    Causes and Factors Associated with Neonatal Seizure and its Short-term Outcome; A Retrospective Prognostic Cohort Study

    AbstractBackground: Neonatal seizures are common, difficult to diagnose and treat, and associated with a great mortality rate and long-term risk of neurodevelopmental impairments. We aimed to determine the etiology, clinical presentation, and neurodevelopmental outcome of neonatal seizures.Methods: In this cross-sectional study, 88 neonates, aged < 28 days, who were admitted to Mofid Children’s Hospital, Tehran, Iran, from September 2011 to 2013 with the initial diagnosis of seizure were included by convenient sampling method. Data, including age, gestational age, birth weight, Apgar of the fifth minute, neonatal intensive care unit (NICU) admission, family history, type, cause, and age of seizure, EEG and paraclinic findings, anticonvulsant used for treatment, neurodevelopmental status, and the final outcome, were collected from medical records and the missed cases were completed by phone calls. The frequency and association of variables were analyzed using SPSS software.Results: Among neonates with seizures, 67% were male, 79.5% were born term, and 72.7% had normal birth weight. The most common type of seizure was multifocal clonic seizures (45.5%). The main diagnosis in neonates with seizures was hypoxic-ischemic encephalopathy (HIE) (23.9%) and hypoglycemia (10.22%). The mortality rate was 11.36% during a mean follow-up period of 21.4±6.4 months. Neurodevelopmental assessments showed that 64% were normal, 27% had global delay, and 9% had motor delay. Positive family history of epilepsy (P=0.006), low Apgar score (P=0.002) and epilepsy (P<0.001) were significantly associated with adverse neurodevelopmental outcome.Conclusion: Since HIE and hypoglycemia were the most common cause of neonatal seizures in the present study, efforts should be made to improve care during delivery and early breastfeeding.Keywords:  Newborn; Seizure; Etiology; Neurodevelopmental outcom

    Impact of water availability and fertilization management on saffron (Crocus sativus L.) biomass allocation

    Purpose: The aim of this study was to investigate the effect of fertilizer type and irrigation scheduling on biomass partitioning in saffron. Research Method: The experimental factors were saffron fertilization (no-fertilizer, cow manure and NPK chemical fertilizer) and irrigation regimes (FI=7200 and DI=3600 m3 ha-1) which were tested during 2015-16 growing season, using a factorial layout. Findings: The weights of roots and leaves had an increasing trend up to 72 and 114 days after first autumnal irrigation (DAFAI), respectively, and then were decreased. Corm production started from 51 DAFAI and its maximum was obtained at DI and chemical or organic fertilizer application. In all combined treatments corm production (maximum rate between 114 and 157 DAFAI) was prior to corm filling (started from 135 DAFAI) and both mentioned indices were higher in DI. The weight of replacement corms per clone in plants that were treated by cow manure and received 7200 m3 ha-1 water, was higher (~10%) than all other treatments. Corm growth rate increased from 50 DAFAI and the maximum value (0.32 and 0.34 g day-1 plant-1 in 56 and 78 DAFAI, respectively) obtained at combined treatment of FI × cow manure. Research limitations: Access to similar scientific references on the topic of research was a limitation. Originality/value: This study was the first research which specifically deals with the allocation of photo-assimilates in saffron and based on its findings, application of 3600 m3 ha-1 water plus cow manure is an appropriate strategy in saffron cultivation during first growing season

    The Effect of Mind Simulation Method on Reducing Stuttering and Communication Attitude

    Background and Aims Fluency disorder or stuttering is a Speech disorder in humans whose prevalence is higher at the ages before puberty. Mind simulation may be effective in reducing the severity of stuttering and some associated problems such as negative attitude towards communication. Therefore, this study aims to investigate the effect of mind simulation method on reducing stuttering and improving communication attitude in children and adolescents with stuttering aged 7-16 years.Methods This is a quasi-experimental study with a pre-test/post-test/follow-up design. Participants were 30 children and adolescents with stuttering aged 7-16 years (9 females and 21 males) referred to a psychology clinic in 2020. They completed DeNil and Brutten’s communication attitude test-revised (CAT-R) and Riley’s Stuttering Severity Instrument- 4(SSI-4) in the pre-test stage. Then, they underwent treatment by the Mind Simulation method for three months. Immediately after and one month after treatment, they completed the CAT-R and SSI-4 questionnaires again. Repeated Measure Analysis of Variance and Friedman Test were used to analyze the collected data.Results There was a significant difference between pre-test, post-test, and follow-up phases in stuttering severity (P=0.0001) and communication attitude (P=0.0001). The severity of stuttering decreased in the post-test and follow-up phases compared to the pre-test phase, while communication attitude improved in the post-test phase compared to the pre-test phaseConclusion The mind simulation method can reduce stuttering and improve communication attitude in children and adolescents with stuttering

