    Penelitian ini bertujuan untuk mengetahui peran ekspresi HSP70 dan Caspase-3 terhadap keberhasilan operabilitas kanker serviks stadium IIB setelah pemberian kemoterapi neoajuvant Paclitaxel-Carboplatin 3 seri. Metode: ekspresi HSP70 dan caspase-3 diperiksa dengan pengecatan imunohistokimia pada 40 potongan biopsi jaringan pasien kanker serviks IIB yang memenuhi kriteria inklusi, yang melakukan pengobatan di RSUD dr. Soetomo, Departemen Obstetri dan Ginekologi, Fakultas Kedokteran Universitas Airlangga, Surabaya, mulai Desember 2006 hingga Juli 2011. Kriteria inklusi meliputi penderita yang telah dilakukan evaluasi keberhasilan terapi (operabilitas) pasca-kemoterapi neoajuvant Paclitaxel-Carboplatin sebanyak 3 seri berturut-turut, dengan interval pemberian 3 minggu. Evaluasi keberhasilan terapi (operabilitas) tidak mengikuti kriteria respons terapi berdasarkan RECIST (Response Evaluation Criteria in Solid Tumours). Waktu pemeriksaan 3 4 minggu setelah pemberian kemoterapi yang terakhir. Operabilitas tumor, yaitu kondisi klinis kanker serviks di mana pada pemeriksaan dalam vagina (vaginal toucher/VT) dan pemeriksaan dalam rektum oleh tim ginekologi onkologi setelah penderita mendapatkan kemoterapi neoajuvant Paclitaxel-Carboplatin 3 seri dinyatakan bahwa pada parametrium secara klinis tidak didapatkan infiltrasi tumor. Jenis dan bentuk penelitian ini merupakan penelitian analitik observasional, studi case control pada penderita kanker serviks IIB yang mendapat kemoterapi neoajuvant Paclitaxel-Carboplatin sebanyak 3 kali dengan interval pemberian kemoterapi 3 minggu.Hubungan antara ekspresi HSP70 dan caspase-3 dengan operabilitas post-kemoterapi neoajuvant Paclitaxel-Carboplatin dianalisis dengan regresi logistik. Ekspresi HSP70 negatif dan HSP70 positif mempunyai jumlah total yang sama, yaitu 20 orang (50%), masing-masing 9 orang pada kelompok operabel dan 11 orang pada kelompok inoperabel. Tidak ada perbedaan yang bermakna hubungan antara HSP70 dengan operabilitas ( p = 1 ), dengan OR = 1,00 ( 95%CI : 0,28-3,47 ). Ekspresi caspase-3 positif sebanyak 24 orang (60,0%), masing-masing 13 orang pada kelompok operabel dan 11 orang pada kelompok inoperabel. Pada ekspresi Caspase-3 negatif, sebanyak 16 orang (40,0%), masing-masing 5 orang pada kelompok operabel dan 11 orang pada kelompok inoperabel. Tidak ada perbedaan yang bermakna hubungan antara ekspresi caspase-3 dengan operabilitas ( p = 0,154 ), dengan OR = 0,06 ( 95%CI : 0,69-9,81). Kombinasi antara ekspresi HSP70 (+) dan ekspresi caspase-3 (-), sebanyak 8 orang (20%), masing-masing 2 orang pada kelompok operabel dan 6 orang pada kelompok inoperabel. Sedangkan pada kelompok selain kombinasi antara ekspresi HSP70 (+) dan ekspresi caspase-3 (-) sebanyak 32 orang (80%), masing-masing 16 orang pada kelompok inoperabel dan 16 orang pada kelompok operabel. Tidak ada perbedaan yang bermakna hubungan antara kombinasi ekspresi HSP70 (+) dan ekspresi caspase-3 (-) dengan operabilitas ( p = 0,217 ), dengan OR = 3,0 ( 95 % CI : 0,525-17,159).Kesimpulan: ekspresi HSP70 dan caspase-3 tidak dapat dijadikan sebagai prediktor operabilitas pada kanker serviks stadium IIB yang mendapatkan neoajuvant kemoterapi Paclitaxel-Carboplatin

    Ekspresi P53 Mutan Dan Caspase 3 Sebagai Faktor Prediksi Terhadap Operabilitas Kanker Serviks IIB Setelah Mendapat Kemoterapi Neoajuvan

