    Stimuli-responsive poly(ampholyte)s containing L-histidine residues: synthesis and protonation thermodynamics of methacrylic polymers in the free and in the cross-linked gel forms

    Methacrylate-structured poly(ampholyte)s were synthesized in the homopolymer and copolymer forms starting from the N-methacryloyl-L-histidine (MHist) and the N-isopropylacrylamide (NIPAAm). They were also obtained in the cross-linked (hydrogel) form, showing a close thermodynamic behaviour as that shown by the corresponding soluble free polymer analogues. Viscometric data revealed that the minimum hydrodynamic volume of the polymer at its isoelectric point (pH 5) shifted to lower pHs as the NIPAAm content increased, and beyond a critical low MHist content the reduced viscosity decreased, even at low pHs. The phenomenon was attributed to hydrophobic forces between the isopropyl groups outweighing the repulsive electrostatic interactions of the polymer in the positively charged form. A similar behaviour was shown by the corresponding hydrogel. The latter also revealed a different phase transition phenomenon induced by external stimuli (temperature, pH, ionic strength, electric current) when compared to the acrylate-structured analogues. The polyMHist, as well as the corresponding monomer, was found for two days to be non toxic against the mouse osteoblasts (MC3T3-E1)

    Terminal ileum ileoscopy and histology in patients undergoing high-definition colonoscopy with virtual chromoendoscopy for chronic nonbloody diarrhea : a prospective, multicenter study

    Background and aims: Ileo-colonoscopy is the procedure of choice for chronic nonbloody diarrhea (CNBD) of unknown origin. Histological evaluation at different colonic sites is mandatory to assess the presence of microscopic colitis. However, the value of routine ileal biopsy on normal-appearing mucosa as assessed by means of standard-resolution white-light ileoscopy is controversial given its reported low diagnostic yield. Hence, we have assessed for the first time the accuracy of retrograde ileoscopy using high-definition and dyeless chromoendoscopy (HD + DLC), thereby calculating the impact and cost of routine ileal biopsy in CNBD. Methods: Patients with CNBD of unknown origin were prospectively enrolled for ileo-colonoscopy with HD + DLC at five referral centers. Multiple biopsies were systematically performed on each colorectal segment and in the terminal ileum for histopathological analysis. Results: Between 2014 and 2017, 546 consecutive patients were recruited. Retrograde ileoscopy success rate was 97.6%. A total of 492 patients (mean age: 53 \ub1 18 years) fulfilled all the inclusion criteria: Following endoscopic and histopathological work-up, 7% had lymphoid nodular hyperplasia and 3% had isolated ileitis. Compared to the histopathology as the gold standard, retrograde ileoscopy with HD + DLC showed 93% sensitivity, 98% specificity and 99.8% negative predictive value. In patients with normal ileo-colonoscopy, ileum histology had no diagnostic gain and resulted in a cost of US $26.5 per patient. Conclusions: Retrograde ileoscopy with HD + DLC predicts the presence of ileitis in CNBD with excellent performance. The histopathological evaluation of the terminal ileum is the gold standard for the diagnostic assessment of visible lesions but has no added diagnostic value in CNBD patients with negative ileo-colonoscopy inspection using modern endoscopic imaging techniques

    Spontaneous polymerization of benzofulvene monomers bearing a 4-Pyri- dylacetylene substituent in different positions of the benzofulvene scaffold

    Two benzofulvene derivatives bearing a 4-pyridylacetylene substituent in different positions (i. e. 2 and 6) of the benzofulvene scaffold are designed and synthesized to evaluate the effects on the spontaneous solid-state polymerization of the presence of the same substituent in two different key positions of the 3-phenylbenzoful-vene moiety. Both the benzofulvene derivatives showed the tendency to polymerize spontaneously in the consequence of solvent removal under reduced pressure without the addition of catalysts or initiators. The macromolecular structure of the stemming polymeric materials was investigated by NMR spectroscopy and MALDI-TOF mass spectrometry. Both NMR and MALDI-TOF studies confirmed the polymeric nature of the materials and suggested for the polybenzofulvene derivative bearing the 4-pyridylacetylene substituent in po-sitions 6 a higher structural homogeneity with respect to the one bearing the same substituent in position 2. The photophysical characterization of the most homogeneous polybenzofulvene derivative led to the discovery of its outstanding hole mobility value, which was found to be around one order of magnitude higher than that pre-viously measured for two oligothiophene-based polybenzofulvene derivatives and almost two orders of magni-tude higher than that of poly(vinylcarbazole), commonly used as hole-transporter matrix. This result places the new polybenzofulvene derivative in an outstanding position as a promising material for field-effect transistor (FET) device applications

    Acute chest syndrome in children with sickle cell disease: Data from a national AIEOP cohort identify priority areas of intervention in a hub-and-spoke system

    : Acute chest syndrome (ACS) is a frequent cause of hospitalization in sickle cell disease (SCD). Despite advances in acute care, many settings still lack knowledge about ACS best practices. After the AIEOP Guidelines were published in 2012, suggesting standardized management in Italy, a retrospective study was performed to assess the diagnostic and therapeutic pathways of ACS in children. From 2013 to 2018, 208 ACS episodes were presented by 122/583 kids in 11 centres. 73 were male, mean age 10.9 years, 85% African, 92% HbSS or Sβ°. In our hub-and-spoke system, a good adherence to Guidelines was documented, but discrepancies between reference centres and general hospitals were noted. Improvement is needed for timely transfer to reference centres, use of incentive spirometry, oxygen therapy and pain management

    Improved high-performance liquid chromatographic assay for determining organic acids in wines

    Lactic, acetic, tartaric, malic, succinic and citric acids in wine samples were determined as phenacyl esters. The sample was buffered at pH 6.8, mixed with a solution of phenacyl bromide and a crown ether in acetone, and heated at 100°C for 40 min. An aliquot of the reaction mixture was subjected to reverse-phase high-performance liquid chromatography. Recoveries of the six acids were higher than 95%. The specificity of this assay was good as assessed by quantifying wine acids with other chromatographic techniques. The within-run coefficient of variation ranged from 1.7% for acetic acid to 3.1% for citric acid. When compared with a similar, recently reported assay, our assay gave more reliable results, especially for citric acid
