Thorax organ dose estimation in computed tomography based on patient CT data using Monte Carlo simulation
Background: This study presents pati ent specific and organ dose esti mati on in computed tomography (CT) imaging of thorax directly from pati ent CT image using Monte Carlo simulati on. Pati ent's CT image is considered as the pati ent specific phantom and the best representati ve of pati ent physical index in order to calculate specific organ dose. Materials and Methods: EGSnrc/BEAMnrc Monte Carlo (MC) System was used for CT scanner simulati on and DOSXYZnrc was used in order to produce pati ent specific phantom and irradiati on of photons to phantom in step and shoot mode (axial mode). In order to calculate pati ent thorax organ dose, pati ent CT image of thorax as voxelized phantom was divided to a 64x64x20 matrix and 6.25 x 6.25 x 6.25 mm3 voxel size and this phantom was imported to DOSXYZnrc code. MC results in unit of Gy/parti cle were converted to absorbed dose in unit of mGy by a conversion factor (CF). We calculated pati ent thorax organ dose in MC simulati on from all irradiated slices, in 120 kV and 80 kV photon energies. Results: Effecti ve dose was obtained from organ dose and organ weighti ng factor. Esophagus and spinal cord received the lowest, and bone received the highest dose. In our study, effecti ve dose in CT of thorax was 7.4 mSV and 1.8 mSv in 120 and 80 kV, respecti vely. Conclusion: The results of this study might be used to provide the actual pati ent organ dose in CT imaging and calculati on of real effecti ve dose based on organ dose
One-Year Efficacy of Expanded Polytetrafluoroethylene Vascular Graft in Eighty-Three Hemodialysis Patients
<p class="MsoNormal" style="margin: 0cm 0cm 0pt;"><span style="font-size: small;"><span style="font-family: Times New Roman;"><strong><span style="mso-bidi-language: FA;">Purpose:</span></strong><span style="mso-bidi-language: FA;"> To evaluate the patency and efficacy of expanded Polytetrafluoroethylene (ePTFE) vascular graft in hemodialysis patients.</span></span></span></p><p class="MsoNormal" style="margin: 0cm 0cm 0pt;"><span style="font-size: small;"><span style="font-family: Times New Roman;"><strong><span style="mso-bidi-language: FA;">Materials and Methods:</span></strong><span style="mso-bidi-language: FA;"> In a prospective study from January 1999 to January 2001 at Sina hospital 41 patients underwent implantation of 6-mm vascular grafts and 42 underwent implantation of 8-mm grafts in order to make vascular assess for hemodialysis. They were followed up to 12 months, observing the complications. </span></span></span></p><p class="MsoNormal" style="margin: 0cm 0cm 0pt;"><span style="font-size: small;"><span style="font-family: Times New Roman;"><strong><span style="mso-bidi-language: FA;">Results:</span></strong><span style="mso-bidi-language: FA;"> Mean patients’ age was 52.2 years. Thirty-seven of them were females and 46 were males. Over a 12 months period of follow-up, 12 cases of graft infection (14.5%), 21 cases of thrombosis (25.3%), 7 cases with both complications (8.4%), and 1 case of pseudoaneurism (1.2%) were observed. One-year patency rate was 34.9%. Diabetes was the only factor associated with lower patency rate (27% versus 57%, p<0.05).</span></span></span></p><p class="MsoNormal" style="margin: 0cm 0cm 0pt;"><span style="font-size: small;"><span style="font-family: Times New Roman;"><strong><span style="mso-bidi-language: FA;">Conclusion:</span></strong><span style="mso-bidi-language: FA;"> ePTFE vascular graft seems to be an appropriate substitute for arteriovenous fistula as a vascular assess in hemodialysis patients. Educating patients and good care can decrease the rate of infection, thrombosis, and other complications, resulting in a better patency and lower morbidity rate.</span></span></span></p>
Overview of hydatid disease in Iranian children
Background: Hydatid disease (HD) is still an important health hazard in the world. This disease is a parasitic infestation endemic in many sheep- and cattle-raising areas such as Iran. Objectives: This study aimed to review the clinical manifestations, laboratory aspects, imaging findings, and management of HD. Patients and Methods: Data were collected from the medical records of patients diagnosed with HD in eight referral hospitals in different provinces of Iran from 2001 to 2014. Results: Overall, 161 children at a mean age of 9.25 ± 3.37 years (age range = 1 - 15 years old) hospitalized with a definite diagnosis of the hydatid cyst between 2001 and 2014 were studied. The male-to-female ratio was 1.6:1. The most commonly involved organ was the lung (67.1), followed by the liver (44.1) and a combined