Complications of Covid-19: Developments for the unified health system [Complicações da Covid-19: Desdobramentos para o sistema único de saúde]
Covid-19 is an infectious severe acute respiratory syndrome, caused by Coronavirus, which quickly reached pandemic levels. In July/2020, Brazil was the second country most affected by the disease, exceeding two million cases. Despite the increasing scientific literature on Covid-19, its containment is still compromised by the lack of understanding about its determinants and complications. This article presents a discussion on aspects related to Covid-19 complications and its effects on Unified Health System (SUS), aiming at planning new coping strategies. Additionally, it is pointed out that the overload of the health system does not result only from aspects associated with the assistance to Covid-19, but adds to the pre-existing health needs, whose care strategies were postponed and/or changed due to the actions transmission control. It is evident, then, the need to reinforce the action of Primary Health Care as the ordering of care in SUS, acting in the management of the reorganization of flows and in the improvement of the physical structures of the units. To this end, the end of measures to limit health resources is essential, since not only does the success of coping with Covid-19 depend on this, but also the future of SUS and Brazilian’s health
Burden of disease from COVID-19 and its acute and chronic complications: reflections on measurement (DALYs) and prospects for the Brazilian Unified National Health System
COVID-19 is an acute infectious respiratory distress syndrome (ARDS) caused by the novel coronavirus SARS-CoV-2. The disease is highly communicable and produces mild to severe symptoms, generating a high demand for intensive care and thousands of deaths. In March 2020, COVID-19 was declared a pandemic and has already surpassed five million cases and 300,000 deaths in the world. The natural history of the disease has still not been fully established, hindering the elaboration of effective clinical protocols and preventive measures. Nevertheless, the disease requires a systemic approach, since there is evidence of acute and chronic complications, in addition to the catastrophic effects on the population\u27s mental health. This highlights the need for a methodology that more effectively captures the effect of COVID-19, considering such aspects as severity, duration, and the potential to generate chronic complications that will increase the demands on Brazilian Unified National Health System (SUS). DALYs, or disability-adjusted life years, are thus an extremely useful indictor that adds mortality, an estimate of years of life lost (YLLs), and morbidity, an estimate of years of life lived with disability (YLDs). This article discusses the relevance and difficulties of studying the burden of COVID-19 and its complications in the Brazilian context, highlighting the natural history of the disease and estimating indicators such as YLDs, considering the high burden of disease in planning strategies to deal with the consequences of COVID-19 after the pandemic. Caracterização dos internamentos no SNS em Portugal de 2015 a 2017: a perspetiva dos tratamentos da estenose aórtica
RESUMO - Introdução: A prevalência da estenose aórtica tem crescido em todo o mundo e
representa um desafio para a saúde pública. O tratamento gold-standard é a
Substituição Cirúrgica da Válvula Aórtica (do inglês, SAVR), no entanto, a utilização da
Implantação Transcateter da Válvula Aórtica (do inglês, TAVI) tem vindo a aumentar,
especialmente em pacientes de alto risco cirúrgico.
Objetivo: Identificar e caracterizar a distribuição espacial e os fatores associados a
utilização de TAVI versus SAVR em internamentos no Serviço Nacional de Saúde (SNS)
em Portugal, de 2015 a 2017.
Metodologia: Trata-se de um estudo quantitativo, analítico, observacional, retrospetivo,
a partir da base de dados de altas de internamento do SNS. Os procedimentos, TAVI e
SAVR, foram classificados de acordo com a Classificação Internacional de Doenças
(CID). A distribuição espacial foi realizada a partir da taxa padronizada por idades para
cada distrito de residência. Os fatores, associados a realização de cada procedimento,
foram analisados pelo teste do Qui-quadrado, teste t para amostras independentes e
regressão logística ao nível de significância de 5%.
