    The experience of “medicine companions” to support adherence to antiretroviral therapy: quantitative and qualitative data from a trial population in Uganda

    Good adherence is critical for antiretroviral therapy (ART) in sub-Saharan Africa. We report on the characteristics of medicine companions (MCs) chosen by Ugandan patients enrolling on ART, and on how MCs were chosen, and what roles they played. Baseline data on MCs of 1453 participants in a randomized controlled trial comparing facility and home-based delivery of ART in Jinja, Uganda were analyzed. Textual data on experience with MCs were collected through in-depth interviews among a subsample of 40 trial participants equally divided by sex and trial arm. Significantly more women (71%) than men (29%) were recruited. The majority (75%) of women participants were either widowed (51%) or separated or divorced (24%), whereas most of the men (66%) were married. Women were most likely to choose a child as their MC while men were most likely to choose their spouse; 41% of women chose an MC under 21 compared with only 14% of men. Only 31% of married women chose their husband, compared with 66% of married men who chose their wife. Qualitative interviews suggested MCs proved useful for reminding and other supportive tasks in the first three months but were generally less essential by six months and beyond. Convenience, reliability, and trust were key considerations in choosing an MC. Children provided the only alternative for many unmarried women, but even some married women felt children made more reliable MCs than husbands. Participants who had disclosed their serostatus usually received drug-taking reminders from multiple household members. One participant in the qualitative sample with poor family relations delayed starting treatment due to unwillingness to identify an MC. MCs were generally welcome and useful in supporting early adherence. However, disclosure to an MC should not be a condition of obtaining treatment

    An efficient approach based on trust and reputation for secured selection of grid resources

    Security is a principal concern in offering an infrastructure for the formation of general-purpose computational grids. A number of grid implementations have been devised to deal with the security concerns by authenticating the users, hosts and their interactions in an appropriate fashion. Resource management systems that are sophisticated and secured are inevitable for the efficient and beneficial deployment of grid computing services. The chief factors that can be problematic in the secured selection of grid resources are the wide range of selection and the high degree of strangeness. Moreover, the lack of a higher degree of confidence relationship is likely to prevent efficient resource allocation and utilisation. In this paper, we present an efficient approach for the secured selection of grid resources, so as to achieve secure execution of the jobs. This approach utilises trust and reputation for securely selecting the grid resources. To start with, the self-protection capability and reputation weightage of all the entities are computed, and based on those values, the trust factor (TF) of all the entities are determined. The reputation weightage of an entity is the measure of both the user’s feedback and other entities’ feedback. Those entities with higher TF values are selected for the secured execution of jobs. To make the proposed approach more comprehensive, a novel method is employed for evaluating the user’s feedback on the basis of the existing feedbacks available regarding the entities. This approach is proved to be scalable for an increased number of user jobs and grid entities. The experimentation portrays that this approach offers desirable efficiency in the secured selection of grid resources

    Fibulin-1 Is Increased in Asthma – A Novel Mediator of Airway Remodeling?

    Background: The extracellular matrix is a dynamic and complex network of macromolecules responsible for maintaining and influencing cellular functions of the airway. The role of fibronectin, an extracellular matrix protein, is well documented in asthma. However, the expression and function of fibulin-1, a secreted glycoprotein which interacts with fibronectin, has not been reported. Fibulin-1 is widely expressed in basement membranes in many organs including the lung. There are four isoforms in humans (A-D) of which fibulin-1C and 1D predominate. The objective of this study was to study the expression of fibulin-1 in volunteers with and without asthma, and to examine its function in vitro. Methodology/Principal Findings: We used immunohistochemistry and dot-blots to examine fibulin-1 levels in bronchial biopsies, bronchoalveolar lavage fluid and serum. Real-time PCR for fibulin-1C and 1D, and ELISA and western blotting for fibulin-1 were used to study the levels in airway smooth muscle cells. The function of fibulin-1C was determined by assessing its role, using an antisense oligonucleotide, in cell proliferation, migration and wound healing. A murine model of airway hyperresponsiveness (AHR) was used to explore the biological significance of fibulin-1. Levels of fibulin-1 were significantly increased in the serum and bronchoalveolar lavage fluid of 21 asthmatics compared with 11 healthy volunteers. In addition fibulin-1 was increased in asthma derived airway smooth muscle cells and fibulin-1C contributed to the enhanced proliferation and wound repair in these cells. These features were reversed when fibulin-1C was suppressed using an antisense oligomer. In a mouse model of AHR, treatment with an AO inhibited the development of AHR to methacholine. Conclusions: Our data collectively suggest fibulin-1C may be worthy of further investigation as a target for airway remodeling in asthma

    Methodology: Systematic Review and Meta-analysis of the AAC Literature for People with Autism Spectrum Disorder or Intellectual Disabilities who have Complex Communication Needs through 2020

    A comprehensive meta-analysis examining the impacts of augmentative and alternative communication for individuals with complex communication needs was conducted, examining the relationship between social-communicative and educational outcomes and use of augmentative and alternative communication devices and across potential moderating variables. This document reports the methodology of the project as a whole, describing overarching procedures. Given the comprehensiveness of the review and meta-analysis, results of this review are reported in digestible groupings of types of research designs, types of research questions, and moderator groupings. Each of the resulting papers cite this primary document, as do additional reviews derived from the assembled data set. The documents herewith report the overarching methodology of the project, including the following. The document searches occurred in 2018 and 2020, resulting in 7,327 documents reviewed for title/abstract indication of meeting inclusion criteria. Full text document review was conducted for 1,758 documents for the next inclusion/exclusion gate. Documents were divided into group design (n = 132) and single-case experimental design (SCED) documents (n = 547) and reviewed for basic design criteria, resulting in 59 group design documents and 257 SCED documents. Lead project authors conducted screening remaining documents for false positives, resulting in 20 group and 176 SCED documents remaining for further analysis. Data extraction and potential moderator variable coding procedures are described in detail, with relevant coding manuals and other materials attached.     Ultrasonic exfoliation of hydrophobic and hydrophilic metal-organic frameworks to form nanosheets

    The modular structure of metal-organic nanosheets (MONs) provides a convenient route to creating two-dimensional materials with readily tunable surface properties. Here we report the liquid exfoliation of two closely related layered metal-organic frameworks functionalised with either methoxy-propyl (1) or pentyl (2) pendent groups intended to bestow either hydrophilic or hydrophobic character to the resulting nanosheets. Exfoliation of the two materials in a range of different solvents highlighted significant differences in their dispersion properties and molecular and nanoscopic structure. Exchange or loss of solvent was found to occur at the labile axial position of the paddle-wheel based MONs and DFT calculations indicated that intramolecular coordination by the oxygen of the methoxy-propyl pendant groups may take place. The nanoscopic dimensions of the MONs were further tuned by varying the exfoliation conditions and through "cascade centrifugation". Aqueous suspensions of the nanosheets were used as sensors to detect aromatic heterocycles and with clear differences in binding behaviour observed and quantified

    Photo-production of Nucleon Resonances and Nucleon Spin Structure Function in the Resonance Region

    The photo-production of nucleon resonances is calculated based on a chiral constituent quark model including both relativistic corrections H{rel} and two-body exchange currents, and it is shown that these effects play an important role. We also calculate the first moment of the nucleon spin structure function g1 (x,Q^2) in the resonance region, and obtain a sign-changing point around Q^2 ~ 0.27 {GeV}^2 for the proton.Comment: 23 pages, 5 figure