    Tujuan: untuk mengetahui ekspresicaspase3 dan p53 mutan dalam memprediksi respons operabilitas (pra-operatif) pada penderita kanker serviks IIB yang mendapatkan kemoterapi neoajuvanpaclitaxel-platinum.Metode: ekspresicaspase3 dan p53 mutan diperiksa dengan pengecatan immunohistokimia pada 40 potongan biopsi jaringan pasien kanker serviks IIB yang memenuhi kriteria inklusi, yang melakukan pengobatan di RSUD dr. Soetomo, Departemen Obstetri dan Ginekologi, Fakultas Kedokteran Universitas Airlangga, Surabaya mulai bulan Desember 2006 hingga Juli 2011. Kriteria inklusi meliputi penderita kanker serviks IIB yang mendapatkan rejimen kemoterapi neoajuvanpaclitaxel-platinum(cisplatin 75 mg/m2 atau carboplatin AUC 5 paclitaxel175 mg/m2) sebanyak tiga seri berturut-turut 3 mingguan dan telah dilakukan evaluasi operabilitasnya pasca-kemoterapi neoajuvan yang dilakukan sebelum operasi, 3 minggu sampai 4 minggu setelah kemoterapi yang terakhir. Penelitian ini merupakan studi analitik dengan studi kasus kontrol.Hasil: hubungan antara ekspresi p53 mutan dancaspase3 dengan operabilitas post kemoterapi neoajuvanpaclitaxel-platinumdianalisis dengan uji regresi logistik sederhana. Dari penelitian ini didapatkan sejumlah 22 (55%) pasien kanker serviks IIB yang mendapat kemoterapi neoajuvan memberikan responsinoperabledan 18 (45%) pasien memberikan respons operabel. Ekspresi p53 mutan terbanyak menunjukkan hasil ekspresi negatif, dengan rentang skor 0-1, yaitu sebanyak 33 (82,5%) penderita. Ekspresi positif didapatkan pada 7 (17,5 %) penderita, dan tidak didapatkan perbedaan yang signifikan (p 0,48; OR: 0,55 (CI 95%:0,11-2,87)). Jumlah ekspresicaspase3 yang positif dan negatif menunjukkan jumlah yang sama besarnya baik pada luaran inoperabel maupun operabel, yaitu masing-masing 13 (72,2%) dan 11 (50,0%); dengan p=0,158, OR:2,60(CI 95%:0,69-9,81).Kesimpulan: ekspresicaspase3 dan p53 mutan tidak dapat memprediksi respons operabilitas pada pasien kanker serviks IIB yang mendapatkan kemoterapi neoajuvanpaclitaxel-platinum

    Objective: to know the result of Hematoxyllin-eosin and immunostainning D2-40 Podoplanin examination in detecting lymph vessel invasion (LVI) and to analyze correlation between lymph vessel invasion with pelvic lymph node metastasis in stage IB and IIA cervical cancer. Design of this research was cross-sectional study. The clinicopathological data of 41 patients with stage IB and IIA cervical cancer who underwent radical hysterectomy and pelvic lymphadenectomy (RHPL) at Dr. Soetomo Hospital, Surabaya, from January 2008 to June 2013 were retrospectively reviewed. Age, histology type, clinical stage, tumor size, number of removed pelvic lymph node, parametrial invasion and LVI were analyzed to define their correlation with pelvic node metastases. Immunohistochemical staining using MAb-D2-40 Podoplanin was used to detect lymph vessel invasion on the corresponding HE slides. Eligible criteria are paraffin wax embedded tissue from cervix cancer patients was diagnosed as cervix cancer, and patients medical record can be found and filled completely. Ineligible criteria are patients which is had chemotherapy or radiotherapy before surgery or patients paraffin wax embedded tissue was damaged. The correlation among variables was analyzed using Phi correlation test. Results: among of total 41 patients , lymph vessel invasion (LVI) was detected in 7 (17.07%) D2-40 Podoplanin slides, and 9 (21.95%) HE slides. Agreement rate with Kappa statistic between two method was significant, k = 0,381, p = 0,014. Lymph node metastasis was found in 6 (14.63%) samples. Positive LVI detected by D2-40 Podoplanin was significantly correlated with lymph node metastasis (? = 0.362; p = 0.02), but positive LVI detected by HE was not significantly correlated (? = 0.281; p = 0.072)

    In advanced ovarian cancer, the main goal of surgery is complete debulking with no visible residual tumor, and predicting it before surgery has become a goal that still unresolved. The ability in advanced ovarian cancer surgery varied between healthcare facilities. Recent discovery put forward the role of fatty acid as one of the main source of energy for ovarian cancer aggresiveness and the role of fatty acid binding protein (FABP) escpecially FABP4 expression as an important biomarker for predicting residual disease in ovarian cancer. There are no data about FABP4 serum in ovarian cancer and it\u27s role in predicting the extensiveness of advanced stage ovarian cancer. To evaluate correlation between FABP4 serum level with PIV Fagotti Score and Peritoneal Carcinomatosis Index (PCI) in advanced stage ovarian cancer. Analytic observational study. Among 28 subjects, most age groups (46.4%) are 40–45 years of age, 46.4% with normal body mass index, 71.4% with ovarian cancer stage IIIC, 75% with high grade serous adenocarcinoma, average ascites volume 3232.1 ± 2006.9 mL, median CA125 serum 1094 u/mL (12–19425 u/mL). The rate of optimal and complete debulking is 53.5% from 28 subjects. Strong expression of FABP4 on 42.9% subjects. MeanFABP serum 69.6 ± 51.4 ng/mL. Mean PCI score is 14.5 (3–29), mean PIV score is 6 (2–12). There is a significant correlation between FABP4 expression and FABP4 serum level (p<0.05) with moderate power (r=0.55), and a significant correlation with moderate power (r=0.421) between FABP4 serum level with PCI and PIV score. There is a significant correlation with moderate power between rising FABP4 serum and the extent of peritoneal carcinomatosis evaluated by PCI and PIV Fagotti Score in advanced stage ovarian cancer