Resultados: Foram analisadas 8.398 hospitalizações sendo 88,5% para SAVR e 11,5%
para TAVI. Na distribuição espacial, ao longo dos anos analisados, observou-se um
aumento na utilização de SAVR na região Norte e uma diminuição na região de LVT, o
padrão inverso foi observado em pacientes submetidos a TAVI. A média da idade para
SAVR foi de 70 anos (±11) e 81 anos (±7) anos para TAVI (p<0,001), 56,9% do sexo
masculino para SAVR e 44,6% para TAVI (p<0,001). O ano de realização do
procedimento (p<0,001), tipo de admissão (p<0,001), localização geográfica (p<0,001),
severidade (p<0,001) e o Índice de Comorbilidade de Charlson (ICC) (p<0,001) foram
associados a realização de TAVI. O modelo ajustado revelou que uma idade mais
avançada, hospitalizações mais recentes, residir em Lisboa e Vale do Tejo (LVT) e ter
ICC≥3 aumentavam as chances de realizar TAVI.
Conclusão: Os resultados do presente estudo indicam a existência de diferenças
geográficas no tratamento da estenose aórtica, de 2015 a 2017, que podem estar
relacionadas com desigualdades no acesso aos procedimentos e, por sua vez, podem
potenciar as iniquidades em saúde em Portugal.ABSTRACT - Introduction: Aortic stenosis prevalence has been growing worldwide and constitutes a
public health challenge. The current gold-standard treatment is Surgical Aortic Valve
Replacement (SAVR) however the use of Transcatheter Aortic Valve Implantation (TAVI)
has been increasing, especially in high-risk surgical patients.
Objective: To identify the spatial distribution and factors associated with the use of TAVI
versus SAVR in inpatients with AS in Portugal, from 2015 to 2017.
Methods: Quantitative, analytical, observational, retrospective study using the
Portuguese National Health Service (SNS) inpatient discharge database. SAVR and
TAVI were classified according to the International Classification of Diseases (ICD). For
the spatial distribution, the age-standardised rate was mapped for the years analysed
using QGIS®. To identify the factors associated with the use of target procedures, using
Chi-square tests, independent t-tests, and logistic regression (5% significance level).
Results: 8,398 hospitalisations were included, with 88.5% SAVR and 11.5% TAVI.
Regarding the spatial distribution, across the years we observed an increased use of
SAVR in the Northern region and a decrease in the LVT region, while for TAVI we
observed the opposite. The mean (SD) age for SAVR was 70(±11) years old and 81(±7)
years old for TAVI (p<0.001), among SAVR 56.9% were male, while 44.6% among
TAVI(p<0.001). Year (p<0.001), type of admission (p<0.001), geographic location
(p<0.001), severity (p<0.001) and CCI (p<0.001) were associated with the TAVI.
Regarding the adjusted model, older age, more recent hospitalisations, living in LVT,
and having CCI≥3 increased the chances of being treated with TAVI.
Conclusions: The results showed geographical differences in aortic stenosis treatment,
from 2015 to 2017. Those differences might be related to unequal access to treatments
which might exacerbate Portuguese inequities in health
Consumo de medicamentos por trabalhadores de hospital
Medicamentos são um componente essencial da terapêutica, no entanto, informações sobre seu consumo em população trabalhadora são escassas. Para estimar a prevalência, caracterizar o perfil daqueles consumidos e avaliar os fatores associados ao seu uso em trabalhadores foi realizado estudo seccional de dados da linha de base da Coorte "PROSEC" (n=417). A prevalência de consumo de pelo menos um medicamento foi de 72,4%, destacando-se fármacos com ação no sistema nervoso (25,4%), especialmente analgésicos (17,8%). Fatores associados ao uso de qualquer número de medicamentos (p<0,05) foram sexo, número de condições diagnosticadas por médico e problema de saúde nos quinze dias anteriores à entrevista. A renda esteve significativamente associada ao consumo de um medicamento ao passo que autoavaliação de saúde apresentou associação independente com o consumo de dois ou mais produtos. A alta prevalência de uso de medicamentos nesta população, particularmente de analgésicos, requer atenção, pois muitos usuários costumam desconhecer ou relegar o fato de que tais fármacos não são isentos de risco. Mulheres e indivíduos em pior estado de saúde constituem os principais candidatos a programas de intervenção visando o uso adequado de produtos farmacêuticos
