    Polycystic Kidney Disease with Renal failure Presenting as Incarcerated Inguinal Hernia in the ED

    Autosomal dominant polycystic kidney disease may present to the emergency department (ED) with vomiting, abdominal pain or hernias, renal insufficiency or failure, or bleeding from cerebral aneurysms. A 37-year-old man presented to the ED with signs and symptoms of incarcerated inguinal hernia. Laboratory studies showed renal failure with anion gap acidosis, and bedside ultrasound showed multicystic kidneys. Computed tomography confirmed the diagnosis. Emergency physicians should be aware of this common connective tissue defect and its serious associated conditions

    A review of the production cycle of titanium dioxide pigment

    Titanium is a very important element for several industrial applications, being one of the ninth most abundant elements in the Earth’s crust (0.63% wt). In this work it will discuss the different mining and industrial activities involved in the production of titanium dioxide. The first step ana-lyzed will treat about the beneficiation mining process of titanium mineral, and secondly, it will discuss the two main processes of the TiO2 manufacturing (sulphate and chloride routes). In addi-tion, we will show different uses of the titanium dioxide pigment as filler in paper, plastics and rubber industries and as flux in glass manufacture, etc. Finally, we will show that the old wastes are currently called co-products since they were valorized, being commercialized by the Spanish industry of TiO2 production in different fields such as agriculture, civil engineering, or cement manufacturing.Government of Andalusia’s project “Characterization and modelling of the phosphogypsum stacks from Huelva for their environmental management and control” (Ref.: RNM-6300

    The industrial sector is responsible for the generation of a large amount of solid waste, of which some is partially recycled, but the majority is deposited in landfills or landfills causing various negative impacts on the environment. Alkaline cements are attracting growing interest for their potential to allow the industry to operate within the constraints imposed on CO2 emissions. The objective of this research was to know the effect of different curing conditions on the compressive strength behavior of alkaline activated ceramic residues. As a result, it was determined that an alkali-activated matrix is significantly influenced when cured at a temperature of 70 ° C, reaching, at 90 days of age, a compressive strength of up to 39.3 MPa in contrast to 27.08 MPa. when curing was carried out in environmental conditions of 20 ° C (± 0.5 ° C) and 60% (± 5%) of relative humidity (RH). This work was complemented with a microstructural analysis that included Scanning Electron Microscopy (SEM) and X-ray Energy Dispersion Analyzer (EDX).El sector industrial es responsable de la generación de una gran cantidad de residuos sólidos, de los cuales algunos son parcialmente reciclados, pero la mayoría son depositados en vertederos o rellenos sanitarios provocando diversos impactos negativos en el medio ambiente. Los cementos alcalinos están atrayendo un interés creciente por su potencial para permitir que la industria opere dentro de las limitaciones impuestas a las emisiones de CO2. La presente investigación tuvo como objetivo conocer el efecto de diferentes condiciones de curado sobre el comportamiento de resistencia a compresión de los residuos cerámicos activados alcalinamente. Como resultado se determinó que una matriz álcali-activada es significativamente influenciada cuando se cura con una temperatura de 70°C alcanzando, a los 90 días de edad, una resistencia a la compresión de hasta 39,3 MPa en contraste con 27,08 MPa cuando el curado se realizó en condiciones ambientales de 20°C (± 0,5 ° C) y 60% (± 5%) de humedad relativa (RH).     Molecular characterization, expression and localization of a peroxiredoxin from the sheep scab mite, Psoroptes ovis

    The sheep scab mite, Psoroptes ovis, induces an intensely pruritic exudative dermatitis which is responsible for restlessness, loss of appetite and weight loss. Within the first 24 h of infection, there is a rapid inflammatory influx of eosinophils and apoptosis of the keratinocytes at the site of infection. The former cell type is capable of a sustained respiratory burst, toxic products of which may directly damage the mite and also contribute to lesion formation. Analysis of a P. ovis expressed sequence tag (EST) database identified a number of antioxidant enzyme-encoding sequences, including peroxiredoxin (thioredoxin peroxidase EC 1.11.1.15), all of which may help the mite endure the potentially toxic skin environment. A full length sequence encoding Po-TPx, a protein of 206 amino acids which showed high homology to a peroxiredoxin from the salivary gland of the tick Ixodes scapularis, was amplified from P. ovis cDNA. Recombinant Po-TPx was expressed in bacteria and antiserum to this protein was used to localize native Po-TPx in mite sections. Peroxiredoxin was localized, amongst other sites, to a subpharyngeal region in mite sections. The recombinant protein was recognized by sera from sheep infested with the mite suggesting that it may be secreted or excreted by the mite and interact with the host immune response