    Effects of thiamine spraying on biochemical and morphological traits of basil plants under greenhouse conditions

    The effects of thiamin (Thi) applied as foliar spray on some morphological and physiological traits were investigated in basil (Ocimum basilicum L.). Four levels of Thi including 0 (distilled water), 250, 500 and 750 µM were tested in a randomized complete design with three replications at research greenhouse of University of Guilan, Iran. Thi exerted a significant effect on the most studied indices. The highest amounts of leaf, stem and root dry weights and number of lateral branches were obtained in plant which were treated by 500 µM Thi. Thousand grain weight increased by increase in Thi levels, as, its value in 700 µM Thi, was 26% more than control. Thi spraying especially in concentration of 750 µM improved the leaf nutrients content (N, P and Ca). Chlorophyll b content in leaves affected positively by Thi, and its highest value obtained in 750 µM treatment (0.75 mg g-1 FW). The highest and the lowest phenolic compounds were gained in 0 and 750 µM levels of Thi (47.2 vs. 40.3 mg 100g-1 FW). Similar trend was observed about total antioxidants, where their content in 750 µM was 13% more than control treatment. Generally, it seems that Thi can be considered as an appropriate growth regulator in basil medicinal plant cultivation

    Comparing the effect of organic and chemical nutritional management and intercropping with clover on the quantity and quality of different wheat (Triticum aestivum L.) cultivars

    Optimal availability of nutrients, particularly nitrogen (N), from authorized resources is one of the most important issues in organic agriculture. Intercropping forage legumes with cereal crops may be a way of providing ecological services such as providing N for companion or following crops by biological N fixation. To determine the effect of biological, chemical, and organic fertilization on the quality and quantity of ten different wheat cultivars, an experiment was conducted during two successive growing seasons. The three experimental factors (F1, F2, F3) were: mix cropping with Persian clover (Trifolium resupinatum L.) (F1: biological N fixation, as control); chemical nitrogen fertilizer (F2: 100 kg ha-1, urea), and organic cow manure (F3: 40 t ha-1). The vegetative growth parameters improved by F1 and F2 treatments. The F2 had the highest values of spikelet and seed number per spike, 1000-grain weight, and seed yield, followed by F1. The cv. ‘Kabul-13’ was the best among all tested cultivars in terms of almost all reproductive parameters. The highest seed yield was obtained by F2 ‘Kabul-13’ (8.02 t ha-1), and F1 ‘Kabul-13’ (7.26 t ha-1). By considering seed yield, ‘Chont-1’ and ‘Kabul-13’ were more suitable to be intercropped with clover. The effect of experimental factors was not significant on seed potassium and phosphorus contents, while percentage of seed protein improved by F3 (13%), and ‘Mugawim’ (13.59%). The highest protein yield was gained by F2 and F1 (820 and 650 kg ha-1) and also by ‘Kabul-13’ (850 kg ha-1). Overall, mixed cropping of wheat and clover can be suggested as a possible alternative for nutrients providing in wheat production in low input agro-ecosystems

    18F-FDG PET/MRI and 18F-FDG PET/CT for the Management of Gynecological Malignancies: A Comprehensive Review of the Literature

    Objective: Positron emission tomography with 2-deoxy-2-[fluorine-18] fluoro- D-glucose integrated with computed tomography (18F-FDG PET/CT) or magnetic resonance imaging (18F-FDG PET/MRI) has emerged as a promising tool for managing various types of cancer. This review study was conducted to investigate the role of 18F- FDG PET/CT and FDG PET/MRI in the management of gynecological malignancies. Search strategy: We searched for relevant articles in the three databases PubMed/MEDLINE, Scopus, and Web of Science. Selection criteria: All studies reporting data on the FDG PET/CT and FDG PET MRI in the management of gynecological cancer, performed anywhere in the world and published exclusively in the English language, were included in the present study. Data collection and analysis: We used the EndNote software (EndNote X8.1, Thomson Reuters) to list the studies and screen them on the basis of the inclusion criteria. Data, including first author, publication year, sample size, clinical application, imaging type, and main result, were extracted and tabulated in Excel. The sensitivity, specificity, and diagnostic accuracy of the modalities were extracted and summarized. Main results: After screening 988 records, 166 studies published between 2004 and 2022 were included, covering various methodologies. Studies were divided into the following five categories: the role of FDG PET/CT and FDG-PET/MRI in the management of: (a) endometrial cancer (n = 30); (b) ovarian cancer (n = 60); (c) cervical cancer (n = 50); (d) vulvar and vagina cancers (n = 12); and (e) gynecological cancers (n = 14). Conclusions: FDG PET/CT and FDG PET/MRI have demonstrated potential as non-invasive imaging tools for enhancing the management of gynecological malignancies. Nevertheless, certain associated challenges warrant attention