    The Change of BDNF Expression in Traumatic Brain Injury After Kaempferia Galanga L. Administration: an Experimental Study

    BDNF has potent effects on neural synapses and its pathway affects cell survival and other biological processes. Kaempferia galanga L. has many active substances with antioxidant and anti-inflammatory effects. We analyzes BDNF in four different group of Wistar rats (Ratus novergicus) into Groups A (no trauma &amp; extract), B (with trauma &amp; no extract), C (with trauma and 600 mg/kgbw extract), and D (with trauma and 1200 mg/kgbw extract). Groups B, C, and D are further divided into those who had their BDNF measured on 24- (B24, C24, and D24) and 48-hours. BDNF expression were found to be statistically significant between Group A with both B24 (p = 0,009) and D24 (p = 0,009) on the 24-hour post-trauma decapitation analysis. On the 48 hours after trauma and extract administration, Group B48 (p = 0,002), C48 (p = &lt;0,001), and D48 (p &lt;0,001) were found to be significantly different with Group A. The administration of Kaempferia galanga L. extract can be considered as an option in increasing brain BDNF levels which are neuroprotective. Larger and specific studies are needed to determine the appropriate dose and duration

    Expression Malondialdehyde (MDA) of Brain After Injury with the Extract of Kencur (Kaempferia Galanga L): Experimental Study Wistar Rats

    Neurological damage in brain injury occurs due to secondary brain injury. Kencur extract has antioxidant potential with total phenolic and flavonoid content including luteolin apigenin and is expected to reduce MDA expression to prevent secondary injury. This study is an experimental laboratory. The treatment of all samples was carried out simultaneously using a post-test-only control group design. Based on the ANOVA test, the significance value of the Kencur extract treatment group was 0.000 (p&lt;0.05) indicating that there was a difference in MDA expression in brain-injured rats without kencur extract with brain-injured rats and given kencur extract. In the 24-hour and 48-hour time groups, a significance value of 0.488 (p&gt; 0.05) showed no significant difference in MDA expression. Then the Kencur extract treatment group with a time group of 0.117 (p&gt; 0.05) showed no significant difference in MDA expression. There was a significant difference in the expression of MDA in brain-injured rats without kencur extract with brain-injured rats and given kencur extract. There were no significant differences in the MDA expression in the 24-hour and 48-hour time groups and the Kencur extract treatment group and the 24-hour and 48-hour time groups

    The Effect of Green Tea with EGCG Active Compound in Enhancing the Expression of M2 Microglia Marker (CD206)

    Background: Stroke is a neurological deficit due to vascular disorders. Microglia are the first line of defense against brain injury. Anti-inflammatory cytokines activate M2 microglia, which upregulate CD206. EGCG is abundant in green tea, which has an anti-inflammatory effect. Objective: To know the effect of green tea with its active compound EGCG on CD206 expression. Settings and Design: True experimental trial design. Material and Methods: Rattus Novergicus were divided into six groups: a negative control group (Sham), a positive control group (P0), MCAO mice given 10 mg/kg BW EGCG (P1), 20 mg/kg BW EGCG (P2), 30 mg/kg BW EGCG (P3), and 30 mg/kg BW standardized green tea extract (P4). CD206 expression was measured using immunohistochemistry and scored according to the Allred scoring guidelines. Statistical Analysis Used: Descriptive test, Levine test, Kolmogorov–Smirnoff test, Independent sample t test, Pearson correlation test Results: We discovered that there is a significant difference in CD206 expression between the Sham and P0 groups (P < 0.05). In addition, there are significant differences in expression between the sham group and the other two groups (P1 and P2) (P < 0.05). Furthermore, when we compared the P0 group with each treatment group, we found that CD206 expression between P0–P2, P0–P3, P0–P4 are significantly different. There is a significant correlation between green tea with its active compound EGCG and CD206 expression enhancement. The correlation is positive. Conclusions: Green tea with EGCG active compound increases CD206 expression as an M2 marker in the Rattus norvegicus with MCAO model