    Effects of Osteopathic Visceral Treatment in Patients with Gastroesophageal Reflux: A Randomized Controlled Trial

    Osteopathic manual treatment has been recommended as a non-pharmacological therapy for Gastroesophageal Reflux Disease (GERD). However, to date, no study has supported the effectiveness of this intervention with respect to the symptoms of the disease. Our goal was to assess the effect of an osteopathic manual technique for the lower esophageal sphincter on GERD symptoms, cervical mobility and on the C4 spinous process pressure pain threshold (PPTs). Methods: A randomized, double-blind placebo-controlled trial was performed. Sixty subjects suffering from GERD participated in this study and were randomly assigned to either an experimental group (EG) (n = 29), who received the osteopathic technique for the lower esophageal sphincter, or to a control group (CG) (n = 31), who received a manual contact, which mimicked the osteopathic technique without exerting any therapeutic force. Randomization was computer-generated, with allocation concealed by sequentially numbered, opaque, sealed envelopes. The GerdQ questionnaire was used to assess symptom changes the week after intervention. Cervical Range of Motion (CROM) and algometer were used to evaluate cervical mobility and PPTs before and after both treatments. Before-after between groups comparison (t-test) was used for statistical analysis of the outcome, with two measurement points (GerdQ), while repeated-measures ANOVA was used for those outcomes with four measurement points (CROM and PPT). Results: The application of the osteopathic manual treatment in subjects with GERD produced a significant improvement in symptoms one week after the intervention (p = 0.005) with a between-groups difference of 1.49 points in GerdQ score (95% CI: 0.47-2.49). PPT C4 improved in the EG after the treatment (p = 0.034; eta(2) = 0.048) (between-groups difference 8.78 Newton/cm(2); 95% CI: 0.48-17.09). CROM also increased in the EG compared to the CG (p < 0.001; eta(2) = 0.108) (between-groups difference 33.89 degrees; 95% CI: 15.17-52.61). Conclusions: The manual osteopathic technique produces an improvement in GERD symptoms one week after treatment, cervical mobility, and PPTs. This may mean that osteopathic treatment is useful for improving symptoms of GERD

    Time evolution of activity concentration of natural emitters in a scenario affected by previous phosphogypsum contamination

    The estuary formed by the confluence of Tinto and Odiel river-mouths is located in the South of Spain, close to Huelva town. This estuary has been deeply studied through the years because it has a double particularity. On one hand, since the beginning of the 1960s, the estuary has been affected by direct and indirect phosphogypsum (pg.) releases from two phosphoric acid and fertilizers factories that are working in the area. On the other hand, the pyrite mining operations upstream the Odiel and Tinto rivers has caused historically the formation of H2SO4, through oxidation of the natural sulphur deposits, the acidification of the waters and the consequent mobilisation of heavy metals from the mining area to the Huelva estuary. As a consequence, enhancement contamination levels in natural emitters from the ^'*U series were found in the surroundings of the factories in the previous years to 1998. However, in 1998 the management policy of waste releases drastically changed in the area, and direct discharges to Tinto and Odiel River had to be ceased. A thorough study of the affected zone is being carried out. Riverbed sediments and water samples have been analyzed from four different sampling campaigns in the estuary during the years 1999, 2001, 2002 and 2005. Different radioanalytical techniques have been employed to obtain the activity concentrations of U-isotopes, Th-isotopes, ^^^Ra, ^'"Pb and ^'"Po. Furthermore, the results for the rates of de-contamination of the area are presented. This data will be discussed in order to establish the present status of the contamination in the area, and moreover, to predict the time-evolution of the self-cleanin
