    Predictors of the decision to retire among nurses in Spain

    Background: Nurses typically retire in their late 50s and since nursing shortages exist in most countries understanding nursing staff decisions to retire might open up possibilities of encouraging and supporting them to remain in the workforce longer.Objective: This study examines potential predictors of retirement intentions amongst nurses working in Spain.Population: All registered nurses in Spain with 50 years old or more.Methods: Survey. Data were collected with the collaboration of the regional nursing associations in Spain using anonymous online questionnaires employed to nursing staff (n=497) for those  who are 50 years or older. Results: Nurses indicated their interest in retiring, their planning for retirement, and their expectations for retiring. Results show that retirement intentions were higher in nursing staff that were older, experienced higher levels of burnout, indicated poorer levels of self-reported health, and reported greater job demands and more negative work attitudes (less affective commitment, job involvement, work engagement). The majority of these were “push†factors which are related to dissatisfaction in the workplace.Conclusion: Organizations can and should create age-friendly workplaces enabling them to cope with the nursing shortage and workplaces can be changed to better accommodate the needs  and expectations of older employees

    Elevated Familial Cardiovascular Burden Among Adolescents With Familial Bipolar Disorder

    Background: Bipolar disorder (BD) is one of the most heritable medical conditions, and certain phenotypic characteristics are especially familial in BD. BD is also strongly associated with elevated and premature cardiovascular disease (CVD) morbidity and mortality. Thus, far, little is known regarding the familiality of cardiovascular risk in BD. We therefore examined the extent of CVD-related conditions among relatives of: adolescents with BD with a family history of BD (familial BD), adolescents with BD without a family history of BD (non-familial BD) and healthy controls (HC).Materials and Methods: The sample included 372 adolescents; 75 with familial BD, 96 with non-familial BD, and 201 HC. Parents of the adolescents completed the CARDIA Family Medical History interview regarding the adolescents' first- and second- degree adult relatives. We computed a “cardiovascular risk score” (CRS) for each relative, based on the sum of the presence of diabetes, hypertension, obesity, dyslipidemia, stroke, angina, and myocardial infarction (range 0–7). Primary analyses examined for group differences in mean overall CRS scores among first and second- degree relatives combined, controlling for age, sex, and race. Secondary analyses examined first- and second-degree relatives separately, controlling for age, sex, and race.Results: There were significant between-group differences in CRS in first- and second- degree relatives combined, following the hypothesized ordering: CRS was highest among adolescents with familial BD (1.14 ± 0.78), intermediate among adolescents with non-familial BD (0.92 ± 0.79) and lowest in HC (0.76 ± 0.79; F = 6.23, df = 2, p = 0.002, ηp2 = 0.03). There was a significant pairwise difference between adolescents with familial BD and HC (p = 0.002, Cohen's d = 0.49). A similar pattern of between-group differences was identified when first-degree and second-degree relatives were examined separately.Limitations: familial cardiovascular burden was determined based on parent interview, not evaluated directly.Conclusions: Adolescents with BD with a family history of BD have elevated rates of CVD-related conditions among their relatives. This may be related to genetic overlap between BD and CVD-related conditions, shared environmental factors that contribute to both BD and CVD-related conditions, or a combination of these factors. More research is warranted to better understand the interaction between familial risk for BD and CVD, and to address this risk using family-wide preventive approaches

    Firearm injury epidemiology in children and youth in Ontario, Canada : a population-based study

