    A variety of pulmonary complications occur in patients during and following nonthoracic surgery. These may include hypoxemia, atelectasis, bronchitis, pneumonia, and respiratory failure. The incidence of these complications is high and remains a major cause of morbidity and mortality in the postoperative state. Even so, this morbidity may be underestimated since hypoxemia, the most common pulmonary complication, exerts its influence throughout many organ systems through inadequate tissue oxygenation

    The potential therapeutic effects of creatine supplementation on body composition and muscle function in cancer

    Low muscle mass in individuals with cancer has a profound impact on quality of life and independence and is associated with greater treatment toxicity and poorer prognosis. Exercise interventions are regularly being investigated as a means to ameliorate treatment-related adverse effects, and nutritional/supplementation strategies to augment adaptations to exercise are highly valuable. Creatine (Cr) is a naturally-occurring substance in the human body that plays a critical role in energy provision during muscle contraction. Given the beneficial effects of Cr supplementation on lean body mass, strength, and physical function in a variety of clinical populations, there is therapeutic potential in individuals with cancer at heightened risk for muscle loss. Here, we provide an overview of Cr physiology, summarize the evidence on the use of Cr supplementation in various aging/clinical populations, explore mechanisms of action, and provide perspectives on the potential therapeutic role of Cr in the exercise oncology setting

    The Importance of Optical Wavelength Data on Atmospheric Retrievals of Exoplanet Transmission Spectra

    Exoplanet transmission spectra provide rich information about the chemical composition, clouds and temperature structure of exoplanet atmospheres. Most exoplanet transmission spectra only span infrared wavelengths (\gtrsim 1 μm\rm{\mu m}), which can preclude crucial atmospheric information from shorter wavelengths. Here, we explore how retrieved atmospheric parameters from exoplanet transmission spectra change with the addition of optical data. From a sample of 14 giant planets with transit spectra from 0.3-4.5 μm\rm{\mu m}, primarily from the Hubble and Spitzer space telescopes, we apply a free chemistry retrieval to planetary spectra for wavelength ranges of 0.3-4.5 μm\rm{\mu m}, 0.6-4.5 μm\rm{\mu m}, and 1.1-4.5 μm\rm{\mu m}. We analyse the posterior distributions of these retrievals and perform an information content analysis, finding wavelengths below 0.6 μm\rm{\mu m} are necessary to constrain cloud scattering slope parameters (loga\log{a} and γ\gamma) and alkali species Na and K. There is limited improvement in the constraints on the remaining atmospheric parameters. Across the population, we find limb temperatures are retrieved colder than planetary equilibrium temperatures but have an overall good agreement with Global Circulation Models. As JWST extends to a minimum wavelength of 0.6 μm\rm{\mu m}, we demonstrate that exploration into complementing JWST observations with optical HST data is important to further our understanding of aerosol properties and alkali abundances in exoplanet atmospheres.Comment: 26 pages, 12 figures.     Recent developments in liquid encapsulated Czochralski techniques for the growth of semiinsulating GaAs for integrated circuit applications have resulted in significant improvements in the quality and quantity of GaAs material suitable for device processing. The emergence of high performance GaAs integrated circuit technologies has accelerated the demand for high quality, large diameter semiinsulating GaAs substrates. The new device technologies, including digital integrated circuits, monolithic microwave integrated circuits and charge coupled devices have largely adopted direct ion implantation for the formation of doped layers. Ion implantation lends itself to good uniformity and reproducibility, high yield and low cost; however, this technique also places stringent demands on the quality of the semiinsulating GaAs substrates. Although significant progress was made in developing a viable planar ion implantation technology, the variability and poor quality of GaAs substrates have hindered progress in process development

    Revisiting the 26.5°C Sea Surface Temperature Threshold for Tropical Cyclone Development

    Abstract A high sea surface temperature is generally accepted to be one of the necessary ingredients for tropical cyclone development, indicative of the potential for surface heat and moisture fluxes capable of fueling a self-sustaining circulation. Although the minimum 26.5°C threshold for tropical cyclogenesis has become a mainstay in research and education, the fact that a nonnegligible fraction of storm formation events (about 5%) occur over cooler waters casts some doubt on the robustness of this estimate. Tropical cyclogenesis over subthreshold sea surface temperatures is associated with low tropopause heights, indicative of the presence of a cold trough aloft. To focus on this type of development environment, the applicability of the 26.5°C threshold is investigated for tropical transitions from baroclinic precursor disturbances in all basins between 1989 and 2013. Although the threshold performs well in the majority of cases without appreciable environmental baroclinicity, the potential for development is underestimated by up to 27% for systems undergoing tropical transition. An alternative criterion of a maximum 22.5°C difference between the tropopause-level and 850-hPa equivalent potential temperatures (defined as the coupling index) is proposed for this class of development. When combined with the standard 26.5°C sea surface temperature threshold for precursor-free environments, error rates are reduced to 3%–6% for all development types. The addition of this physically relevant representation of the deep-tropospheric state to the ingredients-based conceptual model for tropical cyclogenesis improves the representation of the important tropical transition-based subset of development events.</jats:p

    Exercise intervention and sexual function in advanced prostate cancer: A randomised controlled trial

Objectives: Treatments for prostate cancer such as androgen deprivation therapy (ADT), surgery and radiation therapy can adversely affect sexual, urinary and bowel function. Preliminary research has demonstrated the efficacy of exercise to preserve sexual function in men with localised prostate cancer receiving ADT, though this has yet to be investigated in a metastatic setting. We examined the effects of a 12-week exercise programme comprising resistance, aerobic and flexibility training on sexual health and function in men with advanced prostate cancer. Methods: Patients with prostate cancer (70.0±8.4 year; body mass index 28.7±4.0 kg/m2) with bone metastases (rib/thoracic spine, 66.7%; lumbar spine, 43.9%; pelvis, 75.4%; femur, 40.4%; humerus, 24.6%; other sites, 70.2%) were randomly assigned to supervised exercise 3 days/week (n=28) or usual care (n=29). Sexual health and function were assessed using the International Index of Erectile Function, the Expanded Prostate Cancer Index Composite and the EORTC-PR25 at baseline and 12 weeks. Results: Patients attended 89% of planned sessions and there were no adverse events. After adjusting for baseline values, there was no significant difference between groups for any measure of sexual function and activity (p \u3e 0.05). Additionally, there was no significant difference between groups for urinary and bowel function assessed by the EORTC-PR25 (p \u3e 0.05). Conclusions: A short-Term programme of supervised exercise does not appear to enhance indices of sexual health and function in men with advanced prostate cancer. Limitations of the intervention included the conservative modular exercise programme, which deliberately avoided loading bone metastatic sites

    Cloning and expression of a human neutral amino acid transporter with structural similarity to the glutamate transporter gene family

    A cDNA was isolated from human brain that encodes an amino acid sequence 34-39% identical to previously published glutamate transporter sequences. Injection of RNA transcribed from this cDNA into Xenopus oocytes resulted in expression of a transport activity with the properties of the neutral amino acid uptake system ASC. Superfusion of alanine, serine, and cysteine evoked sodium-dependent inward currents in voltage-clamped oocytes expressing the transporter. These currents were dose-dependent, stereospecific, and saturable, with Km values ranging from 29 to 88 microM. Northern blot analyses revealed ubiquitous expression of this gene, termed ASCT1, consistent with the general metabolic role ascribed to system ASC
