Fat infusion and blood coagulation in patients undergoing surgery for esophageal cancer.
Twenty-six patients with stage III and IV esophageal cancer were randomly studied to evaluate the influence of fat emulsions (Intralipid) infusion on blood coagulation during total parenteral nutrition (TPN). The patients were divided into two groups: the first group (13 patients) received 24h. TPN; in the second group (13 patients) 1000 ml of 10\% Intralipid were added to TPN via a peripheral vein over a period of 12 h. (1.8-2.1 g/kg/day). Primary hemostasis, platelet adhesiveness, alterations in fibrin formation and risk factors were evaluated before treatment (Time 0), after 7 days (Time 1) and after 14 days (Time 2). Statistical analysis of results was also carried out. On the basis of the obtained results it is observed that Intralipid at the administered doses, did not cause any alterations in blood coagulation and particularly in platelet adhesiveness

Cervical cancer with paraaortic involvement: do patients truly benefit from tailored chemoradiation therapy? A retrospective study on 8 French centers.
We retrospectively studied the therapeutic significance of extended-field radiotherapy combined with concurrent platinum-based chemotherapy for the management of cervical carcinoma with paraaortic spread. Treatment response and survival outcomes were evaluated. One hundred and fifteen women were retrospectively studied. Radiological staging was conducted in 101 (87.8%) patients and paraaortic lymphadenectomy in 78 (67.8%). Patterns of treatment comprised chemoradiation therapy (100%), intracavitary brachytherapy (81.7%), completion surgery (60%) and neoadjuvant chemotherapy (4.3%). Four-year overall and disease-free survivals were 32.7% and 28.8%, respectively. Progression and relapse mostly involved the locoregional area and distant organs, rather than the paraaortic area. Advanced FIGO stage at baseline was the most significant prognostic factor (HR=3.02, p=0.01). Despite systematic extended-field chemoradiation therapy, paraaortic involvement in cervical cancer is associated with poor survival outcomes. The patterns of progression and recurrence suggest the existence of occult metastatic disease at presentation. Additional systemic treatment might thus be beneficial
Vater's papilla and periampullary area villous adenoma: personal experience about nine cases and review of the literature
Villous tumor of the duodenum is a rare disease. The authors report their personal experience from 1987 to 1993 with nine cases of duodenal villous tumors. The treatment was pancreatoduodenectomy in four cases, segmental resection of the duodenum in two cases and submucosal excision in three cases. The authors review the literature of the last 15 years in which only 241 cases of duodenal villous tumors are reported; in this review the clinical, diagnostic and therapeutic characteristics of the disease are analyzed. This malignancy presents peculiar characteristics for histologic diagnosis and pathology classification. For this reason, even surgical treatment deserves careful reflection to guarantee a curative procedure and to formulate a positive prognosis
