    Recombinant antibodies can be used to diagnose, treat and prevent disease by exploiting their specific antigen-binding activities. A large number of drugs currently in development are recombinant antibodies and most of these are produced in cultured rodent cells. Although such cells produce authentic functional products, they are expensive, difficult to scale-up and may contain human pathogens. Plants represent a cost-effective, convenient and safe alternative production system and are slowly gaining acceptance. Five plant-derived therapeutic recombinant antibodies (plantibodies) are undergoing clinical evaluation, three of which can be used as prophylactics

    Associations of ambivalent leadership with distress and cortisol secretion

    Ambivalent social ties, i.e., whereby a relationship is evaluated simultaneously in positive and negative terms, are a potential source of distress and can perturb health-relevant biological functions. Social interactions at the workplace, in particular with supervisors, are often described in ambivalent terms, but the psychological and psychobiological impact of such interactions has received little scientific attention. The current study examined associations between ambivalent attitudes towards one's supervisor, perceived distress (general and work-related), and diurnal dynamics of the stress hormone cortisol. 613 employees evaluated their supervisor in terms of positive and negative behaviors, which was combined into an ambivalent index. Higher ambivalence was associated with higher perceived distress and work-related stress (p < .001), and with a larger cortisol awakening response and higher day-time secretion post-awakening (p < .01). The present study is the first to identify ambivalence towards supervisors as a predictor of employee distress and stress-related endocrine dysregulation. In consequence, focusing solely on positive or negative leader behavior may insufficiently capture the true complexity of workplace interactions and attempts to compensate negative behaviors with positive are unlikely to reduce distress-but quite the opposite-by increasing ambivalence

    The cost-effectiveness of public health interventions examined by NICE from 2011 to 2016

    Background A review of economic evaluations of public health interventions assessed by NICE between 2005 and 2010 found 85% were cost-effective. Owen et al. (The cost-effectiveness of public health interventions. J Public Health 2012;34(1):37–45). With significant pressure on budgets the role of economics in securing funding remains important. This study updates the earlier analysis. Methods Economic evaluations carried out between 2011 and 2016 were categorized: cost-utility analysis (CUA), cost-effectiveness analysis (CEA), cost-benefit analysis (CBA) and cost-consequences analysis (CCA). Cost-effectiveness estimates were analysed and compared with Owen et al. (The cost-effectiveness of public health interventions. J Public Health 2012;34(1):37–45). Results Of 43 guidelines examined, 23 used CUA for specific interventions yielding 138 base-case incremental cost-effectiveness ratio (ICER) estimates, 11 used CUA for a threshold or ‘what if’ analysis, 1 used CEA, 3 used CCA, 1 used CBA and CUA and 1 used CEA and CUA, 5 did not require economic modelling. Compared with the earlier period, the median ICER for the 138 estimates was substantially higher (£7843 versus £1053) and there was greater variability (a higher proportion in the later period was cost-saving, but a higher proportion was also over £20 000 per quality adjusted life year). Conclusions Nearly two-thirds (63%) of public health interventions assessed were cost-effective. However, increased variability in estimates highlights the importance of assessing cost-effectiveness to ensure good use of scarce resources
