    Evaluation of Dental Surface after De-Bonding Orthodontic Bracket Bonded with a Novel Fluorescent Composite: In Vitro Comparative Study

    The use of a new fluorescent composite can reduce some of the problems related to procedures of de-bonding orthodontic bracket (enamel damage, dentine lesions, and composite residuals). The aim of the presented study was to compare the effect of fluorescent and conventional non-fluorescent composite on dental surface and composite remnants by in vitro de-bonding tests. De-bonding of florescent composite (DFC) and the de-bonding of standard composite (DSC) were performed by operators on an in vitro sample of 48 teeth under UV light (360–370 nm min 20 mW/cm2). Modified ARI (Adhesive Remnant Index), scored under 5.0×/235 magnification, was used for evaluation of dental surface after the procedure, and the duration required for de-bonding was measured. Significant differences in ARI between the two groups were observed (Pearson two-tailed p = 0.006 1.4 ± 0.1 95% C.I.), and the average duration of de-bonding was 38 s (DFC) and 77 s (DSC) per tooth, respectively (Mann–Whitney test p = 0.015; 57.7 ± 19.9 95% C.I.). The use of fluorescent composite could significantly improve the quality of de-bonding by reducing the quantity of composite residuals and visible enamel damage, while reducing time needed for successful procedure performance

    Orthodontic movement of a dilacerated maxillary incisor in mixed dentition treatment

    Dilaceration is a dental deformity characterized by an angulation between crown and root causing non-eruption of the tooth. It generally occurs following trauma to the deciduous dentition the apices of which lie close to the permanent tooth buds. Surgical extraction used to be the first choice in treating the severely dilacerated teeth. In this case report, it is described how a dilacerated central incisor was successfully moved into alignment in a young patient with proper surgical and orthodontic management avoiding use of prosthetic/implant devices. The panoramic radiograph after the traumatic injury is compared with the one at the end of treatment. The results were sustainable over long-term in both periodontal and aesthetic terms

    In recent years there has been a significant increase in the number of elderly patients requesting orthodontist consultations. Masticatory performance is an important factor influencing the quality of life in independent healthy elderly subjects and only the restoration of skeletal and dental relationships in the three planes of space can assure oral health and functions with the consequential nutritional balance of the whole organism. The rehabilitation treatment in elderly subjects must comprehend orthodontic and prosthetic needs. This kind of treatment has two main goals: firstly, the restoration of oral function and secondly the restoration of facial morphology.Objective: In recent years there has been a significant increase in the number of elderly patients requesting orthodontist consultations.Masticatory performance is an important factor influencing the quality of life in independent healthy elderly subjects and only the restoration of skeletal and dental relationships in the three planes of space can assure oral health and functions with the consequential nutritional balance of the whole organism.The rehabilitation treatment in elderly subjects must comprehend orthodontic and prosthetic needs.     Straight-wire appliances: standard versus individual prescription

    In this article the individual patient (IP) appliance is described. It consists of 250 options of bracket and band variations as the straight wire appliances. Increasing the bracket capabilities means using an increasing number of brackets, each with a specific design created for a treatment situation. The objective of IP appliance is to eliminate wire bending from orthodontic treatment and improve the treatment results. To manage this technique, a computer software is needed. Internet offers significant possibilities in managing each patient by patient basis. The clinician is required to make the diagnosis and treatment plan before ordering the appliance.     Headache and transverse maxillary discrepancy

    The purpose of this study is to evaluate the usefulness of rapid palatal expansion in growing patients to eliminate the nasal pyramid stenosis as a cause of primary headache. Patients were invited to participate in the clinical study at the University of Milan, Department of Orthodontics. Forty-one growing patients of both genders suffering from primary neurovascular headaches and having transverse maxillary deficiency were studied before and after rapid palatal expansion. A clinical examination, postero-anterior radiography and rhinomanometry were performed. After rapid palatal expansion (RPE) therapy all patients showed a constant and important increase in the values relating to both skeletal and dental structures, a significant reduction in the mean nasal resistance, and a significant decrease or elimination of headache symptoms. This therapy provides a preventive alternative for surgical procedures, such as the neurovascular decompressive septo-ethmoidosphenoidectomy performed in adult patient

    Space-maintaining management in maxillary sinus lifting: a novel technique using a resorbable polymeric thermo-reversible gel

    Several techniques have been proposed to achieve sinus floor elevation and the formation of new bone through the grafting of autologous, heterologous, or alloplastic materials. The grafted materials act as a scaffold for bone formation inside the maxillary sinus. This study investigated a non-graft sinus lifting procedure using a resorbable polymeric thermo-reversible gel. A space-maintaining approach to sinus lifting, using a resorbable polymeric thermo-reversible gel, was applied in 11 patients undergoing implant treatment in the atrophic posterior maxilla. After a healing period of 6 months, a total of 14 implants were placed; biopsies were taken and evaluated histologically and histomorphometrically. The parameters evaluated included the percentages of new bone formation, residual gel, and fibrous tissue. Histological examination showed the formation of new bone with no fibrous tissue or severe inflammatory cellular infiltration. The percentage of newly formed bone was in the range of 54\u201360%; this consisted of both lamellar and woven bone. No foreign-body reaction was observed. The mean quantities of both residual gel and connective tissue were small. This non-graft sinus lifting procedure using a space-maintaining gel appears to stimulate predictable bone formation; it is thus a useful technique for promoting bone formation in the sinus
