    We have investigated the spatial dependence of the longitudinal piezoelectric stress constant e of PVF2. Experiments were performed on a series of no.minally identical brass-backed PVF2 longitudinal wave transducers in water using commercial PVF2 film. Computer programs were designed to predict the performance of the transducers as a function of ezz (Z) , the thickness mode piezoelectric stress constant as a function of position Z through the thickness of the film. Our experiments indicate that the coupling coefficient is uniform across the film thickness. These computer programs were also used to model the insertion loss and bandwidth performance of the transducers. High voltage pulses were applied to PVF2 transducers to determine the region of linearity and to find the maximum nondestructive voltage that can be used. Transducers of this type were also made using PVF films fabricated in the Stanford Center for Materials Research using commercial resin, and found to pertorm as well as transducers using commercial film. Transducers of the above kind have also been used as bulk wave sources in wedge transducer assemblies for the production of surface acoustic waves on ceramic plates, and initial observations of reflections from surface cracks have been made

    Primary Proton Spectrum of Cosmic Rays measured with Single Hadrons

    The flux of cosmic-ray induced single hadrons near sea level has been measured with the large hadron calorimeter of the KASCADE experiment. The measurement corroborates former results obtained with detectors of smaller size if the enlarged veto of the 304 m^2 calorimeter surface is encounted for. The program CORSIKA/QGSJET is used to compute the cosmic-ray flux above the atmosphere. Between E_0=300 GeV and 1 PeV the primary proton spectrum can be described with a power law parametrized as dJ/dE_0=(0.15+-0.03)*E_0^{-2.78+-0.03} m^-2 s^-1 sr^-1 TeV^-1.     Prospective open-label study of add-on and monotherapy topiramate in civilians with chronic nonhallucinatory posttraumatic stress disorder

    BACKGROUND: In order to confirm therapeutic effects of topiramate on posttraumatic stress disorder (PTSD) observed in a prior study, a new prospective, open-label study was conducted to examine acute responses in chronic, nonhallucinatory PTSD. METHODS: Thirty-three consecutive newly recruited civilian adult outpatients (mean age 46 years, 85% female) with DSM-IV-diagnosed chronic PTSD, excluding those with concurrent auditory or visual hallucinations, received topiramate either as monotherapy (n = 5) or augmentation (n = 28). The primary measure was a change in the PTSD Checklist-Civilian Version (PCL-C) score from baseline to 4 weeks, with response defined as a ≥ 30% reduction of PTSD symptoms. RESULTS: For those taking the PCL-C at both baseline and week 4 (n = 30), total symptoms declined by 49% at week 4 (paired t-test, P < 0.001) with similar subscale reductions for reexperiencing, avoidance/numbing, and hyperarousal symptoms. The response rate at week 4 was 77%. Age, sex, bipolar comorbidity, age at onset of PTSD, duration of symptoms, severity of baseline PCL-C score, and monotherapy versus add-on medication administration did not predict reduction in PTSD symptoms. Median time to full response was 9 days and median dosage was 50 mg/day. CONCLUSIONS: Promising open-label findings in a new sample converge with findings of a previous study. The use of topiramate for treatment of chronic PTSD, at least in civilians, warrants controlled clinical trials

    Retinal Vascular Caliber Is Associated with Cardiovascular Biomarkers of Oxidative Stress and Inflammation: The POLA Study

    PURPOSE: Retinal vascular caliber has been linked with increased cardiovascular risk and is predictive of cardiovascular pathology, including stroke and coronary heart disease. Oxidative stress, as well as inflammatory mechanisms, plays a major role in the pathogenesis and progression of atherosclerosis, plaque rupture and vascular thrombotic propensity. The purpose of this study is to explore the relationship between retinal vascular calibers and biomarkers of oxidative stress and inflammation, in subjects free of cardiovascular pathology. PATIENTS AND METHODS: Cross-sectional analysis from a community-dwelling cohort comprising 1224 individuals aged 60 years and over, without a history of coronary or peripheral artery disease or stroke. Retinal vascular caliber was measured from fundus photographs using semi-automated standardized imaging software. Oxidative stress was evaluated using plasma superoxide dismutase 2 and glutathione peroxidase (GPx-3) activities, and inflammatory state was assessed using plasma high sensitivity C-reactive protein (hsCRP) and orosomucoid. RESULTS: In a multivariate model controlling for cardiovascular risk factors, larger retinal arteriolar caliber was independently related to higher level of GPx-3 activity (p = 0.003) whereas larger venular caliber was associated with higher levels of hsCRP (p = 0.0001) and orosomucoid (p = 0.01). CONCLUSION: In the present study, biomarkers of oxidative stress regulation and inflammation were independently associated with retinal vascular calibers. This suggests that an assessment of retinal vessels may offer early and non-invasive detection of subclinical vascular pathology

    Occlusion pressure analysis role in partitioning of pulmonary vascular resistance in CTEPH.

    Flow-directed pulmonary artery occlusion is posited to enable partitioning of vascular resistance into small and large vessels. As such it may have a role in assessment for pulmonary endarterectomy.To test if the occlusion technique distinguished small from large vessel disease we studied 59 subjects with CTEPH, idiopathic PAH, and connective tissue disease (CTD) PAH. At right heart catheterisation, occlusion pressures were recorded. With fitting of the pressure decay curve, PVR was partitioned into downstream (small vessels) and upstream (large vessels, Rup).47 patients completed the study; 14 operable CTEPH, 15 non-operable CTEPH, 13 idiopathic or CTD-PAH, 5 post-operative CTEPH. There was a significant difference in mean Rup in the proximal operable CTEPH group 87.3(95%CI 84.1;90.5); non-operable CTEPH mean 75.8(95%CI 66.76;84.73) p=0.048; and IPAH/CTD, mean 77.1(95%CI 71.86;82.33) p=0.003. ROC curves to distinguish operable from non-operable CTEPH demonstrated an AUC of 0.75, p=0.0001. A cut off of 79.3 gave sensitivity 100%(CI 73.5-100%) but specificity 57.1%(CI 28.9-82.3%). In a subgroup analysis of multiple lobar sampling there was demonstrable heterogeneity.Rup is significantly increased in operable proximal CTEPH compared with non-operable distal CTEPH and IPAH/CTD.
