIRI low spatial resolution channels. By combining textural, physical and tonal tests, a distinction between fog and low stratus is performed for pixels identified as low/middle clouds or clear by the Classification-MAsk Coupling of Statistical and Physical Methods (C-MACSP) cloud detection algorithm. Suitable thresholds have been determined using a specific dataset covering different geographical areas, seasons and time of the day. MSG-SEVIRI SatFog is evaluated against METeorological Aerodrome Reports (METAR) data observations. Evaluation results in an accuracy of 69.9%, a probability of detection of 68.7%, a false alarm ratio of 31.3% and a probability of false detection of 30.0%

    Feasibility of Roux-en-Y Gastric Bypass with the novel robotic platform HUGO™ RAS

    IntroductionRobotic assisted surgery is a rapidly developing field of minimally invasive bariatric surgery in the last 20 years. Its wide diffusion has led to the development and standardization of robotic assisted approaches for bariatric operations. In this study, we present the first four Roux-en-Y Gastric Bypass (RYGB) operations performed with the new Hugo™ RAS system (Medtronic, Minneapolis, MN, USA).MethodsIn January and February 2023, 4 consecutive patients scheduled for minimal invasive Roux-en-Y-Bypass were selected and underwent the procedure robotic-assisted with the new platform. No exclusion criteria were applied.ResultsFour patients, two females and two males, underwent RYGB with a median BMI of 40 Kg/m2 (range: 36–46) and diabetes mellitus in two cases. The median docking time was 8 min (range: 7–8.5) and the median console time was 127.5 min (range: 95–150). A description of the operating theatre, robotic arms and docking setup is provided. Procedures were performed without intraoperative complications and no conversion to laparoscopy or open surgery was noted. No additional ports were needed to be placed. System's function and docking were uneventful. No early post-operative complications were observed.ConclusionsBased on our initial experience, RYGB with the Hugo™ RAS system is feasible. This study provides the configurations necessary to perform RYGB with the Hugo™ RAS system as well as general information and insights from our preliminary experience
