    Using a fuzzy inference system to control a pumped storage hydro plant

    The paper discusses the development of a fuzzy inference system (FIS) based governor control for a pumped storage hydroelectric plant. The First Hydro Company's plant at Dinorwig in North Wales is the largest of its kind in Europe and is mainly used for frequency control of the UK electrical grid. In previous investigations, a detailed model of the plant was developed using MATLAB(R)/SIMULINK(R) and this is now being used to compare FIS governor operation with the proportional-integral-derivative (PID) controller currently used. The paper describes the development of an FIS governor, and shows that its response to a step increase in load is superior to the PID under certain conditions of load. The paper proceeds to discuss the implications of these results in view of the possible practical application of an FIS governor at the Dinorwig plant

    Influence of hyperhomocysteinemia on the cellular redox state - Impact on homocysteine-induced endothelial dysfunction

    Hyperhomocysteinemia is an independent risk factor for the development of atherosclerosis. An increasing body of evidence has implicated oxidative stress as being contributory to homocysteines deleterious effects on the vasculature. Elevated levels of homocysteine may lead to increased generation of superoxide by a biochemical mechanism involving nitric oxide synthase, and, to a lesser extent, by an increase in the chemical oxidation of homocysteine and other aminothiols in the circulation. The resultant increase in superoxide levels is further amplified by homocysteinedependent alterations in the function of cellular antioxidant enzymes such as cellular glutathione peroxidase or extracellular superoxide dismutase. One direct clinical consequence of elevated vascular superoxide levels is the inactivation of the vasorelaxant messenger nitric oxide, leading to endothelial dysfunction. Scavenging of superoxide anion by either superoxide dismutase or 4,5-dihydroxybenzene 1,3-disulfonate (Tiron) reverses endothelial dysfunction in hyperhomocysteinemic animal models and in isolated aortic rings incubated with homocysteine. Similarly, homocysteineinduced endothelial dysfunction is also reversed by increasing the concentration of the endogenous antioxidant glutathione or overexpressing cellular glutathione peroxidase in animal models of mild hyperhomocysteinemia. Taken together, these findings strongly suggest that the adverse vascular effects of homocysteine are at least partly mediated by oxidative inactivation of nitric oxide

    Quinidine-Induced Immune Thrombocytopenia

    We have identified six cases of quinidine-induced immune thrombocytopenia based on clinical evidence and in association with elevated amounts of platelet surface IgG. The degree of thrombocytopenia did not correlate with severity of clinical symptoms, nor did it predict the amount of IgG on the platelet surface. Three of the patients recovered promptly after drug discontinuation alone whereas the other patients received additional corticosteroid therapy. The clinical presentation, mode of diagnosis, and therapeutic considerations in the treatment of drug-induced thrombocytopenia are discussed

    Tensor to Scalar Ratio in Non-Minimal Ď•4\phi^4 Inflation

    We reconsider non-minimal \lambda \phi^4 chaotic inflation which includes the gravitational coupling term \xi \mathcal{R} \phi^2, where \phi denotes a gauge singlet inflaton field and \mathcal{R} is the Ricci scalar. For \xi >> 1 we require, following recent discussions, that the energy scale \lambda^{1/4} m_P / \sqrt{\xi} for inflation should not exceed the effective UV cut-off scale m_P / \xi, where m_P denotes the reduced Planck scale. The predictions for the tensor to scalar ratio r and the scalar spectral index n_s are found to lie within the WMAP 1-\sigma bounds for 10^{-12} < \lambda < 10^{-4} and 10^{-3} < \xi < 10^2. In contrast, the corresponding predictions of minimal \lambda \phi^4 chaotic inflation lie outside the WMAP 2-\sigma bounds. We also find that r > 0.002, provided the scalar spectral index n_s > 0.96. In estimating the lower bound on r we take into account possible modifications due to quantum corrections of the tree level inflationary potential.Comment: 10 pages, 6 figure

    6G Wireless Systems: Vision, Requirements, Challenges, Insights, and Opportunities

    Mobile communications have been undergoing a generational change every ten years or so. However, the time difference between the so-called "G's" is also decreasing. While fifth-generation (5G) systems are becoming a commercial reality, there is already significant interest in systems beyond 5G, which we refer to as the sixth-generation (6G) of wireless systems. In contrast to the already published papers on the topic, we take a top-down approach to 6G. We present a holistic discussion of 6G systems beginning with lifestyle and societal changes driving the need for next generation networks. This is followed by a discussion into the technical requirements needed to enable 6G applications, based on which we dissect key challenges, as well as possibilities for practically realizable system solutions across all layers of the Open Systems Interconnection stack. Since many of the 6G applications will need access to an order-of-magnitude more spectrum, utilization of frequencies between 100 GHz and 1 THz becomes of paramount importance. As such, the 6G eco-system will feature a diverse range of frequency bands, ranging from below 6 GHz up to 1 THz. We comprehensively characterize the limitations that must be overcome to realize working systems in these bands; and provide a unique perspective on the physical, as well as higher layer challenges relating to the design of next generation core networks, new modulation and coding methods, novel multiple access techniques, antenna arrays, wave propagation, radio-frequency transceiver design, as well as real-time signal processing. We rigorously discuss the fundamental changes required in the core networks of the future that serves as a major source of latency for time-sensitive applications. While evaluating the strengths and weaknesses of key 6G technologies, we differentiate what may be achievable over the next decade, relative to what is possible.Comment: Accepted for Publication into the Proceedings of the IEEE; 32 pages, 10 figures, 5 table

    Second-line failure and first experience with third-line antiretroviral therapy in Mumbai, India

    Background: There are limited data on the failure of second-line antiretroviral therapy (ART) and the use of third-line ART in people living with HIV in resource-limited settings. Since 2011, the Médecins Sans Frontières (MSF) HIV/tuberculosis programme in Mumbai, India, has been providing third-line ART to patients in care. Objective: To describe the experiences and programmatic challenges during management of suspected second-line ART failure and third-line ART therapy for patients living with HIV, including the use of HIV viral load (VL) testing. Design: This was a retrospective, observational cohort study of patients with suspected second-line ART treatment failure, who were followed for at least 12 months between January 2011 and March 2014. Results: A total of 47 patients with suspected second-line failure met the inclusion criteria during the study period. Twenty-nine of them (62%) responded to enhanced adherence support, had a subsequent undetectable VL after a median duration of 3 months and remained on second-line ART. The other 18 patients had to be initiated on a third-line ART regimen, which consisted of darunavir–ritonavir, raltegravir, and one or more appropriate nucleoside or nucleotide reverse transcriptase inhibitors, based on the results of HIV genotype testing. Of the 13 patients for whom follow-up VL results were available, 11 achieved virological suppression after a median duration of 3 months on third-line ART (interquartile range: 2.5–3.0). No serious treatment-related adverse events were recorded. Conclusions: With intensive counselling and adherence support in those suspected of failing second-line ART, unnecessary switching to more expensive third-line ART can be averted in the majority of cases. However, there is an increasing need for access to third-line ART medications such as darunavir and raltegravir, for which national ART programmes should be prepared. The cost of such medications and inadequate access to VL monitoring and HIV genotype testing are currently major barriers to optimal management of patients failing second-line ART