liver and lung involvement was found in 15.5 of the patients. The cysts were found more frequently in the right lobe of the liver and lung than in the left lobe. The most frequent complaints were fever (35.4) and abdominal pain (31.7), and the most frequent sign was an abdominal mass in the liver involvement and cough in the lung involvement. There was a high eosinophil count (> 500/micL) in 41 of our cases. A high erythrocyte sedimentation rate (> 30) or positive C-reactive protein (based on the qualitative method) was found in 18.6 of the patients and leukocytosis > 15000/micL in 29.2 of the children. Ultrasonography was the main imaging test, with an accuracy rate of 96, and chest X-ray was helpful in 88.6 of the cases. Surgery was performed in 89 of the patients, and selective patients underwent percutaneous aspiration-injection-reaspiration drainage or medical treatment. Conclusions: The lung was the most commonly involved organ in the children recruited in the present study. Given the high probability of multiple organ involvement, we recommend that patients with HD be assessed via ultrasonography and chest X-ray. In endemic regions, unexplained eosinophilia should be considered as a parasitic disease like HD and its complications. © 2015 Pediartric Infections Research Center
Characterization of the serum levels of Meteorin-like in patients with inflammatory bowel disease and its association with inflammatory cytokines
Background: Meteorin-like (Metrnl) is an adipokine with insulin sensitizing and anti-inflammatory properties that has been discovered recently. The relation among Metrnl, Inflammatory Bowel Disease (IBD), and obesity has been unexplored yet. Methods: The present study was conducted on 54 healthy control, 42 Ulcerative Colitis (UC), and 43 Crohn�s disease (CD) patients who were diagnosed by pathological examination. In all participants, serum levels of adiponectin, Metrnl, interleukin (IL)-6, and Tumor necrosis factor (TNF-α) were measured using ELISA kits. Results: Metrnl concentration was considerably lower in both UC (85.25 ± 36.55 pg/mL) and CD (76.93 ± 27.92 pg/mL) patients in comparison to control (107.52 ± 35.33 pg/mL). In addition, it was seen that both patient groups have a decreased level of adiponectin compared to the controls. Besides that, the level of IL-6 and TNF-α were significantly greater in the patient groups. Moreover, the result showed that the level of Metrnl is inversely correlated with body mass index (BMI) in the controls and the patients. Metrnl levels are also inversely associated with IL-6, and TNF-α in both of the patient groups. Conclusions: The current study is the first one reporting the decreased levels of Metrnl in serum among patients with IBD, which is inversely related with BMI, TNF-α, and IL-6. These results suggested a possible relation of Metrnl with the pathogenesis of IBD, particularly through inflammatory process, although further studies are warranted to dissect the possible mechanism. © 2020, The Author(s)
Exciton swapping in a twisted graphene bilayer as a solid-state realization of a two-brane model
It is shown that exciton swapping between two graphene sheets may occur under
specific conditions. A magnetically tunable optical filter is described to
demonstrate this new effect. Mathematically, it is shown that two turbostratic
graphene layers can be described as a "noncommutative" two-sheeted
(2+1)-spacetime thanks to a formalism previously introduced for the study of
braneworlds in high energy physics. The Hamiltonian of the model contains a
coupling term connecting the two layers which is similar to the coupling
existing between two braneworlds at a quantum level. In the present case, this
term is related to a K-K' intervalley coupling. In addition, the experimental
observation of this effect could be a way to assess the relevance of some
theoretical concepts of the braneworld hypothesis.Comment: 15 pages, 3 figures, final version published in European Physical

Risk factor investigation for cardiovascular health through WHO STEPS approach in Ardabil, Iran
Objectives: Reliable evidence is the keystone for any noncommunicable disease (NCD) prevention
plan to be initiated. In this study we carried out a risk factor investigation based on the WHO Stepwise approach to Surveillance (STEPS).
Methods: The study was conducted on 1000 adults between 15 and 64 years of age living in Ardabil province, north-west Iran during 2006, based on the WHO STEPS approach to surveillance of risk factors for NCD. At this stage only the first and second steps were carried out. Data
were collected through standard questionnaires and methods analyzed using STATA version 8 statistical software package.
