Single Unprovoked Seizure: Wait Time to Full Medical Assessment, Does It Matter?
Introduction Single unprovoked seizures occur in about 4% of the population and they have significant psychosocial consequences for the patients and their families. Little information is available on the timeliness and safety of assessment of first unprovoked seizures. In this study, we review the timeliness of the referral and evaluation of patients with first unprovoked seizure in a Canadian neurological provincial referral center.
Method Retrospective analysis of 51 patients over a 3.5 year period was performed and data were collected on patient demographics, date of event and time to evaluation by the epileptologist, evaluations completed, treatments initiated and patient outcomes.
Results We found that most patients were seen by the epileptologist within 6 months, there was only a 9% discrepancy in final diagnoses between the epileptologist and the referring physician, and there were no fatalities or serious complications in the patients we studied. However, a few patients waited very long periods before imaging and evaluation by the epileptologist, and restrictions on driving privileges were recommended in only 3% of the patients.
Conclusions We conclude that the referral process for a first unprovoked seizure is timely. Primary care providers need further education with regards to the consequences of seizures and some areas of the referral region need better access to imaging and epileptologists
Assessment of a Nutritional Rehabilitation Model in Two Modern Broilers and Their Jungle Fowl Ancestor
Inclusion of rye in poultry diets induces a nutritional deficit that leads to increased bacterial translocation, intestinal viscosity, and decreased bone mineralization. However, the effect of diet on developmental stage or genetic strain is unclear. Therefore, the objective of this chapter is to evaluate the effects of a well-established rye model diet during either the early or the late phase of development on performance, bone mineralization, and morphometric analysis. Furthermore, intestinal integrity evaluated by liver bacterial translocation, leakage of FITC-d, and gene expression of tight junctions across three diverse genetic backgrounds Modern 2015 (Cobb 500) broiler chicken, 1995 Cobb broiler chicken, and the Giant Jungle Fowl are also discussed
Ianus: an Adpative FPGA Computer
Dedicated machines designed for specific computational algorithms can
outperform conventional computers by several orders of magnitude. In this note
we describe {\it Ianus}, a new generation FPGA based machine and its basic
features: hardware integration and wide reprogrammability. Our goal is to build
a machine that can fully exploit the performance potential of new generation
FPGA devices. We also plan a software platform which simplifies its
programming, in order to extend its intended range of application to a wide
class of interesting and computationally demanding problems. The decision to
develop a dedicated processor is a complex one, involving careful assessment of
its performance lead, during its expected lifetime, over traditional computers,
taking into account their performance increase, as predicted by Moore's law. We
discuss this point in detail

Inguinal pain during pregnancy, secondary to round ligament varicosities: case report
The exact incidence of round ligament varicosities (RLV) is uncertain, due to the fact that there are few cases reported in the literature, and in many cases, they can be confused with inguinal hernias. A 26-year-old female with 26.3 weeks of gestation by date of last menstruation, who went to the emergency service of the Naval Regional Hospital of Tampico with the presence of an increase in volume in the left inguinal region, accompanied by pain, which increased standing and decreasing with the dorsal decubitus, the physical examination confirmed an increase in volume at the left inguinal level. An ultrasound of the inguinal region was requested to rule out an inguinal hernia as the first diagnosis, Doppler ultrasound revealed RLV. The patient was treated conservatively and at two weeks after the resolution of the pregnancy, the RLV presented a spontaneous regression and her symptoms completely resolved. This is the first case of round ligament varicosities reported in present institution. It is important to be aware of this pathology, to perform extension studies (Doppler ultrasound) to avoid unnecessary surgery in pregnant women