    BACKGROUND AND OBJECTIVE: Despite firearms contributing to significant morbidity and mortality globally, firearm injury epidemiology is seldom described outside of the USA. We examined firearm injuries among youth in Canada, including weapon type, and intent. DESIGN: Population-based, pooled cross-sectional study using linked health administrative and demographic databases. SETTING: Ontario, Canada. PARTICIPANTS: All children and youth from birth to 24 years, residing in Ontario from 1 April 2003 to 31 March 2018. EXPOSURE: Firearm injury intent and weapon type using the International Classification of Disease-10 CM codes with Canadian enhancements. Secondary exposures were sociodemographics including age, sex, rurality and income. MAIN OUTCOMES: Any hospital or death record of a firearm injury with counts and rates of firearm injuries described overall and stratified by weapon type and injury intent. Multivariable Poisson regression stratified by injury intent was used to calculate rate ratios of firearm injuries by weapon type. RESULTS: Of 5486 children and youth with a firearm injury (annual rate: 8.8/100 000 population), 90.7% survived. Most injuries occurred in males (90.1%, 15.5/100 000 population). 62.3% (3416) of injuries were unintentional (5.5/100 000 population) of which 1.9% were deaths, whereas 26.5% (1452) were assault related (2.3/100 00 population) of which 18.7% were deaths. Self-injury accounted for 3.7% (204) of cases of which 72.0% were deaths. Across all intents, adjusted regression models showed males were at an increased risk of injury. Non-powdered firearms accounted for half (48.6%, 3.9/100 000 population) of all injuries. Compared with handguns, non-powdered firearms had a higher risk of causing unintentional injuries (adjusted rate ratio (aRR) 14.75, 95% CI 12.01 to 18.12) but not assault (aRR 0.84, 95% CI 0.70 to 1.00). CONCLUSIONS: Firearm injuries are a preventable public health problem among youth in Ontario, Canada. Unintentional injuries and those caused by non-powdered firearms were most common and assault and self-injury contributed to substantial firearm-related deaths and should be a focus of prevention efforts

    Work intensity: potential antecedents and consequences

    Purpose - The purpose of this exploratory research is to examine the relationship of a measure of work intensity with potential antecedents and consequences. Design/methodology/approach - A questionnaire was developed and pre-tested. It included a new 15-item measure of work intensity. Data were collected from 106 respondents enrolled in three university business courses using anonymously completed questionnaires. Regression and factor analyses were used in developing the measure and testing the relationships. Findings - The 15-item measure of work intensity was found to have high internal consistency and reliability. Work intensity was significantly related to respondents' organizational level and work status. In addition, respondents indicating higher levels of work intensity also reported working more hours, a higher workload, and greater job stress. Work intensity was unrelated to organizational values supporting work-personal life imbalance, three workaholism components, or to indicators of work engagement. Factor analysis of the work intensity measure produced three factors: emotional demands, job demands, and time demands, the first two were fairly consistently related to other study variables, whereas time demands was not. Research limitations/implications - The sample was relatively small and the data were collected using self-reports. The design was cross-sectional, thus limiting causal inferences. Practical implications - Managers will find the study useful in assessing the effects of work intensity and working long hours for employees, including stress levels and work engagement. Originality/value - The study developed a work intensity measure and examined its properties and correlates, something that is lacking in the literature

    COVID-19 has resulted not only in widespread illness and death, it has also upended most spheres of social life including the economic/financial one in that it has had large impacts on local economies, resulting in widespread job loss, job insecurity and loss of income. Employability, a psychological construct, refers to the belief that one can get a (another) job in the event of job loss, and financial threat refers to feelings of threat and anxiety associated with one’s finances. During the pandemic, many people experienced job loss due mainly to business closures. The present study examined the relationship between employability, job insecurity due to COVID-19, and financial threat in a Canadian (n= 487) and U.S. (n=481) sample of adults recruited on MTurk early on in the pandemic (April 2020). Participants in the Canadian sample, compared to their American counterparts, were less likely to be employed full-time, 37% vs. 67%, respectively, were more likely to be unemployed, 40% vs. 13%, respectively, and had lower self-reported socio-economic status.  A theoretical model was put forward in which employability was associated with less job insecurity and this was related to less financial threat. Results revealed that financial self-efficacy was associated with greater employability, less job insecurity and less financial threat in both samples. Further, feelings that one had enough income to “get by” since the advent of COVID-19, were positively related to employability in both samples, but in the Canadian sample only, these feelings were also related to less job insecurity and less financial threat. Implications of the study’s results are discussed within the economic climate resulting from the pandemic