Results: 29.0% of men and 2.6% of women were current daily tobacco smokers. The mean number of manufactured cigarettes smoked per day was 18.9 among current daily smokers.
Smoking was most prevalent among men of low-income families and those of lower education The mean body mass index (BMI) was 26.6 kg/m2, and was significantly correlated with systolic blood pressure. 58.9% were overweight or obese; 18.0% had raised blood pressure and 3.7%
had isolated systolic hypertension. The mean number of servings of fruit consumed per day was 1.1; 33.1% had low levels of activity. Combined risk factor analysis showed that 4.1%of participants were in the low-risk group (up to 5.1% among men and 3.2% among women).Those in the high-risk group comprised 25.6% in the 25- to 44-year age group and 49.7%in the 45- to 64-year age group. Mean BMI increased by age in both sexes at least at the firstthree decades of adult life.
Conclusion: Based on observed status of risk for cardiovascular health, burden of cardiovascular diseases is expected to increase if an effective prevention strategy is not undertaken
Detection of very high energy gamma-ray emission from the gravitationally-lensed blazar QSO B0218+357 with the MAGIC telescopes
Context. QSO B0218+357 is a gravitationally lensed blazar located at a
redshift of 0.944. The gravitational lensing splits the emitted radiation into
two components, spatially indistinguishable by gamma-ray instruments, but
separated by a 10-12 day delay. In July 2014, QSO B0218+357 experienced a
violent flare observed by the Fermi-LAT and followed by the MAGIC telescopes.
Aims. The spectral energy distribution of QSO B0218+357 can give information on
the energetics of z ~ 1 very high energy gamma- ray sources. Moreover the
gamma-ray emission can also be used as a probe of the extragalactic background
light at z ~ 1. Methods. MAGIC performed observations of QSO B0218+357 during
the expected arrival time of the delayed component of the emission. The MAGIC
and Fermi-LAT observations were accompanied by quasi-simultaneous optical data
from the KVA telescope and X-ray observations by Swift-XRT. We construct a
multiwavelength spectral energy distribution of QSO B0218+357 and use it to
model the source. The GeV and sub-TeV data, obtained by Fermi-LAT and MAGIC,
are used to set constraints on the extragalactic background light. Results.
Very high energy gamma-ray emission was detected from the direction of QSO
B0218+357 by the MAGIC telescopes during the expected time of arrival of the
trailing component of the flare, making it the farthest very high energy
gamma-ray sources detected to date. The observed emission spans the energy
range from 65 to 175 GeV. The combined MAGIC and Fermi-LAT spectral energy
distribution of QSO B0218+357 is consistent with current extragalactic
background light models. The broad band emission can be modeled in the
framework of a two zone external Compton scenario, where the GeV emission comes

A search for spectral hysteresis and energy-dependent time lags from X-ray and TeV gamma-ray observations of Mrk 421
Blazars are variable emitters across all wavelengths over a wide range of
timescales, from months down to minutes. It is therefore essential to observe
blazars simultaneously at different wavelengths, especially in the X-ray and
gamma-ray bands, where the broadband spectral energy distributions usually
peak.
In this work, we report on three "target-of-opportunity" (ToO) observations
of Mrk 421, one of the brightest TeV blazars, triggered by a strong flaring
event at TeV energies in 2014. These observations feature long, continuous, and
simultaneous exposures with XMM-Newton (covering X-ray and optical/ultraviolet
bands) and VERITAS (covering TeV gamma-ray band), along with contemporaneous
observations from other gamma-ray facilities (MAGIC and Fermi-LAT) and a number
of radio and optical facilities. Although neither rapid flares nor significant
X-ray/TeV correlation are detected, these observations reveal subtle changes in
the X-ray spectrum of the source over the course of a few days. We search the
simultaneous X-ray and TeV data for spectral hysteresis patterns and time
delays, which could provide insight into the emission mechanisms and the source
properties (e.g. the radius of the emitting region, the strength of the
magnetic field, and related timescales). The observed broadband spectra are
consistent with a one-zone synchrotron self-Compton model. We find that the
power spectral density distribution at Hz from the
X-ray data can be described by a power-law model with an index value between
1.2 and 1.8, and do not find evidence for a steepening of the power spectral
index (often associated with a characteristic length scale) compared to the