    Challenges nurses experienced in caring for patients with COVID-19 admitted to the Intensive Care Units: A Qualitative Study

    Background: Nurses working in intensive care units (ICU) have experienced numerous challenges while caring for patients with COVID-19.Objectives: The aim of this study was to explore ICU nurses’ experiences of caring for patients with COVID-19.Methods: This research presents part of a qualitative study conducted using a grounded theory approach. Participants consisted of nurses and physicians from the selected hospitals in Sabzevar, Neishabour, and Mashhad, Iran. Data collection was performed using semi-structured in-depth interviews. Participants were enrolled via purposive sampling during 2022–2023. A total of 21 participants were interviewed. The method of Corbin and Strauss (2015) was used to analyze the data. We used Guba and Lincoln’s four criteria of credibility, dependability, transferability, and confirmability to ensure data trustworthiness.Results: Four main categories emerged from the data that collectively reflect the experiences of Iranian ICU nurses in caring for COVID-19 patients. These categories included ward overcrowding, changing patterns, complexity of care, and feeling exhausted. All of these categories show that Iranian ICU nurses were under intense pressure during the COVID-19 pandemic.Conclusion: This study showed that taking care of patients was very exhausting for nurses during the COVID-19 pandemic. Hospital authorities and nursing administrators have a responsibility to support nurses and provide them facilities to feel less stress and uncertainty, while caring for patients with COVID-19

    Saffron Flower and Stigma Yield Changes in Response to Application of Different Levels of super Absorbent Polymer

    Saffron production will be increased in arid regions by providing sufficient water through appropriate agronomic strategies. Therefore, in this investigation the impact of different rates of super absorbent polymer (SAP) including 0,10, 20, 30, 40 and 50 kg ha-1 was studied on saffron flowering traits during two growth cycles. Results showed that the influence of experimental factor was significant on almost all studied indices in both years. Number of flower was on average 22% more than control when 30-40 kg ha-1 SAP was used. Flower yield in the first growth cycle was higher in 40 kg ha-1 (24.9 g.m-2) and in the second one in 30 kg ha-1 (89.4 g.m-2) SAP application, while these values in control treatment were 19.7 and 71.2 g.m-2 for the first and second seasons, respectively. Altogether in two years application of 40 kg ha-1 SAP produced the highest fresh stigma + style yield (4.54 g.m-2) compared with the control (3.47 g.m-2). SAP application at the rates of more than 30 kg ha-1 increased dry stigma yield in average of two years (0.45, 0.50, 0.49 and 0.44 g.m-2 for 0, 30, 40 and 50 kg.ha-1 treatments, respectively). In addition, SAP usage at the rate of 40 kg ha-1 produced the highest number (12.6) and weight (86 g per plant) of replacement corms at the end of second growth season. In total, it seems that SAP consumption can be considered as a suitable option for improvement of saffron flowering in areas affected by drought stress

    Barriers to support nurses as second victim of medical errors: A qualitative study

    BackgroundGiven the inevitability of medical errors and their impact on health workers, providing support to those who suffer is vital for their physical and mental recovery. Identifying the barriers to obtaining support is imperative in this regard.AimsThe current study was conducted to identify the barriers regarding supporting nurses as second victims of nursing errors in clinical settings in Iran.MethodsThis qualitative study was conducted with a sample, which was included 18 nurses. The subjects were selected through the purposive sampling method, and data were collected using in-depth and semi-structured interviews. The data were analysed using methods as described by Graneheim and Lundman (citation needed). The research context included the general and specialized departments of hospitals in Tehran, Iran, during 2017.ResultsAccording to the results, mismanagement, Cultural barriers, inadequate information, and Legal barriers were the main barriers to supporting nurses.ConclusionTraining nurses about the second victim phenomenon is recommended as well as the methods to manage the effects of this phenomenon, the supportive resources, and legal issues

    Impact of Ssuper Absorbent Polymer and Irrigation Management on Seed and Essential Oil Yields of Cumin

    Two field and laboratory experiments were conducted to investigate the effects of superabsorbent polymer (SAP) and irrigation management on seed and essential oil yields of Cumin, as well as the impact of water quality on water holding capacity of SAP. Salinity had a negative effect on the amount of water absorbed by SAP (335 and 59 g H2O per g SAP, for distilled water and solution of 0.5% NaCl, respectively). SAP application (30 kg.ha-1) along with three times irrigation at sowing, flowering and seed filling stages increased the amounts of seed and essential oil yields by 2.79 and 3.05 times, compared to control. Positive effects of SAP were related to enhancement of soil water holding capacity (120 gr irrigation water per gr SAP), leaf area duration (one week) and subsequently grain filling period
